Antihematomic preparation for injection procedures: how to minimize the risk of complications after traumatic procedures

Objectives: The aim of this study was to develop recommendations that shorten the recovery period after invasive procedures and reduce the risk of post-traumatic infection of soft tissues.

Introduction: The rhythm of modern life dictates the conditions under which the employee cannot fall out of the schedule for a long time. In this regard, there is an acute question of reducing the time for rehabilitation and prevention of complications after injection anti-aging procedures.

Materials / method: The study involved 20 volunteers who were shown contouring and thread lifting procedures associated with a high risk of hematomas in the post-treatment period. They received anti-hematoma therapy, including taking anti-hematoma drugs inside, as well as using external forms 5 days before the procedure, on the day of the procedure, and 2 days after the procedure. The control group consisted of 20 patients receiving the same procedures, but without anti-hematoma therapy.

Results: When comparing the course of the rehabilitation period, it was found that after anti-hematoma prophylaxis, the rehabilitation period was reduced by an average of 3 to 4 days, the size of the hematomas was much smaller and faster than in the control group.

Conclusion: Accordingly, it can be concluded that the anti-hematoma therapy has a beneficial effect on the post-treatment period, and reduces the risk of post-traumatic infection and other complications.

Stem cell-derived exosome and skin barrier

Objectives: 1. To know that stem cell-derived exosomes can be used as next generation regenerative aesthetics and therapeutics
2. To know that stem cell-derived exosomes can reconstruct skin barrier functions and be a new therapeutic candidate for atopic dermatitis (AD)
3. To know that stem cell-derived exosomes are safe for skin

Introduction: Exosomes, nano-sized extracellular vesicles, are the most important mediator for intercellular communication. These days, stem cell-derived exosomes are being developed as cosmetics/cosmeceuticals and therapeutics as well. It was found that stem cell-derived exosomes can be very effective for skin rejuvenation/regeneration and atopic dermatitis through reconstitution of skin barrier functions. It was also found that stem cell-derived exosomes showed safe toxicological profiles.

Materials / method: Human adipose tissue-derived mesenchymal stem cell-derived exosomes (ASC-Exosomes) were used for in vitro, ex-vivo, and in vivo studies.

Results: ASC-Exosomes:
1. contain active cargo: >1,000 proteins & ~600 miRNAs. Many of them are functional for skin;
2. effective for skin rejuvenation: increased collagen & elastin synthesis;
3. effective for atopic dermatitis in an inflammatory model: reduced clinical score, IgE level, cytokine mRNAs, & inflammatory cells;
4. effective for atopic dermatitis in a skin barrier model: reduced TEWL, epithermal thickness, & cytokines; increased the lipids; normalized gene expression for skin barrier, lipid metabolism, cell cycle, & immune response; &
5. safe for skin: no toxicity in toxicology evaluation.

Conclusion: 1. ASC-exosomes can be a novel anti-aging product for next generation regenerative aesthetics, which is superior to conventional skin boosters.
2. ASC-exosomes can be a novel promising cell-free therapeutic modality for AD treatment.
3. ASC-exosomes can be applied on skin with no significant toxicity.

Interest of tannic acid (TA) as a new cosmetic ingredient in anti-aging

Objectives: Tannic Acid (TA) has the following properties for treating:
Bruises and redness
Anti pigmentation and redness
Antipollution
Anti aging
Free radical protection and scavengers
Vasoprotection
Regulation of pigmentation
Antipollution agent

Introduction: TA presents a clear innovation as cosmetic ingredient for anti ageing products and dermatology. Because of the difficulties to formulate TA and its poor stability it was never commercialised before because TA is completely instable and present great difficulties to formulate.

Materials / method: TA is coming form plants and is concentrated in Sequoia tress bark. The extract containing the purified molecule is a light brown powder purified at 90 %
The molecule posses a very great number of hydroxyl groups which brings its intense original polyphenol activity.

Results: Studies will show the results of the activity of TA works as, iron ions chelator, strong antioxidant, H2O2 scavenger, superoxide anion radical scavenging , anti glycation and it presents also effects on the endothelium of blood vessels and some application in post cancer treatment side effects.

Conclusion: As conclusion we can assume the following:

Impact of carcinine supplementation on skin quality and correlation with metabolic parameter in metabolic syndrome

Objectives: Objective: we hypothesized that carcinine would improve skin quality, glycemic control, insulin resistance and lipid profile in overweight and obese patients.

Introduction: Introduction: carcinine is a dipeptide found in mammalian tissues (SONG et al., 2014). Studies have demonstrated its anti-inflammatory, anti-glycant and antioxidant activity by different mechanisms (Hinkley et al., 2007) and it could be used in the treatment of metabolic syndrome (MS) (Anderson et al., 2018). On the other hand, aging process shares aspects of MS pathophysiology and could be treated with carcinine (Babizhayev 2011), although there is no study evaluating both skin and MS simutaneously.

