Introduction: The market in aesthetics is filled up with skin tightening devices. Liposuction and Body Contouring treatments are rising with 2digit numbers in the last years. As physicians in the private sector we have to manage to get an overview on which technology would suit the needs of my patients. In this presentation I would like to share my unbiased 10 year review on the different devices I use for these minimal invasive procedures

Materials / method: I will share the technologies behind devices like BodyTite/ FaceTime and the Renuvion Plasma. Showing videos and pics on the procedures and the outcome

Management and treatment of scarring through auto-crosslinked HA 200 kDa combinated with endodermic technology

Objectives: 1. Non-invasive management of hypertrophic scars in acne and after dermal remodeling.
2. Modify the structure of the reparative tissue and limit the hypertrophic component in the inflammatory skin healing phase.
3. Lower the score on the Vancouver and P.O.S.A.S. scale.

Introduction: Hypertrophic scarring represents one of the most insidious complications in the skin repair in post acne and in surgery outcomes that can be addressed by hypodermic injections of auto-cross linked hyaluronic acid 200 KDa combined with endodermal radiofrequency technology and high flow carboxytherapy.

Materials / method: 5 surgical scars and 2 acne scars in the comedo-cystic phase were selected and treated with the following protocol:
One application of auto-cross linked hyaluronic acid 200 kd every 15 days for two months.
Alternate use of emitting Radiofrequency (RF) followed by galvanic current via ultra-thin electrodes for two times in the same session with epidermis insufflation of high flow Carboxy-therapy every 15 days apart for 2 months.
Application of trichloroacetic acid 33% modulated with hydrogen peroxide for 5 minutes at the end of all session.
Use of SPF 130+ for 4 months two times a day.

Results: In scars that had a score of 7-9 points on the Vancouver scale at baseline, a score of 2-4 points was obtained 15 days after the combined approach and 75 days after the start. At the same time, scars that at baseline had a POSAS score from observer perspectives of 28-33 points and a from patient perspective score of 45-56 points, after 15 days from the combined approach and 75 days from the beginning, they achieved a total score of 17-24 points respectively of 23-35 points.

Conclusion: This combined treatment for scars resulted in all treated cases in tissue remodeling with reduction of the inflammatory component, smoothing of the scar and replacement of disorganized fibrous tissue with homogeneous repair tissue that could be confused with the surrounding skin tissue associated with contraction and reduction of amplitude along the transverse axis.

Treatment of static lateral periorbital wrinkles with combination of fractional CO2 laser, PRP and botulinum toxin A: a case series of 89 patients

Objectives: The aim of the study is to evaluate the efficacy of combination treatments such as fractional CO2 laser and PRP (Platelet Rich Plasma) along with botulinum toxin in lateral periorbital static wrinkles in 89 patients. Static expression wrinkles is a challenging problem to solve as patients satisfaction rates are relatively low when a monotherapy is applied.

Introduction: Fractional CO2 laser, through the induction of thermal damage, leads to collagen production and skin rejuvenation. Autologous mesotherapy PRP (Platelet Rich Plasma), promotes the activation of stem cells, targeting cell rejuvenation and tissue healing. In the present study, we examine the effectiveness of combined fractional CO2 and PRP performed in the same session plus botulinum toxin A as a way to correct stubborn static periorbital lines.

Materials / method: The protocol consisted of 3 sessions of fractional laser & PRP every 4 weeks on the crow’s feet in 89 patients (68 women, 21men, mean age: 48 y). All patients had undergone Botulinum toxin A as a monotherapy in the past. Neurotoxin was performed 15 days before the first combined session at an average dose of 15 units on orbicularis oculi (Onabotulinum toxin Α). Photographs were taken before, 15 days and 2 months after the last session. Fractional laser settings were: Energy 15-20 mJ, Power 8-10 W, density 89-100 dots/cm2. PRP was administered injectably after centrifugation and activation.

Results: A significant reduction of static wrinkles was noticed 15 days after the last treatment, with a patient satisfaction rate of up to 80% on month 4, compared with 60% when botulinum toxin was used as monotherapy. The patients reported better results in the depth of wrinkles and the duration of the result. Nearly all patients mentioned a better appearance regarding the skin quality and elasticity, whilst none of them complained about any severe side effects, except for a mild feeling of flushing and a slight roughness of the skin in the following 2 days.

Conclusion: The combination of Fractional laser CO2 plus PRP with botulinum toxin A is a safe, minimally invasive therapeutic approach with great results in the treatment of static wrinkles in periorbital area. PRP reduces the symptoms of skin injury caused by the application of Fractional co2 laser alone, promoting a faster healing and collagen production of the skin. In addition botulinum toxin seems to have better performance and longer duration when applied as part of fractional CO2 skin rejuvenation protocol.

