Objectives: (1) To understand the mechanism of action of minoxidil in the treatment of androgenetic alopecia
(2) To understand the role of SULT1A1 in minoxidil activation
(3) To understand how to up-regulate SULT1A1 in-vivo

Introduction: Here we aim to evaluate a novel topical adjuvant to minoxidil (booster) developed to increase sulfotransferase activity in hair follicles of patients with AGA. The formula uses a liposomal encapsulated alkalizing agent (sodium bicarbonate) to change the pHi of the ORS. The encapsulated chemistry was needed to increase penetration of the alkalizing agent into the cells.

Materials / method: Twenty patients with clinical diagnosis of AGA were recruited for a fourteen day study. Twelve hairs from boarder of scalp with alopecia and normal scalp were plucked for sulfotransferase analysis at the first visit. Subsequently, the patients were provided the adjuvant therapy and instructed to apply 2 ml of the new minoxidil booster twice a daily in the same target area. After fourteen days, twelve hairs from the same location were collected again for sulfotransferase analysis. Follicular sulfotransferase activity was conducted following the method of Goren et al.

Results: Nineteen subjects successfully completed the study. Follicular sulfotransferase activity, as measured by the sulfotransferase assay (OD), increased for 10 of 19 patients (Table 1). More importantly, subjects predicted to be non-responders to minoxidil (OD<0.4) had a significant increase in their minoxidil response. Pre-treatment, the average OD in the non-responder group was 0.2206 (95%CI: 0.1661 to 0.2750) compared to post-treatment 0.4946 (95%CI: 0.2036 to 0.7855) (p<0.03).

Conclusion: This study demonstrates the ability of a topical booster to increase the activity of SULT1A1 in the ORS of the hair follicle. Increasing the activity of SULT1A1 can improve the efficacy of topical and low dose oral minoxidil for the treatment of AGA.

Introduction: Since the beginning of the twentieth century, there have been attempts at creating artificial hair to treat baldness. Major evolution took place at the end of 1970's when, unfortunately, artificial hair treatments were applied without appropriate medical controls, resulting in sub-standard results from the use of unsuitable materials and technique. The large improper use of this technique in North America from no medical personnel and with dangerous fibres led the Food and Drug Administration (FDA) to suspend the procedure in 1983.

Objectives: androgenetic alopecia (AGA), commonly known as common baldness affects up to 80% of the general male population and 50% of the female population at some point in their lives.
Currently, the first-line treatments for AGA in men are oral antiandrogen drugs, 5-α reductase inhibitors. Likewise, Sabal serrulata has the ability to inhibit the two isoenzymes of 5-α reductase is active component of some cosmetic preperations.
The objective of this study is to verify the efficacy and safety of transdermal treatment of sterile cosmetic preparations already marketed and conveyed through microneedling

Introduction: Microneedling is a minimally invasive technique, which allows the opening of microchannels in the skin, favoring the topical absorption of sterile medical and/or cosmetic preparations. Its use is widespread in the treatment of different pathologies, including androgenetic alopecia (AGA).
The aim of the study is to assess the efficacy of microneedling in the treatment of AGA, by serial micropuncture, with commercial sterile cosmetic preparations.

Materials / method: Ten male volunteer patients diagnosed with AGA, were included in the study. They underwent 10 treatment sessions, performed weekly, using the microneedling technique followed by the application of 2 sterile topical cosmetic products containing Sabal serrulata and biomimetic growth factors. Daily home treatment with topical lotion and intake of a nutritional supplement was also prescribed.The following variables were considered:
- Average number of hairs per control area [N/cm2]
- Average hair shaft thickness [μm]
- Accumulated thickness of hair by surface area [mm/cm2]
- Sinclair scale

Results: At the 5th session, a reduction in hair loss was observed in all patients. The 60% presented a decrease in the AGA on the Sinclair scale, compared to the beginning of the treatment, which was objectified by trichogram. At the 10th session, 70% of the patients treated presented an improvement in cumulative hair thickness (mm/cm2).

Conclusion: Despite the limited number of patients, the transcutaneous administration of sterile topical cosmetics by microneedling together with home treatment could be a successful approach to androgenetic alopecia.
