Treating subcutaneous fat and cellulite using combination of non-invasive energy-based devices

Introduction: Always treat from the inside (deep) going out (superficial). For the sequence, remove fat first then tone the area by firming. Anti-cellulite treatment follows and the entire aesthetic experience is finished with body figure shaping. Doing it the other way around is not only ineffective, but even counterproductive. If you remove cellulite without removing the underlying fat first, cellulite will just redevelop when fat redeposit in the dermis. If you tone the area without removing the underlying fat first, the toned area will just sag again due to the weight and volume of the underlying fat.

Materials / method: In order to remove cellulites, you already need non-invasive aesthetic machines that employ the different properties of: skin curving (production of skin fold using positive and negative pressure by way of rollers/ball bearings & suction), ultrasound (to loosen solidified fat with vacuum for lymphatic drainage), light (red and infrared for fat leakage & fat breakdown) and radio frequency (Unipolar, Monopolar, Bipolar, Tripolar, Multipolar) w/ or w/o an Magnetic Pulse (for Subdermal Heating to produce Collagen Contraction, Neocollagenesis, and Neovascularization). But the sequence is important.

Results: Initially do Non-surgical Liposuction using Ultrasound w/ Vacuum coupled w/ Activity & a Sensible Diet. Otherwise, fat will just be re-deposited. This is followed by machines w/ Body Shaping capabilities like Deep RF, better if w/ Magnetic Pulses. Then if cellulite is present, treat it w/ electro-optical synergy of Bipolar RF w/ Infra Red plus Skin Curving. Loose skin will develop when you lose fat volume so tighten it w/ superficial RF. Finally for bulges & fatty protuberances left behind after doing all of these is the ideal target for Ablative Ultrasound and/or Cryolipolysis per area.

Conclusion: The idea is if a patient comes in like a “log of wood” that is straight, bulky, & rough; the Aesthetician should be able to make them curvaceous, slim, and smooth. Initially, you promote General Weight Loss thru Diet & Exercise. Then, you treat problem areas for Specific Inch Loss. The final touches are Skin Tightening, Muscle Toning, & Anti-Cellulite for Smoothening. Spot Reduction is done for remaining problem areas. The best is actually a combination of machine treatments for synergy of their individual strengths. But the sequence should be correct and the parameters suited.

Body shaping via establishing hormonal balance and how to achieve it

Objectives: Previous studies have demonstrated a hormonal increase after effortless exercise, a London University invented motor nerve artificial intelligence method that offers 8 seconds full body contractions simulating strenuous workouts on gym machines. The technology was originally designed and built in London University by the co-inventor of the Pacemaker Gerald Pollock and was upgraded over a period of 40 years empirical research.

Introduction: In our current study 8 subjects received Treatments with the most recent invention of this technology the Virtual Gym Unique II. Treatments were offered twice weekly for 30 to 45 minutes for a total of three weeks. Blood tests were taken immediately before and after the 6 treatments. The sample consisted of 4 males and 4 female subjects, ages 27-45 years. All subjects were of Chinese descent. BMI, visceral fat, subcutaneous fat and muscle fat varied from subject to subject. All subjects were given 6 Virtual Gym Unique II treatments, 30-45 minutes per treatment twice weekly for 3 weeks.

Materials / method: Results revealed a very high statistically significant decrease in VLDL (p<0.001), the bad cholesterol that carries triglycerides that store energy in cells that is utilized for muscle building and the overall body functioning. Additionally, they revealed a statistically increase in Free T-3 (p<0.05) suggesting an increased metabolic rate. Cortisol levels remained relatively unchanged. HDL, the good cholesterol necessary to avoid cardiovascular disease almost reached statistical significance (p=0.25 / 75% of individuals show an HDL increase).

Results: In previous studies where 19 subjects were evaluated after 12 treatments, we found a significant increase of HDL, therefore our current result may simply be an artifact of our small sample and relatively inadequate number of treatments. The same explanation holds for IGF-1 increase that was well above chance (p=0.29 / 71% of individuals show an IGF-1 increase)

Conclusion: DHEA increase that was also above chance (p=0.37 / 63% of the individuals get DHEA increase ). None of these variables reached statistical significance. Testosterone for men was statistically insignificant. However, the testosterone for women almost reached significance with a probability level p=0.16 /84% of female subjects get testosterone increase). All hormones remained within the normal range suggesting that the technology increases metabolism and burns fat to produce energy as suggested by the significant VLDL decrease, without disturbing the body’s hormonal balance.

