Objectives: Thread lifting of the nose is a relatively recent technique: it offers the chance to approach nose morphology defects without the need for surgery in order to obtain their temporary improvement.
Neverthless, it lacks any codification therefore offering variable results and short durability.
The Authors' experience is presented along with a recommended methodological approach in order to deliver reliable techniques for predictable results

Introduction: The principles of surgical approach drive thread lifting techniques when treating nose reshaping: PLLA-PCA threads insertion with proper methods allow to obtain temporary morphological reshaping, relief of functional impairment, alone or even coupled with surgery

Materials / method: The Authors' experience in presented as a case series to support the purposed algoritmic approach, defined in order to codify the different techinques with PLLA-PCA threads for non surgical nose reshaping

Results: The presented algorithm will allows doctors to choose the most indicated technique in order to obtain the desired result. Contraindication and encountered complications are discussed by Authors in order to provide a complete 'state-of-the art' presentation of these techniques

Conclusion: Codification of nose reshaping techniques by PLLA-PCA threads is presented.
In Authors experience this is as a reliable and safe approach to obtain temporary amelioration of nose defect via non surgical, minimally invasive method.

Objectives: To learn the patient selection and dedicated surgical techniques for different patient groups with different wishes, varying from minimally invasive suspension techniques to less invasive tipplasty and simplified rhinoplasty.

Introduction: It is more and more common to see patients seeking modest improvements of their nasal shape, which does not necessarily require all components of rhinoplasty. The author describes a classification method for different nasal problems and proposes alternative less invasive ways to improve the nasal shape.

Materials / method: The nasal shape problems are classified according to the anatomy of the bony nasal structures and tip variations. Shape problems, which do not heavily involve bony structures, are solved with open tip suspensions under sedation. Tip length problems are solved by closed suspensions, achieving the rotation of the nasal tip with a percutaneous purse-string suture applied through small access punctures. This is obtained under local anesthesia with a double-sided needle or shuttle, smoothly anchoring the lower lateral cartilages in a vertical direction to the glabellar periosteum excluding the ski

Results: The techniques are successfully applied under local or sedative anesthesia in selected 296 patients between December 2005 and June 2019 with a follow-up changing from 8 months to 10 years. Outcomes were satisfactory in all but 21 cases, of which 13 found the result inadequate and 3 of them underwent a definitive rhinoplasty operation. Operation times were around 15 minutes in closed suspensions and less than 30 minutes for open suspensions in all patients with an uneventful rapid recovery.

Conclusion: Many patients desire less dramatic changes for their nasal shape complaints. The described classification and related less invasive techniques add an important tool to our toolbox to achieve long-lasting, satisfactory results in selected patients with minimal hump deformity and under-rotated tip.

Objectives: Learn the pros and cons about threading for the nose. Make judgment about to do or not to do nasal threading base on good assessment.

Introduction: It is well known that facial skeletal structures differ between Asians and Caucasians. In Asians, a wider face with flatter facial features, such as a flat nose or a short chin, are quite common. This makes nose reshaping a highly demanded procedure in Asian patients.

Materials / method: While multiple surgical procedures for nose reshaping and augmentation are available, nonsurgical nose reshaping options such as filler injections has seen a surge in popularity in the last 15 years. Nasal threading is another relatively novel technique, with increased popularity in the last 8 to 10 years.

Results: Using threads for nasal reshaping offers its own advantages and disadvantages over fillers. For example, there are increasing reports in literature of vascular occlusion, a possible side effect of filler injection, which may result in devastating sequelae such as filler-induced blindness. A recent publication in 2019 indicates the nasal area as the most high-risk area for blindness in facial injections. Use of threads may greatly reduce or eliminate this risk.
