Endo-radiofrequency medical lifting combined with platelet-rich plasma

Objectives: The purpose of this study is to confirm the regenerative and lifting effect of the combined procedure endo-radiofrequency & Platelet rich plasma. The neocollagenesis & neoelastogenesis stimulation generated by the heat produced by the endo-radiofrequency is amplified by the intracutaneous PRP injection. The perfect temperature control on the subdermal layer is indispensable to ensure a total safety of the procedure.

Introduction: The regenerative effect of the growth factors contained in the PRP on the dermal fibroblasts is well approved since many years in the field of anti-aging medicine.
The thermo-regulated endo-radiofrequency is a new technology which allows to reach a temperature of 57 degrees on the deeper dermal layer using a thermo cannula. Combining both procedures during the same session allows us to improve the skin texture and to obtain a medical lifting improving the jaw-line defintion.

Materials / method: I have treated two female patients aged 45 & 55 years. The treatment was performed on the middle & lawer face with thermoregulated endo-radiofrequency using a thermo-cannula introduced in the subdermal layer from a lateral entry-point on the mandibular angle and reaching a temperature of 55 degrees for both patients. The procedure lasted 30 minutes ,15 minutes on each half face. Immediately after the endo-radiofrequency, I drawed 20 ml of blood that i spined for 3 minutes 3000 rpm (1500G), i obtained 13 ml of pure PRP that i injected in the dermis using a 25 g cannula & microneedling.

Results: No medication was prescrived afetr the treatment, no side effects as edema swellling or bruising was observed. I controlled the patients twice after 1 month & 3 months. The immediate result was a remarkable lifting effect with an immediate skin tightening improving the jawline definition. The improvement of the skin texture was more evident after 1 month. The best result was obvious on the third month as the maximum dermal neocollagenesis & neoelastogenesis is noticed 3 months after fibroblast stimulation.

Conclusion: The combined procedure using both the lifting effect of the endo-radiofrequency & the regenerative effect of the platelet rich plasma is extremely effective to obtain a medical lifting improving the jawline definition & the skin texture at the same time.
The procedure is absolutely painless very safe and doesn't cause any side effects. The result is immediately visible because of the immediate skin tightening due to the endo-radiofrequency tissue heating, although the skin texture improves drastically thanks to the growth factors as VEGF, iGF, FGF contained in the platelet rich plasma

The future applications of adipose-derived stem cells in clinical dermatology

Objectives: To review and evaluate that have used the stromal vascular fraction in order to have a scarless wound healing and to identify the current gap in the techniques of preparation and use of SVF.

Materials / method: The author systematically reviewed several different studies and case reports available on different search engines. The data was analyzed for any new unconventional tools or techniques that is using Stromal vascular fraction in scar reduction.

Results: Scar tissue consists mainly of myofibroblasts formed by fibrillar collagen. Factors that are essential for myofibroblast activation include the availability of active growth factors such as TGFβ1 and PDGF; activation of inflammatory signaling; and mechanical response to stress.4 Results of studies have been done on animals showed that collagen in the group that had SVF was arranged more regularly with a broader gap between collagens. And also the mRNA expressions of TGF-β and Smad3 in the SVF group were significantly down-regulated.5 another experiment showed that SVF contains EGF and VEGF cyt

Conclusion: SVF injections improve tissue regeneration by adding stem cells and growth factors to boost
Results in scar revisions or in tissue grafts. Important SVF-related scoring gains suggest SVF 's worth as an element of traditional scars administration.7

Efficacy and quality evaluation of cell-based harvested material: facial fat grafting

Objectives: The main objective of the present review was to examine the most common cell-based harvested tissue regenerative medicine treatments outcome evaluation.

Introduction: Multiple regenerative methods, appeared during the last years, are applied for elderly skin rejuvenation, wrinkles reduction, scar treatments. Nonetheless, no standardized, homogeneous assessment protocols and follow up timings have been established.

Materials / method: This review research method is based on a literature search. 586 Papers regarding macrofat, microfat, nanofat, stromal vascular fraction and Rigenera for face rejuvenation procedures have been found. Therefore, 68 papers were selected for the further quality analysis assessment.

