Objectives: Defining the advantages of combining fat grafting with mastopexy operations (composite mastopexy) and presenting our experience and outcomes with the technique. Introduction: Mastopexy and mastopexy with augmentation are the most challenging surgeries in aesthetics. Combining fat grafts with these surgeries helps to achieve more symmetrical and aesthetic results. Materials / method: Presentation of different cases who undergone mastopexy and mastopexy with augmentation surgeries. Comparison of the before and after photographs and 3D images of the patients. Results: Postoperative results of the patients show symmetrical improvement of the breasts. Good tissue quality and coverage on the upper poles regarding fat transfer. Also the implant edges are not visible considering we mostly used round implants in subfacial plane. We can also avoid the complications of anatomical implants and subpectoral plane such as implant rotation and waterfall deformities. In difficult cases with severe asymmetry and tubular deformity fat grafting provides predictable and natural results. Conclusion: Composite mastopexy with or without augmentation is a reliable and safe method which improves the outcomes regardless of implant type and placement plane. It is also an option for patients who have ptotic but small sized breasts and do not want implants. We highly recommend this technique to optimize the results, lower the revision rates and improve the patient satisfaction after operation.