A pilot study: combining fractional carbondioxide laser and polydioxanone threads in treating skin laxity and brow ptosis in Asian patients

Objectives: To evaluate the efficacy and safety of combining fractional carbon dioxide laser and polydioxanone threads in treating skin laxity and brow ptosis in Asian patients

Introduction: For much of the Asian population, skin laxity around the eye and brow ptosis are the first to be observed with aging. Descending of the upper lid due to skin laxity over the entire eyes and less arching of the brows translates as “tired or sad look” which prompts patient to seek consult.
Blepharoplasty although considered as the gold standard in upper lid ptosis treatment is a surgical procedure that may inevitably lead to surgical complications such as under/over correction, scarring and infection. Due to this possibility, minimally invasive non-surgical procedures are now the treatment of

Materials / method: Study of 15 patients male and female was selected, aged 35-50 years old with Fitzpatrick skin type IV. All subjects had 1 session of Fractional carbon dioxide laser around the eye area over the orbital rim at 5% concentration at 10 J/cm2 followed by PDO thread insertion over the brow area using 10 pieces , mono, 29G, 25mm/30mm,6.0 and lower lid 15 pieces, mono, 31G, 12mm/10mm threads.
Topical antibiotics were applied twice a day for 5 days. Inflammation and bruising were managed by ice packs and Vitamin K cream and oral analgesics .

Results: RESULTS
A single combined treatment of fractional carbon dioxide laser and PDO thread insertion increased brow height of an average of 4 mm, widened the upper lid crease by 1-3 mm and improved skin laxity scores by minimum of 1-2 Grades.
All patients observed significant degrees of skin tightening, brow elevation, texture improvement and wrinkle reduction in the eye area. Statistical data analysis was significant.

Conclusion: This study proved that by combining fractional carbon dioxide laser and polydioxanone threads showed significant improvement in upper lid ptosis and brow ptosis based on measurement of the 1) Midline brow height with reference to the mid pupillary line and 2) Margin crease distance on all patients. This also showed improvement in scores of patient’s lid distraction test. Safety of the treatment is also established

Treatment of post-inflammatory hyperpigmentation with a combined protocol between chemical peeling and mesotherapy

Objectives: the aim is to propose a combined treatment to improve the skin in acne patients and in patients with PIH

Introduction: Acne vulgaris is an extremely common condition affecting the pilosebaceous unit of the skin and is characterized by the appearance of comedones, papules, pustules, nodules, cysts, which can cause permanent scarring. Acne vulgaris commonly involves adolescents, although there is a second group of incidence in young women in their thirties to forty years of age.
It often results in the appearance of post inflammatory hyperpigmentation (PIH), also common to other pathological conditions, extremely complicated to treat, for which our paraphernalia is often unguarded.

Materials / method: The objective of the study is to show how to improve mild and moderate acne with a combined protocol that involves the association of a chemical peel (containing 20% salicylic acid, 5% citric acid and 5% mandelic acid) , associated according to a specific timing to infiltrations of low molecular weight hyaluronic acid with a skin-restructuring complex.
Materials and methods: 40 patients with mild-moderate acne or with PIH were enrolled and underwent 3 chemical peeling sessions three weeks apart, followed by mesotherapy.

Results: Results: satisfactory in 80% of patients, who demonstrated a reduction in Michaelsson acne scores of over 60%, and a reduction of PIH.

Conclusion: Conclusion: this new protocol shows promise in the treatment of mild-moderate acne and PIHs for which we often do not have many weapons at our disposal.

Treatment of atrophic scars combining microcannule subcision and polycaprolactone-based filler device

Objectives: 1. To treat atrophic/fibrotic scars from causes other than acne with the combination of microcannula subcision and polycaprolactone infiltration.

2. To evaluate the improvement and satisfaction of the patient through interview, clinical analysis and photographic comparison.

Introduction: Subincision (SI) as been used to improve the appearance of depressed/atrophic scars1,2. The combination of SI plus filler materials that stimulate neocollagenesis, such as polylactic acid and polymethyl methacrylate has suggested greater efficacy in the results3,4. Polycaprolactone (PCL) is a bioabsorbable polymer widely used in medicine; as well as in the aesthetic industry in rejuvenation treatments with minimal adverse effects and long lasting results with evidence in dermal thickness improve. We found one paper about the use of PCL in acne scars but not from causes other than acne.

Materials / method: We present 3 cases, all scars from various causes (one for trauma; and two post aesthetic surgical complication).
In all treated cases we perform debridement of fibrosis with 23G microcannula
plus infiltration of 0.5 to a maximum of 1 ml of PCL.
Photographic evidence was taken before the treatment and at one month in two cases and at 6 months in the last case.

Results: We were able to observe improvement in all treated cases. In clinical assessment and photographic comparison we appreciate better texture , greater thickness, and smoother surface in each study case.

Conclusion: The use of PCL biopolymer associated with fibrosis debridement by sub-incision with microcannula offers satisfactory results in the medium term in the improvement of atrophic/fibrotic scars.
