Key points in upper eyelid blepharoplasty

Objectives: the main objective of our presentation is to show the evolution of upper eyelid blepharoplasty after thousands of surgeries performed under local/general anesthesia , with strict and professional patient/treatment selection, using a simple surgical design,applying and respecting standard steps, adding out tips and performing with an obvious improvement of patient satisfaction rate

Introduction: Upper lid blepharoplasty is one of the most demanded aesthetic procedures, all statistics are showing an increased number of surgeries and also the interest for this kind of surgeries in teen agers , the main concerns are the outcome and the scar after surgery, good results and scar quality can be improved by respecting rules and performing tips and tricks

Materials / method: with almost 200 blepharoplasties / year performed in our clinic we were trying to compare results according to our records and we found that patient selection and treatment plan are essential elements to perform blepharoplasty, the second crucial moment is the surgical design , where to place the final scar? How much skin we can excise? we show our personal approach to do surgical marking in a simple, safe and effective way, our decision is to trim part of the Orbicularis-oculi muscle , then to apply a muscle plication in order to fill the gap between the upper and the lower edge of the skin

Results: our results are evaluated by the experts and the patients, scar quality is obviously improved, general satisfaction is better, and the longevity of results is good.

Conclusion: upper eyelid blepharoplasty is not a trend , it is one of the most attractive areas for aesthetic treatment , the number of cases is bigger and good results are the only way to perform in high standards as a plastic surgeon

Anchoring lower eyelid subcutaneous to lateral orbital periosteum in improving blepharoplasty

Objectives: Although lower blepharoplasty is one of the most common requests from people in search of beauty, it is one of the most challenging techniques in the field of plastic surgery because it may be associated with subsequent complications so there is not be pleasant for the surgeon. Although a detailed study has not yet been performed, it may the significant rate of ectropion after lower blepharoplasty be related to lack of eyelid support so according to some theories, lateral cantopexy concomitant with blepharoplasty, decrease the incidence of this condition.

Introduction: Anchoring the subcutaneous tissue of the lower eyelid through a permanent suture to the periosteum outside the orbit can further increased the support of the lower eyelid, which is an innovative technique and the basis of this study.

Materials / method: The patients underwent lower blepharoplasty with percutaneous subciliary technique and then lateral cantopexy perform as fixation of the external retinaculum to the supralateral orbit periosteum. Subsequently, as a new technique, the subcutaneous tissue of the eyelid is anchored laterally to the lateral periosteum with non-absorbable suture

Results: Follow-up visits are performed after one week, one month and six months later. In these sessions, quality structures, including patient satisfaction, eyelid symmetry, crease status, lateral commissure status, visual acuity and possible complications, specially Ectropion is performed and finally the collected data will be analyzed.

Conclusion: Lower lid blepharoplasty is a critical surgery and prevention of complication such as sclra show and ectropion is very important and with this simple suture we could secure lower eye lid and reduce complication .

Advanced techniques in periorbital rejuvenation

Objectives: This lecture presents a comprehensive review of the contemporary techniques applied in periorbital rejuvenation, including but not limited to blepharoplasty, fat grafting, neuromodulator injections, laser resurfacing, and soft filler injections, I will further discuss the underlying principles of these techniques, their indications, potential complications, and methods to reduce risks.

Introduction: Periorbital rejuvenation is an evolving field in aesthetic surgery that focuses on enhancing the appearance of the area around the eyes, a vital facial feature responsible for transfer emotions and general vitality. The eyes are often referred to as the, windows to the soul, and they are one of the first areas where signs of aging may appear. The periocular area is a complex anatomical region, therefore understanding its aging process is crucial for effective rejuvenation.

Materials / method: The periorbital area’s complex anatomy and high functional demands necessitate a sophisticated understanding and application of various treatments. An individualized, patient centric approach is paramount, respecting the balance between achieving desired aesthetic results and maintaining the natural dynamic functions of the periorbital region.

Results: Dear Scientific Committee, this is the key note lecture, not the research, that is why I did not put any results here.

Conclusion: By combination of both invasive and non- invasive techniques, I have tailored the most suitable treatment plan for patients, enabling optimal aesthetic outcomes while minimizing potential adverse effects. Continuous research and innovation in periorbital rejuvenation techniques promise to further refine these approaches, ensuring better patient satisfaction and outcomes in the future.

Orbicularis muscle folding technique in transcutaneous lower blephroplasty

Objectives: OMF technique for augmenting (and extra support) of thin anterior lamella in transcutaneous lower blepharoplasty

Introduction: Introduction: Orbicularis oculi Muscle Folding (OMF) technique is an innovative surgical method we use in lower blepharoplasty for selected patients having hollow lower eyelids due to thin anterior lamella, by using the excess part of the preseptal orbicularis oculi muscle and folding it inward on itself for auto augmentation.

Materials / method: Method: in our technique, we dissect the thin lower lid skin from the preseptal orbicularis muscle then lateral 1/3 of the separated muscle is cut vertically to use as suspension flap and we fold the rest medial 2/3 inward over itself to stitch it in 3 points to the inner surface of the muscle, finally the excess skin is discarded.