HIFEM - The European study

Objectives: High intense focussed electromagnetic waves, HIFEM, are well known in medicine and used to contract muscle fibers to induce muscle growth and reduce fat cells. The European Study was designed to specify the amount of muscle growth and fat layer reduction and to have a closer look at other indications like rectus muscle diasthasis.

Introduction: US studies have identified the potential of HIFEM for muscle growth and fat layer reduction.

Materials / method: A group of female and male patients were treated with HIFEM four times, 1-2days apart, and were told to maintain an protein uptake of 1gr/kgBW per day and otherwise keep their daily routine and diet. Muscle thickness and fat layers were measured by ultrasound before and up to 3mths after the treatments.
Histologies were taken to analyze the reason for fat layer reduction.
In another group of females with rectus muscle diasthasis the same procedures and measurements were applied.

Results: In the first group significant fat layer reduction of fat layers, up to 18% were measured, muscle growth was significant, up to 20%.
Histologies showed signs of fat cell apoptosis.
In the second group clear reduction of rectus muscle diasthasis up to 50% was seen.

Conclusion: The European Study could clearly prove, that HIFEM is not only a great tool for nonsurgical body enhancement such as fat reduction and muscle growth. HIFEM is also a nonsurgical therapy option for rectus muscle diasthasis.

Biochemical investigation of fat disruption effect following HIFEM procedure: veterinary study

Objectives: The goal of this study is to follow-up on findings of previous single-study histological evidence and to investigate further the proposed mechanism of apoptosis induction.

Introduction: Several studies showed that patients undergoing HIFEM procedure, based on muscle stimulation, experience beside muscle mass gains also reduction in the subcutaneous fat. Yet, the underlying mechanism behind the HIFEM effect on fat is still not fully understood. The goal of this study is to follow-up on findings of previous single-study histological evidence and to investigate further the proposed mechanism of apoptosis induction.

Materials / method: Two Large White pigs were treated with HIFEM based device for 30 minutes on the thigh. Biopsy specimens of subcutaneous fat tissue were collected from the area of application and from the control site (abdomen) before, immediately after and 8 hours post-treatment. The levels of pro-apoptotic DNA markers (BAX, BCL-2, TXNIP, MMP9, TNFα) and the levels of free-fatty acids (FFA) were measured in the collected samples. In addition, pH measurements of the fat tissue were performed at the biopsy site.

Results: Quick lipid disintegration was seen through the levels of FFA that increased by 127.1% immediately after the treatment and by 134.1% eight hours post-treatment. Immediate increase in FFA levels was accompanied by a drop in pH that dropped from 7.30±0.12 before the treatment, to 6.60±0.07 after the treatment. Eight hours post-treatment the pH was nearly returned to the pre-treatment level; to 7.19±0.12. The expression of BAX, TXNIP, MMP9 and TNFα was increased after the treatment, with the highest changed seen for the TNFα; the expression of BCL-2 was attenuated by 26%. The control samples did

Conclusion: Rapid increase in FFA levels accompanied with an increase in fat tissue acidity is compatible with the proposed mechanism of fat apoptosis based on an fat cell overflow by free-fatty acids, which trigger processes leading to apoptosis. Increased apoptosis is indicated through the changes in expression of DNA markers. Additional studies are required to thoroughly explore the potential of non-thermal induction of fat cells apoptosis.

My personal technique of gluteal augmentation with HA gel injection and tissue stabilized-guided subcision (TS-GS) system

Objectives: Assess the right indication for non surgical gluteal Augmentation and cellulite treatment.
Validate my personal techniche of combined gluteal augmentation with hyaluronic acid gel injection and tissue stabilized-guided subcision (TS-GS) system.
Validate the cellulite reduction with a new FDA-cleared tissue stabilized guided subcision (TS-GS) system.

Introduction: For improving the appearance of gluteus, in my busy centers for cosmetic surgery in Italy, I have introduced a protocol of volume increasing with ialuronic acid and cellulite reduction with a new FDA-cleared tissue stabilized-guided subcision (TS-GS) system.

