Objectives: In this study, we tried to restore the perioral "barcode" wrinkles with a combination treatment of PDO threads (polydioxanone) with mesobotox, which offered a natural result. Introduction: Signs like lengthening and sagging of the upper lip as well as Cupid's bow and philtrum flattening appear along with the aging of the perioral region.” Barcode” wrinkles present a very common aesthetic issue affecting most women after the age of thirty. Genetics, sun exposure, and smoking/vaping are the strongest contributors to the development of these wrinkles. The main reason that this region remains often untreated is the unnatural result after filler injections. Materials / method: We treated 87 patients, all women after the age of 35, with positive smoking habits and sun-damaged facial skin, suffering from deep vertical perioral lines. Their main point of interest was the disappearance of wrinkles without adding volume at the perioral area. The treatment protocol included 10 PDO threads 30g 25mm and 1 ml mesobotox. The threads were inserted parallel to the lip vermillion border, crossing the vertical wrinkles at both sides. The mesobotox cocktail consisted of 10 units of onabotulinum toxin mixed with a 1ml mesotherapy solution of non-crosslinked hyaluronic acid and Results: The first evaluation was performed 2 weeks post-treatment. All 87 patients were satisfied with the result, especially due to the natural-looking appearance. Minor adverse events like light bruises (high vascularization due to smoking) as well as light oedema subsided after the first 24 hours. All patients were prompted to repeat the treatment after 3 months to maintain the desired result. Conclusion: Thread-mesobotox shows excellent results for the treatment of “barcode” lines, providing a natural look with almost no downtime. This favorable combo- treatment can be repeated in 3-month intervals to maintain the result.
