Face and neck lift using permanent non-absorbable silicone threads with results comparable to surgery

Objectives: Identify facial ptosis resulting in formation of nasolabial folds, marionette folds and jowls causing squaring of the face.

Identify laxity of neck and submental area resulting in a "double chin" and neck folds.

The use of permanent non-absorbable threads to achieve a great lift of the face and neck comparable to surgery that lasts 3-5 years

Introduction: A main requirement of patients seeking cosmetic treatments is to achieve a lift of the lower face and neck to give a youthful appearance. There are very few non-surgical procedures that produce an immediate lift of the face and neck which is long lasting. I have demonstrated achieving a surgical level lift with a minimally invasive procedure using non-absorbable threads. This treatment is suitable for patients with moderate ptosis and laxity who don’t want surgical lifting or have had a surgical lift previously with results starting to fade. The results are expected to last 3-5 years.

Materials / method: A flexible and elastic thread made from silicone and polyester and containing 24 tiny cogs per cm that point in 4 directions was used. It is 300mm in length with long blunt-end needles at each end. The thread suspends the underlying skin tissue thus lifting sagging skin. The procedure is carried out using local anaesthetic infiltration without sedation. 3 threads are place on each side of the face terminating at nasolabial, marionette and jowl region. The proximal ends are fixed into the scalp. 3 threads are placed on each side of the neck terminating in midline and also fixed to scalp.

Results: The patient achieved remarkable lifting of the nasolabial fold, marionette fold and jowls. The repositioning of cheek also improved the tear trough region. By lifting the nasolabial fold, the upper lip is slightly everted making it appear fuller.

The lifting of the neck resulted in reduction of “double chin” and softening of the platysmal bands.

The overall effect of the face and neck lift gave the patient a youthful appearance with transformation of a square face into a V shape and tightening of the neck.

Conclusion: The permanent thread helps to fill the gap between the dissolving PDO / PLLA threads and surgical facelift. It can be performed under local anaesthetic, has a low downtime, the lift produced is substantial and the results can last over 3 years. The procedure is well tolerated by patients and the technique is relatively easy to master. The risk profile is broadly similar to PDO and PLLA. The two biggest factors that stand it apart from dissolving threads is the longevity and the amount of lift achieved. This makes it a true alternative to a surgical facelift.

The most popular thread lifting methods in aesthetic and plastic surgery

Objectives: my abstract aims to provide information for doctors, what popular methods includes thread lifting methods nowadays, and how to achieve desirable result.

Introduction: Thread lifting procedure can be performed in every area on the faca and body, nowadays a lot of patients have their favorite treatment parts, as well for doctors that they have favourable treatment zones on the face.

Materials / method: very popular nowadays in thread lifting procedures is correction of eyebrow with canthoplasty, chin, otoplasty and correction of nipple areola complex (NAP). all this area can be treated effectively and safely with thread lifting methods. in my study I will show how to perform proper techniques on target zones to achieve satisfied results

Results: thread lifting methods for several part of the face and body gives desirable results, with minimal downtime and long standing results. patients afterwards are very satisfied of their results.

Conclusion: thread lifting methods is very popular, mini invasive, safe, cost-effective procedures, that gives us possibility to save result for a long time.

AFU protocol: advanced facial ultrasound with elastography and 5D ultrasound

Objectives: Our goal is to present the AFU PROTOCOL
Advanced Facial Ultrasound for the identification of anatomical structures using high technology such as Elastography and Three-dimensional Analysis with 5D Ultrasound

Introduction: Facial Ultrasound is here to stay in the world of high-resolution facial aesthetics by avoiding complications by knowing and interpreting the antomy more efficiently.
Our research shows the most advanced ultrasonographic tools such as shearwave elastography to determine the degree of tissue tension with the different fillers and visualize the 5D image for final verification.

Materials / method: We present a series of 15 clinical cases where we show the usefulness of high-resolution ultrasound, performed with high-resolution general electric ultrasound machines, techniques performed by expert specialists in ultrasonography during 2 years of research in our hospital.

