Objectives: We want to show how fat grafting give a révolution in breast reconstruction

Introduction: We perform more than 1000 injections during 7 years , with very good results for breast reconstruction with or without implant.

Materials / method: 1200 fat grafting in more than 600 women for all indication : before or after implant reconstruction, for exclusive reconstruction with fat, for esthetic sequallae ………

Results: The results are very good, very stable and with a very low rate OF complications …
We have 120 patientes with immediate or delayed total reconstruction with fat grafting and more than 500 womens who get or or two injections to get a better result OF the breast reconstruction.

Conclusion: Fat grafting is a good alternative OF flaps for total breast reconstruction without implant and a good technique to get better results with implant breast reconstruction

Objectives: We present our long-term follow-up of radiated breast cancer patients who underwent LipoBed protocol. This consist of prior 2-3 fat grafting session with regenerative and reshaping purposes when needed, in order to downgrade the radiation damage, before to proceed with definitive implant placement.

Introduction: Breast reconstruction in radiated patients represents a challenge to the plastic surgeon when autologous tissue reconstruction is not feasible or refused by the patient, as implant placement is often fraught with significant complications and morbidity. In 2012 we publish an alternative reconstructive approach for this subset of patient, that we called Lipobed with a very promising preliminary evidence.

Materials / method: The authors retrospectively reviewed 60 cases of irradiated breasts treated with fat grafting and subsequent alloplastic reconstruction/breast augmentation with at least 5 years follow-up. The evaluation methods were clinical and photography-based assessments. Comparative histological assessment of implant capsule in radiated cases treated with Lipobed and not-radiated ones was also performed. The BREAST-Q was used to quantify patient satisfaction.

Results: The reconstructive outcomes were excellent to good in 93.7% of patients. Patient satisfaction was marked high, to very high. Comparative histology analysis included capsular thickness, collagen fiber alignement, α-SMA, ER-α and ER-β. We registered a Baker grade 1 capsule contracture in all patients.

Conclusion: The authors’ experience shows that Lipobed represents a valid reconstructive option in selected patients with irradiated breasts with good-long term follow-up. Fat grafting seems to reduce the radiation-induced complications in implants.

Objectives: Fat transfer and use of mechanically processed adipose derived stem cells in the breast - angel gift or devilish act?



Participants will learn about the biologic functions of fatty tissue and the role of adipose derived stem cells ASCs in the healthy breast as well as the oncological context when we use this surgical technique for both aesthetic and reconstructive indications.

Introduction: Since the revival of fat grafting by Sid Coleman and the discovery of adipose derived stem cells there has been considerable debate about the use of either of those techniques in aesthetic and reconstructive breast surgery due to its potential oncological implications.

Materials / method: The author will outline the physiology of fat in the healthy breast - specifically during pregnancy and lactation, define standard methods for fat harvesting and grafting to the breast as well as
describing mechanical processing of fat in order to obtain SVF and ASCs without the use of enzymatic digestion.

Results: Recent research shows not only the safety of fat grafting to the breast in aesthetics and reconstruction but also the oncological safety and superior results of the use of ASC enriched fat grafting in the reconstructive setting. This is the first long term follow-up of breast reconstruction patients after the use of ASCs in the reconstructed breast and together with mechanical processing this represents a milestone in making fat grafting more reliable and predictable.

Conclusion: Adipose tissue and adipose derived stem cells ASCs are natural components of the breast. Both cell types are activated repeatedly during pregnancy and lactation and interact in a positive fashion with the glandular tissue.
Today fat grafting but also ASC enriched fat grafting should be considered a viable option for breast surgery.