Results: Results: Through the 18 months follow up of our 25 cases the results were satisfactory and no complications observed.

Conclusion: Conclusion: We found that this technique will give the lower eyelid a natural looking tight and full shape addressing lower lid fat bulging or hollowness which is due to thin anterior lamella by the support provided with the muscle flap and it’s filling effect.

Correction of dermatochalasis with sunken upper eyelid: sub-brow excision using Brow Orbicularis Oculi Septomuscular Turnover (BOOST) flap

Objectives: We present the practical use of sub-brow excision using brow orbicularis oculi septomuscular turnover (BOOST) flap to correct dermatochalasis with sunken upper eyelids.

Introduction: Subbrow excision (SBE) is one of the most popular procedures to correct senile blepharochalasia of the upper eyelids. Unlike the upper blepharoplasty, SBE shows minimal postoperative swelling, rapid recovery to social life, and a natural postoperative appearance.

Materials / method: Patients with blepharochalasia and sunken upper eyelids were selected for BOOST flap correction. We made elliptical skin incision designs along with the lower border of brow hair or brow tattoo with a maximal diameter of 10-12mm in the lateral brow apex. Then, we removed the subbrow skin as the conventional method using a 15-blade. The BOOST flap was elevated based on the medially or inferiorly, according to the sunken area to be augmented. Following submuscular dissection, the BOOST flap was transferred to augment the sunken area and fixed using a bolster suture. The bolster was removed in 3

Results: All patients showed good postoperative results. Blepharochalasia and sunken could be corrected simultaneously without additional filling procedures such as fat graft. Swelling or bruising can be sustained relatively longer than conventional SBE. Patients complained about mild numbness postoperatively but recovered in 1 month.

Conclusion: BOOST flap is easy to use and does not need additional fat graft to correct sunken. It can correct sunken in any direction of the upper eyelid, mainly in the medial or inferior area. Surgeons can modify the amount direction of the BOOST flap according to the personal variation.

Technical refinements in upper blepharoplasty: pearls to success

Objectives: The aim of this study to present technical refinements in upper blepharoplasty in a restropective one single surgeon experience of 200 cases.

Introduction: Upper blepharoplasty is one of the most commonly performed aesthetic procedures. It deals mainly with skin excess on the upper eyelid or orbital fat hernia, issues that can be congenital or appear as a consequence of aging. The symptoms with which patients come into the office are various and unspecific from the tired-looking eyes to restricted vision. The main purpose of blepharoplasty is achieving a younger look, but also having a better eyesight.

Materials / method: A retrospective analysis was done for 200 consecutive cases of upper blepharoplasty performed under local anaesthesia by a single surgeon. Surgical plan is elaborated through an stepwise approach. Complications and grade of satisfaction among patients are noted.

Results: All the patients reported the surgery as completely painless and as a good experience they would repet it if needed. After sutures removal all patients had different degrees of edema at one week after the surgery but only minimal bruising. Three patients had wound breakdown in the following 24 hours after suture removal. No major complications was noted. Three patients had prolonged edema for 3 months after the surgery and 2 patients reported lash line hypoesthesia 1 month after the surgery.

Conclusion: Blepharoplasty is a common surgical procedure that can be done safely under local anesthesia with quick recovery and almost invisible scarring. With technical refinements and tissue preservation a natural look is created with high grade of satisfaction among patients.

Lifting hooded upper eyelid without the need of skin removal

Objectives: To introduce a novel technique to lift hooded upper eyelid without the need of skin removal.

Introduction: Upper blepharoplasty is the most common eyelid surgery to rejuvenate the upper eyelid. The surgery involves excision of skin, orbicularis oculi muscle, orbital fat. The recent trend of has been towards volume preservation where orbicularis oculi muscle and orbital fat are preserved but the mainstay of surgery still focuses on removal of excess eyelid skin.

Materials / method: A new technique is described to address eyelid hooding without the need of skin removal. The excess upper eyelid skin is tugged in by means of shifting the crease to a higher level. After determining the new crease height, 2-3 mini skin incisions measuring 3-5mm are made. Through each incision, the subcutaneous tissues are removed layer by layer to reach the levator aponeurosis. A bite of levator aponeurosis is taken and anchored to pretarsal orbicularis using Nylon 7/0, forming the new crease. Skin incisions closed with Nylon 7/0 whenever necessary.

Results: 402 patients (960 eyes) underwent minimally invasive eyelid lift by a single surgeon from Jan 2021 to November 2023. Age ranged from 18 to 69 years old with a mean age of 42. There were 366 females and 36 males in this study. The follow up period range from 1 week to 34 months. More than 90% of patients were happy with the result. There were no major complications from this procedure. The common complications were loosening of crease and asymmetrical crease where touch up surgery were done in 189 eyes (19%). Complete disappearance of crease was seen in 2 patients (0.5%).

Conclusion: Minimally INvasive Eyelid Lift (MINEL) is a novel technique to lift the upper eyelid without any skin removal. This technique of upper eyelid lifting without removing any skin has never been described in the literature. In selected cases this can be a good minimally invasive alternative for patients who are not ready for a more invasive upper blepharoplasty.
