Objectives: The origins of ESCAD
For many years in Europe cosmetic dermatology was not taught as a subject in its own right. Dermatologists only learnt the subject by apprenticeship and by attending congresses. In the last 50 years, life expectancy has increased considerably and people have become much more concerned with their appearance. Dermatologists are the best trained people to meet these new needs.

Introduction: In 1997, three dermatologists and longstanding friends, Pierre André from Paris, Leonardo Marini from Trieste and Christopher Rowland Payne from London, felt there was a need for a scientific Society which would gather together research scientists from the fields of cosmetics and skin ageing together with clinicians specialized in the therapeutic techniques of cosmetic dermatology.

The idea of the European Society for Cosmetic and Aesthetic Dermatology (ESCAD) was born.

Materials / method: The New ESCAD
ESCAD is a democratic, non-commercial, non-profitmaking medical scientific Learned Society, registered in Paris. It consists in a President and an Executive Board.

In order to meet the present challenges and to support the need of scientific knowledge of its 6,000 members, ESCAD was refunded in Copenhagen in October 2015 and gave birth to New ESCAD. The Society is at the service of its members and of the industry to collaborate and contribute to the well-being of patients all over the world.

Results: The most important scientific event promoted by ESCAD is the Annual Scientific Meeting that takes place on the occasion of the European Academy of Dermatology and Venereology (EADV) Congress. This mutual collaboration strongly stresses the synergy between ESCAD and EADV. As a matter of fact over the last two years, ESCAD Board members have been actively involved in the scientific organization of the EADV Congress. Many ESCAD members are part of the EADV Board.

Conclusion: An international Scientific Committee, which includes members from Europe and other continents, supports ESCAD: during these years, the group has been raising up a strong network of cooperation with other international and national cosmetic dermatological societies.
ESCAD aims at fostering the highest standards of patient care in cosmetic and aesthetic dermatology, at facilitating continuing professional development and offers a friendly forum for the sharing of scientific research and innovative techniques.

Objectives: To learn about the criteria of providing a good quality service in Aesthetic Dermatology

Introduction: Every practitioner working in the aesthetic field is exerting the best efforts to learn the best techniques to provide the best quality service however the best outcome of the aesthetic service does not depend only on a good technique but also on a good patient selection and making sure that the most appropriate technique is selected for the patient. Learning the patients motive, the expected outcome and evaluating the psychological status of the patient play a vital role in the final outcome and patient satisfaction

Materials / method: Patient psychology plays a crucial impact on his/her self esteem and demands and this is reflected on the demands and satisfaction of any procedure he/she might undergo.

Results: Keeping that in mind, it is mandatory for the treating physician to be able to evaluate the psychological status of the patient before discussing any cosmetic treatment modality and starting to implement it.

Conclusion: In Conclusion, It is important to:
Understanding the Patient psychology
Understanding the patient needs
Using the best available procedure for the patient’s condition/problem
Trying to achieve the desired results with the most natural result possible
The goal should be rejuvenation and not a temporary fix or permanent damage

Objectives: To relate some of the many ideas on injectable tretments that I picked up during the 25 years I am practicing cosmetic dermatology

Introduction: I have had my own private clinic in cosmetic dermatology since 1995. Almost from the start I have trained young doctors in order to become part of my medical staff.

Conclusion: The lessons I learned are related to fillers, toxins, complications and how to treat male patients.

Objectives: Botulinum (BTX) is are very effective drug against hyperhidrosis. You learn in this overview presentation not only about the history of the drug but also about the diferent indications and treatment modalities of focal hyperhidrosis: Axillary, palmar and plantar, head and chest. Many tricks of better results when treating patient with excessive sweating are given as well as the unique anaesthesia form for palmoplantar hyperhidrosis after Kreyden.

Introduction: BTX is very effectful in the treatment for focal hyperhidrosis, a skin disease with a high impact of life quality. However it is important to perform the injections correctly in order to create good clinical response. This includes not only the injections themselves but also a correct reconstitution of the drug and a perfect handling including correct materials. Even if BTX in the indiaction of hyperhidrosis has be neglected in the last year due to the immense success of aesthetic indication in the face, BTX still is the star when it comes to treating patient with focal hyperhidrosis.

Materials / method: This presentation will cover a short historical insight of developing the drug from the poison and will give you many practical tips in the daily use of BTX when treating patients not only for axillary hyperhidrosis but also sweating of the head, the hands and feet or the chest. A unique anasthesia technique after Kreyden will be shown for palmoplantar hyperhidrosis

Results: Even forgotten a little bit in the last years, BTX is a excellent drug to treat patients with focal hyperhidrosis. Most physicians are not awre that BTX can be used in the same efficacy for sweating of the front, neck or inter- and submammary as well. With the special anaesthesia technique for palmoplantar treatment no hand block is needed.

Conclusion: This presentation is an overview of treating focal hyperhidrosis with BTX from history over preparing the drug including materials. The different indications of focal hyperhidrosis are discussed and the special anaesthesia technique after Kreyden for palmoplantar hyperhidrosis is presented.

Objectives: Office-based Dermatological Surgery is an important part of modern Dermatology. It can give great satisfaction to both Patients and Physicians when all details are properly considered. Local anaesthesia plays a major role to create a trustworthy professional environment.

Introduction: Besides the obvious evaluation of post-op short and long-term clinical results, Patients tend to remember very vividly the initial steps of their surgical experience. These steps are extremely important due to the higher levels of psychological stress felt immediately before potentially painful surgical procedures.

Materials / method: In spite of the most accurate description of what controlled painful sensation could be when local anaesthesia is administered, Patient attention peaks immediately before the first needle injection as documented by pulse oximeter monitored heart rates. Five tips and tricks can help to achieve a smooth local anaesthesia.

1. Patient comfortable position on surgical chair
2. Anti-stress balls – soft music – “soothing” talking
3. Super-sharp small bore needles + super-rapid trans-cutan/mucosal needle insertion
4. Slow, low pressure subcutan. injection technique
5. Post-injection gentle massage

Results: Complex procedures might require longer lasting local anaesthesia. Double anaesthetic techniques can be sequentially performed starting with strategically selected nerve blocks subsequently followed by local infiltrations. A bloodless surgical field is the dream of all surgeons. Local anaesthesia can help to reach a nice level of capillary bleeding control when enough time is given to adrenalin to achieve a temporary vasoconstriction.

Conclusion: Smooth local anaesthesia is an art that needs to be mastered and implemented to help Patients reducing their anxiety and potential complications. Surgical experiences will be remembered in a much better way when pain control is achieved through nicely performed local anaesthesia techniques.

Objectives: The cosmetic use of Botulinum Toxin-A continues to increase since its approval some years ago. Despite the fact that more than 80% of patients using Botulinum Toxin-A are satisfied with the results, there still remains the 20% of patients who express dissatisfaction with the final outcome.

Introduction: Among many, some reasons for pitfalls when using Botulinum Toxin-A can be:
(a) Higher or lower dosage (usually the latter), due to hesitation by doctors, especially young and/or inexperienced who are afraid of possible side effects.
(b) Wrong positioning of the injection point (e.g. women who pluck or draw new brow lines can cause confusion of the exact injection point).

Results: (c) Superficial veins, especially around the eyes (crow’s feet area) at times cannot be identified and may in some cases cause bruising.
(d) Existing asymmetries which are not explained to the patient by the doctor prior to using Botulinum Toxin-A may result in patient dissatisfaction afterwards.

Conclusion: Although there are some difficult indications in the use of Botulinum Toxin-A, it can prove to be quite successful if correct methods are adopted.