Results: According to the literature analysis, there is an important heterogeneity among the papers regarding cell-based treatments.
In 33 papers out of 68 (48,5%) standardized photography has been reported, while in 22 papers out of 68 (32,3%) non-standardized photography has been reported.
In 8 papers out of 68 (11,8%) 3D photography has been used as an evaluation tool, while in 11 out of 68 papers (16,2%) other forms of imaging evaluation, such as TC and MRI have been used.
Validated PROMs have been proposed in 9 papers out of 68 (13,%), while in 10 out of 68 papers (14,7%) not-validated PROM

Conclusion: Ideally Validated PROM, corresponding to an objective evaluation 3D photography that allows skin analysis, standardized photography, and short (1 and 3 months), medium (1 year) and long follow up (2, 3 and 5 year) results as an ideal protocol for the best outcome assessment.
Findings in this research may be a general guideline for the cell regenerative medicine treatmetns and, therefore, may be the basis for the future evidence-based medicine.

Introduction: Nanofat is emulsified (liquified) and filtered fat. The fat cells have been crushed and filtered out and what remains is a liquid that contains a high concentration of stem cells.

At first we used tumescent solution for the area that has determined before.

Finally some of the harvesting fat ,changed to nanofat by special conectors.
Now we have two types of fat that used in face injections by the cases needs

Results: During 4 years we have used both nonofat & millifat in face rejuvenation simultanously in more than 500 cases & have achieved very wonderful results.
This method canbe used instead of other methods of face rejuvenation & face filling due to less costs & natural components.

Conclusion: This method canbe used instead of other methods of face rejuvenation & face filling due to less costs & natural components.
In this method the final results after 30-45 days are very natural augmented face with amazing rejuvenation of face skin.

Platelet-rich fibrin and autologous tissue filler as an effective method of skin revitalization

Objectives: Objectives:
1. to verify with the use of high-frequency ultrasonography whether platelet-rich fibrin (PRF) and autologous tissue filler can increase skin density and become an effective method of skin revitalisation
2. to determine an optimal procedure protocol for the use of blood derivates to revitalise skin

Introduction: Platelet rich plasma was the first generation of autologous blood derived product that was tissue compatible, non-toxic, not causing allergy reactions, and effective in tissue regeneration and wound healing. However, the addiction of anticoagulants and different preparation methods limited the sage of PRP in aesthetic medicine. Further improving preparation and centrifugation methods led to platelet rich fibrin concept. Autologous filler is the next generation of blood derived product providing skin bio stimulation. It can be also used as a filler placed in atrophic areas of the aging face.

Materials / method: The author will present detailed protocol of how to prepare autologous filler and the cases of the patients who underwent this therapy. A high-frequency ultrasound device was used to measure skin density before every treatment and a month and 2 months after procedure.Changes in skin density and in thickness of epidermis and dermis in the assessed areas of the face will be shown on the ultrasound scans as well as on the photographs of the patients.

Results: The results have demonstrated an increase in skin density in the assessed areas in all patients after just one procedure.The enrolled patients’ satisfaction with their facial skin appearance was measured with the Visual Analogue Scale (VAS) before and after treatment.

Conclusion: 1. Platelet-rich fibrin and autologous filler appear to effectively revitalise facial skin and increase its density.
2. Although the results are very promising, there is need for larger controlled research to definitely confirm platelet-rich fibrin and autologous filler efficacy in the skin revitalisation processes.

Stromal vascular fraction and emulsified fat as regenerative tools in rejuvenation of the lower eyelid area

Objectives: Our aim was to investigate the clinical results of transferring SVF and emulsified fat into the lower eyelid area.

Introduction: In the lower eyelid area, dermal melanocytosis, fine lines, skin atrophy, dryness, and loss of subcutaneous fat tissue represent the initial signs of aging. Beside the addition of volume, adipose tissue injections can also improve pigmentation and skin texture. Clinical studies of simultaneous stromal vascular fraction (SVF) and emulsified fat transfers have not been reported so far.
Objective: Our aim was to investigate the clinical results of transferring SVF and emulsified fat into the lower eyelid area.