Results: In 100% of our patients we improve the anatomical references since we do not perform blind procedures and we know beforehand the different degrees of muscular tension, as well as the pressures at the level of nerves, arteries and facial veins.
Patients with the application of new technologies felt 100% satisfied.
We avoid in 100% of patients complications inherent to interventional techniques when we use fillers

Conclusion: We conclude that the use of high technology applied to a better knowledge of facial anatomy is key when it comes to avoiding complications.
Shearwave Elastography is a scientific tool that has demonstrated its usefulness to determine the degree of elasticity and deformation suffered by tissues before and after the application of fillers.
Facial Ultrasonography is the gold standard of the new era of facial aesthetics and 5D and transillumination techniques guarantee efficacy and safety.

Lip remodeling: where to start to avoid duck effect

Objectives: Due to the increase of lips filler’s demand, it is of upmost importance for doctors to know better the anatomy related to these injections and the latest studies on advanced lip remodeling techniques. A well-trained doctor can evaluate the reasons behind the request of lip remodeling and help the patients in the definition of the right approach. A surgical anatomical knowledge of the face can guide the injector to a safer and better outcome procedure. The aim of this study is to demonstrate the effectiveness of a technique with very few complications and high-quality predictable results.

Introduction: The treatment of lips is nowadays a very common and often a really underestimated injection in terms of difficulty of execution and quality of results.
Lips injections require a very challenging approach even for experienced Aesthetic Physicians. The vast majority of patients looking for lip remodeling are very young and are looking for lips filler in order to follow current fashion or to improve their sexual appealing.

Materials / method: The technique called “GB lips” was selected because it seems the most adequate to fulfil patients’ natural effect expectations and guarantee high levels of safety.
It requires a deep understanding of the role of vermilion and SMAS to ensure a safer and more predictable injection.
The choice of the specific Hyaluronic Acid (HA) is linked to its rheology and to the need to use high quality materials in such a sensitive area of the face.
A 22,5 mg/ml HA was used and an amount of 0,8 or 1,5 ml was placed in 1 session to more than 400 patients aged 21-55 years.

Results: Results were evaluated comparing pictures before and after treatment and through patient assessment.
The respect for the anatomy and motor function of the lips was preserved in 100% of the cases. The only complications were prolonged edema (+ 6 days) in 3% of cases and minor hematomas in 9% of cases.
The aesthetic complication causing the more frequent imperfection and error in lips injection is the spread and/or migration of the product.
The spread and migration of the product was 0%.
The patients reported a nice result with harmonic movement of the lips. No AE in mid term.

Conclusion: These results confirm the important role of a solid medical background, a deep knowledge of anatomy and a long-term experience with techniques in the approach to lip remodeling.
Moreover, the relationship with the patient, the explanation of the expectations, the definition of objective to reach contribute to the success of this treatment.

Strategy differences male and female patients using hybrid injectable fillers

Objectives: - Describe the differences in assesssment between male and female patients when treating them with fillers
-Summarize special gender considerations in facial anatomy and aging process
-Discuss the male and female treatment planning using fillers avoiding undesired masculinization or feminization of the face.

Introduction: The use of injectables has grown in popularity in recent years. It is crucial for the injector the correct assessment and treatment planning of the patient before the filler injections to avoid undesired masculinization or feminization of the face. As usual, the injector may choose biodegradable fillers like hyaluronic acid because they are safe, reversible with long-lasting results. Calcium hydroxyapatite is also safe and effective. Recently, there has been an option with both products in the same syringe, the hybrid product.

Materials / method: The author presents special considerations to use hybrid product in male and female patient based in a follow up of 3 years, more then 100 patients , single center of study. The author is involved in a retrospective study on the safety of the hybrid product as principal investigator in a multicenter study in Brazil.

Results: The audience will be able to recognize the differences between male and female facial treatments with filler injections using hyaluronic acid, calcium hydroxyapatite and hybrid product minimizing the risk of complications and unwanted effects.

Conclusion: The use of filler treatments with hybrid product is a new and safe option for injectors when treating male and female patients respecting gender differences.