Materials / method: Materials and Methods: female patients, aged between 40 and 60 years, body mass index between 25 and 34.9 (kg / m2) were eligible to participate in a randomized, double-blind, placebo-controlled clinical trial. 38 patients were recruited and assigned to intervention group (n = 19, receiving 1 capsule of 20 mg of carcinine daily) or control (n = 19). Photographs were taken and samples of blood and anthropometry were collected at baseline, after 45 and 90 days. A photo analysis software was used to evaluate wrinkles, erythema and pigmentation. Results: Results: after 90 days, carcinine supplementation resulted in a significant decrease in wrinkles, spots, and texture (p<0.005), paralled by reduction of fasting glycemia (p = 0.001), fructosemia (p = <0.001) glycated hemoglobin (p = 0.003), cholesterol (p = 0.004), insulin (p = 0.038), abdominal circumference (p = 0.013) and HOMA-IR (p = 0.039).

Conclusion: Conclusions: the parameters investigated showed a significant improvement after 90 days of supplementation. Additionally, beyond its own results, this study reinforces the need to treat the patient as a whole, since improving their clinical conditions (e.g., MS) has a positive impact on all tissues, including skin.

Do we need the lasers for melasma removal?

Objectives: Melasma is not only hyperpigmentation disorder. VEGF is increased at the places of the hyperpigmentation. If we want successfully remove melasma we need to destroy capillaries.

Introduction: Melasma is a very common aesthetic problem which is sometimes very challenging not only for the doctor but also for the client. There are many causes of melasma. Besides UV rays, the visible light also plays a very important role with the mast cells, keratinocytes, suprarenal hormones and VEGF.

Materials / method: Until recently the laser was the only solution for that. Today pico lasers represent one of the most effective treatments for the melasma removal. There are many effective ingredients to remove melasma. We need to use the ingredients which affect the capillaries like tranexamic acid.

Results: We want to represent such cream which represents a big breakthrough for the melasma removal because it gives very often the result which is comparable or better than a laser.

Conclusion: The main limit of the previous cream for the melasma removal was that they did not affect the capillaries. Thanks to this creams the lasers don t play any more the main role for the melasma removal.

Clinical efficacy of a topical serum containing 15% L-Ascorbic acid, neohesperidin, pycnogenol, tocopherol and HA on the skin aging signs

Objectives: Evaluating the efficacy of a topical potent antioxidant serum containing 15% L-Ascorbic Acid, Neohesperidin, Pycnogenol, tocopherol and Hyaluronic Acid in skin ageing signs of Brazilian subjects with clinical, instrumental and skin imaging assessment techniques

Introduction: Skin Aging is a process linked to chronological aging and exposome such aschronic UV exposure and pollution which both induce, among other effect, reactive oxygen species. Therefore, topical formulations which have strong antioxidant effect could counteract this process. The current formulation combines strong antioxidant ingredients such as 15% of L-Ascorbic Acid, Neohesperidin, Pycnogenol, tocopherol with fragmented Hyaluronic Acid to improve the extracellular matrix.

Materials / method: 40 female subjects 40 to 65 years old, with phototype II to IV, presenting wrinkles (frontal, glabellar and naslolabial folds) score ≥ 2 to 5 (from Skin Aging Atlas), and facial dark spots (excluding ephelides and melasma) and/or lentigo were included in the study. All of them were evaluated for clinical efficacy and safety. 20 of them underwent instrumental (Primos, Visia CR, Cutometer, Chroma Meter) and 16 skin imaging (Confocal). The evaluation were performed after 45 and 90 days of product use. The confidence interval considered in the comparative analysis between time-points was of 95%.

Results: A statistically significant improvement on skin ageing signs were observed at day 90: frontal, nasolabial and glabellar wrinkles improved by 23.9%, 22.7% and 19.1%. Similar results were observed for skin tone homogeneity, dark spots color intensity and hyperpigmentation which were confirmed by VISIA. Instrumental measures at day 90 demonstrated improvements of skin texture, depth and volume of wrinkles; skin firmness and elasticity; skin homogeneity; in thickness of the epidermis and of the stratum corneum; and the morphology of the dermis and stratum corneum. No adverse events were observed.

Conclusion: The topical potent antioxidant serum containing 15% L-Ascorbic Acid, Neohesperidin, Pycnogenol, tocopherol and Hyaluronic Acid demonstrated a clinically relevant efficacy on skin ageing conditions, with a good safety profile after 90 days of daily use.
