Objectives: Aging is the natural process of skin laxity, loss of facial fat, hypercontraction of the facial muscles , relaxation of the retaining ligament and lastly resorption of the facial bony structure. There are several modality of the treatment including the energy based devices, Fillers, Botulinum Toxin, PRP, fat graft and Thread to rejuvenate all the facial structures. Thread is becoming more acceptable and reliable outcomes. We have many kinds of thread starting from plane suture, coned thread, cogs and barbed thread with different kinds of materials.
Introduction: We had utilized Nylon, Gold, Polylactic acid, PDO and Polycaprolactone Thread in our 20 years of experiences in thread suspension devices. We also combine with other injectables and energy based devices to increase the duration of the outcome.
Materials / method: We have conducted our study using Bidirectional barbed thread for 15 cases with the new approach for Asian face shape to lift up and repositioning of the fat to contour the nice looking of the face. We inserted one direction of the thread to the jowl and nasolabial fold area in superficial fat compartment plane and another direction up to the scalp in the loose space interfascial plane above the Deep temporal fascia to avoid the vascular structures. We use 2-3 threads on each side of the face. We used Toxin and Fillers in some cases. We also perform HIFU and RF combining together.
Results: We can get improvement of the lifting effect and contouring the shape of the face accessed by photo before and after the treatment. We have one case of hematoma. It resolved by itself. We don't have other serious complication such as major nerve injury or parotid gland injury. We also can get the longer duration of the outcome.
Conclusion: Bidirectional Barbed Thread can be one good alternative treatment for facial laxity by the fat repositioning and contouring of the face. By combining with other injectables and energy based devices We can get longer duration of the outcome.