Materials / method: 16 patients underwent tumescent liposuction and injection of SVF and emulsified fat into the lower eyelid area. For preparation of SVF and emulsified fat, ACP double syringes with 2.4 mm, 1.4 mm, and 1.2 mm connectors, and a swing-out rotor centrifuge, were used. At follow-up, improvements on before and after pictures were rated by the treating physician and two independent physicians, using the Global Aesthetic Improvement Scale (GAIS).

Results: Clinical outcomes were rated as exceptional, very improved or improved in all patients, with an average GAIS score of 1.6. No serious adverse events occurred.

Conclusion: Our initial results suggest that SVF and emulsified fat are safe and effective tools for skin rejuvenation and correction of volume deficiencies in the lower eyelid area. More studies need to be conducted to corroborate these encouraging findings.

Exosomes and extracellular vesicles derived from PRP and hyperacute serum - are they the same?

Objectives: 1. Exosomes and extracellular vesicles (EVs): definitions, isolation techniques, composition.
2. The effects of exosomes and EVs on cell proliferation and inflammation.
3. Possible clinical applications.

Introduction: Modern aesthetic treatments work at the cellular level to activate the natural regenerative processes of our tissues. Blood-derived products (BPs) like PRP or hyperacute serum (hypACT) received considerable attention in regenerative medicine since the elucidation of their mechanism and reports of their biological efficacy. The discovery of exosomes and extracellular vesicles (EVs) as novel active components of blood and their recognition as relevant signalling players in tissue homeostasis have increased the level of complexity in understanding the effects of blood-derived therapies.

Materials / method: EVs and exosomes were isolated from freshly prepared blood products via ultracentrifugation. Sizes and concentrations of particles were determined via NTA. To characterize EVs and exosomes in PRP and hypACT, their cellular origin was determined via flow cytometry. To observe effects of EVs on cell morphology, metabolic activity assay was performed as well cytokine (IL-6, TNF-α, and IL-1β) analysis of cell culture supernatants.

Results: Mode sizes were similar, but not identical between exosomes and EVs from PRP and hypACT. PRP had more EVs (AnnV+, CD41+) than hypACT, but fewer exosomes (CD63+). In freeze-dried hypACT higher concentration of EVs was observed. Most EVs originate from platelets in both blood derivatives even in absence of platelets in pure hypACT. Fibroblasts supplemented with EVs from hypACT had higher metabolic activity than cells cultured with EVs from PRP. EV-supplemented cells showed lower levels of the pro-inflammatory cytokines IL-6, TNF-α and IL-1β compared to cells supplemented with the respective BPs.

Conclusion: Exosomes and EVs isolated from hypACT were able to increase the metabolic activity of fibroblasts compared to exosomes and EVs isolated from PRP. The decrease of pro-inflammatory cytokines was observed in cells supplemented with exosomes and EVs from both BPs. Exosomes and EVs isolated from stem cells have been already suggested as therapeutic agents in the context of regenerative medicine, however, exosomes and EVs from BPs offer more benefits as there is no requirement for cell culture – exosomes can be produced as stable BPs.

Exosomes in regenerative aesthetics & dermatology: my three year clinical experience

Objectives: 1. Outline of the profiling of Adipose Stem Cell Exosomes (ASC-exosomes), their mRNA composition, their toxicology and safety profile.
2. Understanding the mechanism of action of ASC-exosomes and the cellular pathways of exosomes in skin rejuvenation, rosacea, melasma and in facial eczema.
3. Understanding the clinical applications of ASC-exosomes in regenerative aesthetics and in the treatment of complex dermatology conditions.
4. Critical evaluation of my three-year experience in using exosomes in facial regenerative aesthetics, rosacea, melasma, facial and body scars and stretch marks.

