Improving static & dynamic appearance of facial paralysis with suspension sutures
Objectives: With different cases there are multiple treatment modalities exist. The most challenging ones are facial paralysis. Objective of this study is to use multiple threads suspensions to improve functional and aesthetic concern of patient.
Introduction: Facial paresis is a condition associated with loss of VIl cranial nerve function. The facial nerve has an intricated course from the brain to the periphery. It consists of afferent somatic efferent, parasympathetic and branchiomotor fibres. It innervates up to 20 muscles of the face and head.
The severity of facial paresis manifestation depends on the underlying cause Facial reconstructive surgery approaches treating severe facial paresis can be divided in two group groups static and dynamic. The dynamic treatment approach address's function while static treats facial deformities.
Materials / method: H/O 42 year’s old male patient was diagnosed in 2019 Bell’s palsy on left side of face. he lost his muscle tone. Plan was to do mid facial improvement (L) side angle of mouth and NLF improvement relaxing the opposite side with neuromodulators Only Static need to be addressed as dynamic asymmetry is less, hence plan was to achieve (L) N/L and drooping angle of mouth improvement with suspension sutures suspending by looping of absorbable bidirectional PCL/PLA threads in hypodermic tissue along with overall harmony of face with multidirectional PCL/PLA sutures coated with HA.
Results: Minimal invasive methods procedure correct both functional and cosmetic problems effectively in patients with facial paresis. Satisfactory facial symmetry is obtained without surgical interventions, results in combination can lasts up to 2 years but due to muscle inactivity result can stay longer
Conclusion: The static facials deformities and asymmetries can bring psychological depression in patients with facial paralysis. Many patients are not willing to undergo invasive procedure. Therefore, with minimal rehabilitation time with plausible and long lasting results can do wonders to fixing both function and aesthetic needs with minimal risk and results
