Biostimulators: how dilution/diffusion interferes with outcome

Objectives: to show through in vivo, in vitro analysis how the dilution of calcium hydroxyapatite can influence the diffusion therefore the stimulation of collagen as a bioestimulator

Introduction: Calcium hydrodylapatite has been in the market as a bioestimulator for at least 4 years but we did not know yet the influence in different uses and dilutions in the clinical outcome

Materials / method: in vitro analysis of fibroblast culture infused with different concentrations of Caha particle
in vivo- skin excised after abdominoplasty injected with different dilution caha and measure the spread depth nd lateral
in vivo skin excised from face lift injected with different dilution of caha and histology using HE was analyzed to count the umber of particles per area

Results: we found that in vitro the number of particles can upper regulate the collagen expression and the more we dilute the less particles we have per are. 1:1 have double than 1:2 and the spread of 1:1 is much smaller concentrating the particles in the area but still offering good coverage

Conclusion: we have to customize the dilution according to patient BMI more than skin thickness cause the aim to reinforce skin envelope (fascia system plus skin ) is queen to body and facial laxity, so the dilution and amount injected should be chosen accordingly

Hydroxyapatitis - Is it so safe? Evidence-based data about different products

Objectives: On this work we will show some new evidence data about collagen stimulation by CaHA, type, duration and stability, cytotoxity of different types of CaHA

Introduction: Hydroxyapatitis is the chemical substance that is used in many fields of medicine, not just aesthetic. So, it’s necessary to understand all the possible ways of interacting with the human tissues.

Materials / method: We have make the meta-analysis of many works dedicated to CaHA in different fields of medicine and compare them with our own experience.

Results: The efficacy and safety of hydroxyapatitis is connected with many data: concentration, particle sizes, place of injection and others.
Changing of each can gives totally different result.

Conclusion: Taking decision about possible ways of using any material in aesthetic medicine is useful to learn data a from other, some times more evident and learned fields.

Cellular biomodulation with injectables

Objectives: The objective is to demonstrate different options for cellular control for many indications using injectables for rejuvenating, aging prevention, bleaching, emptying, regenerating, muscles and glands control and bioestimulus.

Introduction: After an extensive bibliographical review about many indications I performed treatments in more than three hundred patients only with injectables and I realized that it´s possible to control different kinds of cells just using them.

Materials / method: More than three hundred patients were treated only with injectables for various indications such as prevention of aging, wrinkles, pores, oily skin controlled by botulinum toxin, stem cells stimulus using hyaluronic acid, whitening with tranexamic acid and vitamin C, emptiers using substances as deoxycolate and phosphatidyl choline and other actives.

Results: All the patients got satisfied with their cellular control treatments which used only injectables for various indications. In some cases, they underwent the use of two to three substances simultaneously without any side effects.

Conclusion: Biomodulation is an advance for the dermatology because it allows us to have a wider view and understanding of where to act intracellularly to get better results in the aging prevention, rejuvenating and the treatment of many diseases.

Safety and efficacy of polymethylmethacrylate collagen gel (PMMA) for the correction of mid-facial volume loss

Objectives: To determine the safety and efficacy of using PMMA for mid-face volume restoration. As PMMA has been approved for volume restoration of the naso-labial folds and atrophic acne scars in the mid-face, it should also be an excellent filler for long-term volume restoration in the mid-facial area. Additionally there is no long-term filler that has been approved for this indication (there are only hyaluronic acids approved for mid-face treatment). PMMA has not been injected with a blunt micro-cannula which should further improve the safety of the procedure.

Introduction: 25 male and female subjects >30 years of age with a pre-treatment Midface Volume Deficit Scale (MFVDS) grade of 3, 4 or 5. All subjects were injected with PMMA using blunt tipped cannulas in the mid-face area to achieve 100% correction.

Materials / method: Primary endpoint was a two-point Improvement from baseline at 6 months on the MFVDS as measured by 3-D imaging as evaluated by 3 independent observers. Improvement from baseline on the Global Aesthetic Improvement Scale (GAIS) as rated by subjects & observers at 6 months. Assessment of adverse events by subjects and physicians.

Results: A two point improvement from baseline on the MFVDS by 3-D imaging was found in 82% of cases by independent observers. On the GAIS, ratings of improved or much improved were given by 88% of observers and 90% of subjects. Only minor adverse events of mild bruising and swelling were reported which resolved within a week with no sequela.

Conclusion: PMMA microspheres were safe and effective for the correction of mid-face volume loss when injected with blunt tipped cannulas.
