Objectives: Over Decades the treatment approach for aesthetic reasons usually has been regeneration, turning back what has been lost during the aging process.

In the Millenial generation we are treating in a prophylactic manner. a total different approach with a lot of responsibility.

Introduction: Today we are facing a young generation, the so called MILLENIALS living in a world of picture taking/selfies with a high focus on a beautiful face with beautiful skin. This generation lives with aesthetic treatments through us doctors as normal as going to the hairdresser.

Materials / method: Us doctors should understand the responsibility we are having for these young generation guiding them with minimal invasive treatments through their young lifes without overcorrecting or treatments with a long term risk or damage.

Results: Topical, skin booster, toxins and superficial peels should be used in this young generation, as well as energy based devices as noninvasive RF or Microfocussed Ultrasound, which can be all used long term without changing the individual personal look over years or even decades

Conclusion: In this generation we are treating young patients with a more prophylactic approach than a regenerative approach. In this lecture different good treatment options will be discussed for this patient group

Introduction: In the last years neuromodulation has been one of the most used therapies in Aesthetic Medicine , in Europe and worldwide.

Materials / method: On the European and American markets, for Aesthetic Medicine, there are three licensed drugs containing botulinum toxin type A: abobotulinumtoxin A distributed by Galderma, incobotulinumtoxin A produced and distributed by Merz and onabotulinumtoxin A produced and distributed by Allergan.

The three toxins come from the same bacterial strain and are all type A but differ significantly according to their power unit, auxiliary proteins and therefore molecular mass, storage conditions and dilution rules.

Objectives: Knowledge of complications makes their recognition easier.

Introduction: In the field of complications in aesthetic medicine the most important role is played by dermal fillers.

Materials / method: Dermal fillers are used for the correction of skin defects or wrinkles. They are medical device, not subjected to the preliminary drug regulation, even if they are not completely inactive substances. Fillers can determine adverse events, related not only to their chemical – physic and biological characteristics, but even to unexpected reactions of the organism guest. The growth of more different dermal filler in the market and of the operators of the sector has determined an increase of the incidence of filler complications.

Results: This has made necessary to constitute a center of excellence that represents a point of reference on the territory for the management of these complications.
In this structure, the patient is clinically examinated, submitted to dermal US (to characterize damage and material). In some cases further investigations (blood test, CT, MR) can be prescribed. The patient is treated with specific therapy (pharmacological, surgical) up to the resolution of the case and submitted to follow up.

Conclusion: The possibility of recognizing and managing adverse events in Aesthetic Medicine makes the choice of treatment safer for the patient.

Objectives: The objective of this presentation is to present the risk of nose injection and how to avoid complications.

Introduction: The indications for medical rhinoplasty have improved last years, because fillers offer a non-surgical alternative for nose augmentation. Several fillers can be used: Hyaluronic acid, Autologous fat, Calcium hydroxyapatite, PolyCaproLactone. They can be used to improve the nose bridge height, nose tip projection, hid a hump or project a columella. But severe complications can occur in non-expert hands.

Materials / method: The risks and the different possible complications during medical rhinoplasty are presented.
A reminder of the nasal vascular anatomy, as well as of the risk zones, allows us to understand the mechanism of these complications and how to avoid them.

Results: To avoid arterial or venous complication, anatomy must be well known.
Never inject too much product to avoid excessive tension of the tissues, especially the tip of the nose, which could cause hypoxia or necrosis.
Never inject under too much pressure.
Avoid risk areas: columellar arteries, arteries and lateral nasal veins.
The injection on the dorsum of the nose must be done in depth in contact with the periosteum, being at a distance from the nasal dorsal artery.

Conclusion: Respect for precise rules and knowledge of the vascular anatomy of the nose are fundamental to avoid the occurrence of complications that are difficult to manage.
