Efficacy and Safety of Pegylated Hyaluronic Acid Dermal Filler Injections in Patients with Titanium Facial Implants: A Case Report

Objective

To evaluate the safety and efficacy of Pegylated Hyaluronic Acid (PEG-HA) dermal filler injections in enhancing facial contour in patients with titanium facial implants, focusing on the unique challenges and considerations in this patient population.

Background

Titanium implants are commonly used in craniofacial surgery for their strength, stability, and biocompatibility. However, they can pose challenges in aesthetic procedures, particularly in dermal filler injections, due to altered tissue dynamics and potential for visibility under thin skin. This case report examines the application of PEG-HA fillers, known for their biocompatibility and long-lasting effects, to enhance facial contour tightness while maintaining safety and minimizing complications.

Methods

A 28-year-old male with titanium implants in the mandible, chin, and maxillary bones underwent facial contouring using PEG-HA dermal fillers. The treatment focused on enhancing the jawline and mid-face contour while carefully considering the implant positions. A blunt cannula technique (30 mm size 27G) was used to ensure precise placement and minimize trauma. Clinical outcomes were monitored at 2.5 weeks and 15 weeks post-treatment.

Results

The patient demonstrated significant improvements in facial contour with enhanced definition and a more balanced aesthetic appearance. No adverse effects, including erythema, edema, or implant visibility, were observed. The patient reported high satisfaction with the natural-looking results, and the filler maintained its effect at the 15-week follow-up.

Conclusion

PEG-HA dermal fillers can be safely and effectively used in patients with titanium facial implants, offering enhanced contour and aesthetic improvements without adverse reactions. The use of a blunt cannula technique minimizes complications, ensuring precise placement around implant areas. This case supports the potential of PEG-HA fillers as a safe and effective option for patients seeking facial rejuvenation with pre-existing facial implants.