Introduction: Adipose Stem Cell exosomes (ASC-exosomes)are extracellular microvesicles 30-200nm in diameter with potent regenerative functions of mesenchymal stem cells. They are the mediators of cell-to-cell communication with numerous growth factors and functional miRNA.They have regenerative, anti-inflammatory and immunemodulatory effects. Exosomes therapy has surged across USA and Asia as a "novel cell free regenerative therapy". Exosomes have synergistic biostimulatory effects in combination with energy-based devices and my case series will be reviewed amongst other cases worldwide.

Materials / method: Invivo studies using electron microscopy and nano tracking analysis were performed to characterise exosomes. My Clinical Case Cohort:40 patients were treated for facial skin rejuvenation, rosacea, melasma, eczema and 10 patients were treated for body stretch marks and scarring with exosomes either in isolation or in combination with Microneedling Radiofrequency. Standardised Facial Photographs and skin analysis were recorded using VISIA Skin Analysis at Day0, Day 30 and Day 60.My clinical cases were performed in Dr Damree Aesthetic Clinic,London and reviewed by a surgeon.

Results: 1. Exosomes have1008 growth factors and 598 miRNAs for cell proliferation, wound healing, anti-inflammatory and anti-ageing.Review of my Clinical Case Series:(a) Face and Neck Rejuvenation with skin tightening, reduction in fine lines, correction of hyperpigmentation and improvement in skin texture(b)Periorbital, MidFace and Lower face rejuvenation(c)Scarless facial wound healing (d) Repaired Skin Barrier Periorbital Eczema(e) Resolution flare up rosacea, skin sensitivity post COVID 19 Vaccination(f) Periorbital Hyperpigmentation Correction(g)Abdominal Stretch marks and Burn Scars resolution.

Conclusion: The powerful regenerative and anti-inflammatory properties of ASC-exosomes have been reflected in clinical my practice for regenerative aesthetics, in rosacea and melasma and in the treatment of facial and body scars. The synergistic bio stimulatory effects when used in combination with energy-based devices yield superior results. My three-year clinical experience with minimal complications has proven its superior clinical efficacy with a high safety profile. I conclude that ASC-exosomes can be the next generation ‘cell free’ regenerative treatment in regenerative aesthetics and dermatology.

Fat Juice: a novel approach on the usage and preparation of adipose tissue byproducts

Objectives: The primary goal of Fat Juice's development was to create a lipoaspirate byproduct that could be used to treat and improve variety of skin quality issues without having the volumizing effects of nanofat, allowing it to be applied more widely into the dermis, and was also simple to make and reproduce, making it an accessible therapy for a variety of medical fields. The authors devised and described a novel mechanical technique to isolate Fat Juice, then examined the cellular and acellular components of Fat Juice extracted from 11 healthy patients and presented the findings in the current study.

Introduction: The current study introduce a novel approach on the usage of Stromal Vascular Fraction (SVF) in conjunction with fat extract (FE) termed Fat Juice, presenting a comprehensive biochemical analysis and potential relevance of this new combination .

Materials / method: The technique employs a non-enzymatic mechanical method. A biomolecular analyses of the mixture based on the quantity and viability of stem cell, presence and quantification of connective tissue fibers (collagen type I, III, and elastin), interleucine-6, MRC 1 and VEGF is presented. Stem cell isolation and enumeration were performed on 11 samples. Stem cells average sizes and viability rates were analyzed.

Results: With the laboratorial, histological and biomolecular analysis of the samples becomes clear the many clinical possible applications of the preparation, specially when taking into notice the fact that the authors advise it's use not for augmentation or volumization but for rejuvenation and revitalization of the skin by means of intra dermal injection.

Conclusion: Fat juice mixture is an easy to inject concentration of adipocyte/preadipocytes, red blood cells, adipose-derived stem cells, endothelial-derived cells and cell residues. The study observed, quantified and compared between samples the presence of connective tissue proteins, İL-6, MRC1 and VEGF. The results showed potential for usages on skin rejuvenation and regeneration due to the combined properties of the SVF and Fat Extract. The authors agree that its easy isolation process and abundant availability Fat Juice is an effective skin quality enhancer widely useful in many medical fields .
