Objectives: The aims of the presentation are to present the information that should be given to the patients and the precautions that should be taken before carrying out this procedure, the surgical technique, the results which may be expected, and the determining factors on Aesthetic Results of Lipomodeling in this particular indication.
Introduction: Breast deformations after conservative treatment of breast cancer (BCT) are a real challenge for the surgeon. No technique, before Lipomodeling, gave entirely satisfactory results. The only options offered to patients, such as musculo-cutaneous latissimus dorsi flaps, were often disabling and disproportionate to the breast deformities. But few years after treatment, patients are very eager for surgical correction of their deformity in order to erase or attenuate the visible signs of their disease. It was therefore important to seek a solution which would correct these sequelae.
Materials / method: I have a personal experience of 5000 cases of breast Lipomodeling, starting in 1998, in all indications of breast plastic surgery, Aesthetic surgery or Reconstructive surgery.
In this particular presentation we want to focus on a consecutive series of 100 cases of breast conservative treatment sequelae treated with the Lipomodeling technique from 2002 to 2010.
We have analyzed the number of sessions needed to obtain the correction, the volume of each session, the screening before surgery and the follow-up, and the number of micro-biopsies needed during the follow-up.
Results: The mean-age was 51 years (37-66). The mean BI was 21,48 (19,61-21,59). One session of Lipomodeling was done in all cases (mean volume of 176ml); 2 sessions in 35% of cases (mean volume of 175ml); 3 sessions in 5 % of cases (mean volume of 189ml).
Aesthetic evaluation showed 72% of very good result, 16% of good results, 9 % of acceptable results, and 3% of insufficient results. Patient’s satisfaction evaluation shoved 72% of very satisfied, 22 % of satisfied, 1 % of moderately satisfied, and 5 % of no satisfied patients.
Determining factors of aesthetic results are analyzed.
Conclusion: The lipomodeling technique represents a major advance for the correction of Breast Conservative treatment sequelae.
This technique must be done by a plastic Surgeon with a specific training. The learning curve is relatively important and allows a good analysis of the determining factors of good results.
A good pre-operative screening is crucial, as well a good post-operative follow-up. Close collaboration with skilled radiologists is essential.
Plastic Surgery consultation must be now systematically proposed after BCT.
