Endoscopic breast augmentation with implant in loco-regional anesthesia: personal experience
Objectives: The goal of our study was to develop a safe technique under local anesthesia, minimally traumatic with preparation of the pocket through the use of the endoscope and fast which would allow patients to return to normal life activities in 24-48 hours .
Introduction: According to data reported by the ISAPS (International Society of Aesthetic Plastic Surgery) in the year 2020, breast augmentation surgery with prostheses was the most performed in the world with over 1,600,000,000 procedures. In Italy alone, over 39,000 procedures were carried out in the same year. The increasing demand from women combined with the need to have an ever faster recovery and minimize the risks of the intervention, including anesthetic ones, has led us to develop a local-regional anesthesia technique that could allow us to guarantee maximum safety with a fast recovery.
Materials / method: The BANP (Block Anesthesia Nerve Pectoralis) technique of loco-regional anesthesia allows for the execution of breast augmentation surgery in safety. By the use of an endoscope we prepare the breast implant pocket by a minimal access (< 2cm) on the inframammary fold. Inclusion criteria: women aged 18 to 65, BMI in the normal range of 19-23, with first breast size and wishing to obtain a third size with ergonomic prosthesis in the 240-330 cc range. All patients undergo dual plane technique sec. Tebbets with incision in the sub-mammary sulcus.
Results: From January 2021 to July 2021, of the 216 women who underwent breast augmentation surgery, 100 met the inclusion criteria and underwent BANP surgery in day surgery. No complications occurred after the surgery. The average age was 36.5 years; the average duration of the intervention was 56 minutes; patients were discharged from the clinic after an average of 3.4 hours. All patients completed the 6-month follow-up and no complications were reported. Patient satisfaction assessed on the operating experience was excellent for 86% and good for 14%.
Conclusion: the BANP technique (Block Anesthesia Nerve Pectoralis) is a safe loco-regional anesthesia technique and allows to reduce the risks associated with general anesthesia and allows a more complete recovery of normal daily activities after breast augmentation surgery
