Objectives: To understand the safety and efficacy of photodynamic therapy in the treatment of acne vulgaris

Introduction: Photodynamic therapy (PDT) in acne is widely recognised as a safe and effective method for treating both inflammatory and non-inflammatory lesions and different severity of acne from mild to severe.
In my presentation, beside a brief review of the literature on the application of PDT in the treatment of acne, I will show the promising results of an assessor blinded proof-of-concept trial showing the safety and efficacy of a novel formulation of 5-aminolevulinic acid 5% thermosetting gel in the treatment of acne.

Materials / method: 35 patients (13 male, 22 female), were treated using PDT with 5-aminolevulinic acid 5% thermosetting gel for a mean of 3 PDT sessions targeted to the face and held every other week (range 2-6 sessions). Global Acne Grading System (GAGS) score was recorded at the baseline and after each PDT session and at a 6 months follow up. A skin tolerability score (0-3 range) was reported by each patient after each PDT session.

Results: From a mean baseline GAGS score of 21 (SD +/-6), the GAGS score decreased to a mean value of 6.5 (SD +/- 5.7) after the last PDT session and mantained itself at a mean value of 6.7 (SD +/- 6.8) at a 6 months follow-up. Tolerability assessed by the patients was excellent, with all patients completing the study and with a reported discomfort mean level score of 0.5 (SD +/-1) on a scale from 0 to 3 (where 0 is no discomfort at all and 3 is the worst possible discomfort). No adverse events were reported.

Conclusion: PDT using 5-aminolevulinic 5% thermosetting gel is safe and effective for the tratment of mild to moderate acne vulgaris. The small size of the study is a limting factor. The fact that GAGS calculation included the chest and back, that were not treated during the study (only the face was treated), raises the possibility that the results could have been even better including chest and back in the treatment protocol.

Objectives: To obtain knowledge and techniques how to modulate Photodynamic therapy for optimize outcome or local skin reactions for different skin conditions such as actinic keratoses, photorejuvenation and other skin conditions.

Introduction: Photodynamic therapy (PDT) is an effective treatment modality for actinic keratosis (AK), selected cases of basal cell carcinomas and photorejuvenation.
Major adverse effects include pain during illumination, local skin reactions post-treatment and lower effect on thick keratotic lesions.

Materials / method: To enhance the efficacy of PDT for difficult-to-treat AK, PDT has been combined with physical pre-treatments to increase photosensitizer delivery. Furthermore, modulation of local skin responses after PDT has been attempted with topical treatments, low level light therapy and short term photosensitise application.

Results: In the session, own experience with pretreatments and particularly ablative fractional laser resurfacing to increase PDT efficacy will be presented. Methods to alleviate post-treatment inflammation will be reviewed.

Objectives: Clinical experience and modulations of the technique according to therapeutic objectives: conventional PDT, Day Light PDT, Light Emitting Fabrics, PDT with Artifical White Light and laser pre-treatments

Introduction: We will discuss through clinical experience different situations and the strategy adopted according to the objective to be achieved with PDT.

Materials / method: There are now many ways to practice PDT: pre-treatments, conventional PDT, Day Light PDT, Light Emitting Fabrics, PDT with Artifical White Light. The PDT indications are expanding from year to year. There are also several types of photosensitizers available and the use of fluorometry becomes easier.

Conclusion: From medical indications to acne and pure aesthetic management, we will see how to adapt the different tools at our disposal to minimize side effects and optimize the result.

Objectives: Photodynamic therapy (PDT) using daylight as the photoactivating light source (DL-PDT) is an effective treatment for actinic keratosis (AK). Unfortunately, DL-PDT can be carried out only in suitable temperature and weather conditions. Artificial daylight sources have therefore been proposed as alternatives to daylight with the aim of allowing treatment to be carried out year round in all weather conditions.

Introduction: This study aims to report the clinical outcomes of a new and innovative White LED device (Dermaris, Surgiris, Croix, France) in patients treated for AK lesions of the scalp in a private medical dermatology centre.

Materials / method: Thirty-eight patients with grade I–II AK of the scalp were treated with SDL-PDT using the Dermaris with a drug-light interval (DLI) of 0 minutes and a photoactivation time of 2.5 hours. The primary endpoint was the rate of patients with less than five AK lesions at three months post-treatment. Secondary endpoints included scores of pain, crusts, discomfort and pruritus during or/and post the treatment.

Results: Three months following treatment, the rate of patients with less than five AK lesions was 68%. The median pain score was 0 out of 10 (IQR: 0-0). Crusts, discomfort and pruritus were rated as mild or less in more than 87% of patients.

Conclusion: Artificial daylight -PDT using the Dermaris as administered in this study is an effective and nearly painless treatment with minimal side effects for patients with AK lesions of the scalp.