Materials / method: I enrolled only women with stabilized cellulites since at least 5 years and volume reduction. I analyzed patients by digital 2D macro-photographs and by a 3D biomechanical device. Patient satisfaction was assessed through physician review of 2D and 3D photography, and the Global Assessment of Improvement Scale (GAIS) at baseline, 3 and 12 month post-treatment.

Results: Sixty women of average age 39.6 years with a total of 1124 lesions of cellulite received a single treatment to the buttocks and/or posterolateral thighs with the study device and 100 ml of hyaluronic acid. All completed the follow up at 24 months. 96.8% of patients were satisfied with treatment results and patients displayed a 1-point improvement in their cellulite grades. Moreover, GAIS scores indicated that all patients had visible improvement in the global appearance of their cellulite. Analysis of 3D imaging yielded 87.6% average improvement in minimum height of dimples.

Conclusion: The physician ratings, patient satisfaction, and photographic evidence prove the safety and efficacy of a protocol of volume increasing with hialuronic acid and cellulite reduction with a tissue stabilized-guided subcision system.

Gluteal augmentation with polymethylmethacrylate (PMMA): a 12-year follow up

Objectives: Describe the author's experience in over 3,000 treatments performed with polymethyl methacrylate on gluteal volume in 12 years of follow-up

Introduction: Many techniques for buttocks augmentation have been developed and published, for a more natural, satisfactory, and safe result for the patient. It has been a challenge to find a technique that presented not only volume gain but a natural result and skin quality improvement.

Materials / method: More than 3,000 patients underwent a procedure to butt augmentation with polymethyl methacrylate at 12-year follow-up. A retrospective study was performed showing side effects and correlating with already established techniques such as prosthesis and liposuction.

Results: The authors observed only 1.88% side effects. The aesthetic result is very satisfactory and natural, allowing the procedure to be performed many times with the patient standing accompanying the procedure. It was possible to verify not only the muscle volumetric gain but also the collagen biostimulation provided by the particulate products contributing to the improvement of skin quality and sagging.

Conclusion: This study has demonstrated that gluteal augmentation with PMMA is one of the best options for this type of procedure.This study demonstrated not only that polymethyl methacrylate is a safer procedure than techniques with the same purpose (prosthesis and liposculpture), but a procedure performed with local anesthesia, in the office with natural result and very satisfactory for the medical team and especially for the patient.

Gluteal projection and augmentation using polycaprolactone-based filler: a promising new indication

Objectives: Describe a new promising indication for polycaprolactone based filler
To achieve nonsurgical buttock and gluteal enhancement by injecting with a fanning technique 2 ml of Polycaprolactone based filler.
To describe de differences between usging Polycaprolactone and other Soft tissue fillers for buttock augmentation.

Introduction: Minimally invasive treatments to achieve the perfect buttock have been requested since decades ago. But with multiple complications such as nodule formation or granulomas that took place with the use of silicone and other non-absorbable products, it became a surgical only answer. However, with all the minimally invasive procedures on the rise, a new field for minimally invasive body shaping emerges from its latent state. In the quest to find a better option to treat this specific area, the idea of using polycaprolactone to enhance the gluteal area emerged.

Materials / method: We present the case of a 26-year-old woman who was searching for a nonsurgical option to give projection and volume in the gluteal area.
Two centrifugal vectors were marked, the first one starting from the upper transgluteal line, and the second one from the upper external gluteal zone, ensuring some of the lines from both vectors could cross paths. After thorough disinfection using 1% clorhexidine, we proceded to apply two 1 ml syringes per side following the markings using a 22Gx60mm cannula.

Results: The immediate results were evaluated right after the treatment was completed demonstrating an outstanding enhancement, both in volume and shape. In a subjective satisfaction test applied to the patient, she referred a 10 out of 10 post-treatment score.
Three months after the procedure, a review appointment was performed. The Patient referred in the same subjective satisfaction test previously applied an overall 9 out of 10.
Photos showed a slight volume loss, compared to the previous photos. Yet an outstanding result compared with the baseline picture remained.

Conclusion: In conclusion, using polycaprolactone could become a new alternative to nonsurgical buttock enhancement. The main difference is that there is no need to use a big amount of product to achieve immediate and long-lasting results. And because of the collagen biostimulation property, the treatment will also enhance the skin quality, giving the buttocks an overall and satisfactory improvement.
