Objectives: Our goal is to show that a concentrate SVT can be effective in theses indications. This presentation aims to present our technique, analyzing effectiveness, patient satisfaction, and complications.

Introduction: We aimed to access whether a novel concentrate SVT procedure improves skin quality, while yielding a regenerative effect, and whether this novel technique can also achieve good results in upper lip, tear trough, and others tricky areas.

Materials / method: Fat was aspirated from inner face of the knee, or medial thigh. Following aspiration and flushing, microfat was obtained after washing with saline. This microfat was centrifuged to obtain a concentrate suspension, wich was emulsified to obtain a stromal vascular tissue (SVT) suspension. Finaly the SVT was centrifuged to obtain a lipoconcentrate witch was injected in the upper lip using with a 30G needle into the subcutaneous layer. Images were obtained before and after at 3, and 6 months. Number of cells, viability, cytometrie, CFU-F were measured.

Results: 10 patients were included. Concentrate SVT contained significantly higher numbers of cells per gram compared with all other fractions. The clinical results were apparent between 4 and 6 weeks after injection, and improvements were continuously observed until 6 months postoperatively. All patients confirmed an improvement in texture, elasticity, firmness, fine wrinkles, and skin hydration, color of the skin, along with a regenerative effect. Patients also exhibited considerable improvements in skin regularity. Only minor complications were noted, including redness and edema between 2 and 4 day

Conclusion: Skin rejuvenation with concentrate stromal vascular tissue injections at upper lip, tear trough and other delicate areas appears to be an effective method, centrifugation of stromal vascular tissue increases the number of cells, This autologous procedure is able to provide a very good improvement in all this indications, although additional studies are necessary.

Introduction: We aimed to access whether a novel topical washing buffer procedure improves skin quality, while yielding a regenerative effect, and whether this novel technique can also achieve good results in, tear trough area, or acne scars.

Materials / method: Following aspiration and flushing, microfat was obtained after washing with saline. This microfat was emulsified to obtain a stromal vascular tissue (SVT). SVT was sent to Swiss Stem Cell foundation. Isolated stromal vascular fraction (SVF) cells in culture start dividing after 75 hours defining how and when the washing buffer should be produced and collected. Then the washing buffer was added in a topical cream, and applied morning and evening on all parts of the face for 3 months.
Images were obtained before and after at 3, and 6 months. Number of exosomes, viability, were measured.

Results: 10 patients were included. The clinical results were apparent between 4 and 6 weeks after starting the treatment, and improvements were continuously observed until 6 months postoperatively. All patients confirmed an improvement in texture, elasticity, firmness, fine lines, skin hydration, and skin pigmentation, with a regenerative effect. Patients also exhibited considerable improvements in skin regularity. Only minor complications were noted, including redness and edema between 2 and 4 days, and some bruises and pain in donor site.

Conclusion: Skin rejuvenation with topical washing buffer in skin rejuvenation appears to be an effective method. Extra cellular microvesicles released from cells seem to be the key components of the cell secretome. This autologous procedure can provide a very good improvement in all these indications, although additional studies are necessary.

Objectives: For the standardized intraoperative harvest of adipose tissue and concurrent isolation and concentration of stromal vascular fraction (SVF), the compact Q-graft® system provides one closed semi-automated sterile environment.

Introduction: The cells of the SVF are known to be a promising tool for regenerative medicine and tissue engineering. It is necessary to be able to consecutively harvest adipose tissue, extract the SVF and apply it for regenerative treatments on the patient during one procedure. For this purpose, non-manual closed isolation systems that can easily be handled in the operating room are necessary.

Materials / method: In combination with a device for the water-jet assisted liposuction, adipose tissue can be harvested right into the Q-graft® system. The adipose tissue is dissociated through enzymatic digestion under constant mixing and heating to 37 °C, with Humanase®, a blend of proteolytic GMP grade enzymes. The SVF cells are separated from the adipose tissue through a series of steps of washing and filtration. The so collected suspension of the SVF can be concentrated in the device by the innovative process of radial filtration. This makes the use of a centrifuge and thus an additional device obsolete.

Results: The Q-graft® system is able to separate the SVF cells from the adipocyte fraction of the adipose tissue. Preliminary results show that the Q-graft® system was able to isolate an average of 3.51E+05 SVF cells per ml processed tissue. With a processing volume of 75 ml of adipose tissue, this results in a total cell number averaging 26.4E+06 cells. No bacterial contamination was found in the tested cell suspensions.

Conclusion: At this point, the Q-graft system is being used in the course of clinical studies for various applications. The most prominent being the treatment of arthritis of the knee and thumb joints. Another promising field of application is the treatment of chronic wounds caused by diabetes or peripheral vascular disease. Thus, the successful isolation of the SVF in the closed Q-graft® system clears the way for the clinical application of this regenerative cell fraction.
