Bespoke fat grafting: adjustable regenerative adipose tissue transfer

Introduction: Filling applications are one of the most frequently performed minimally invasive procedures in aes- thetic plastic surgery. Although hyaluronic acid and similar ready-made products are often preferred, their high complication rates and the need for repetition have led surgeons to apply more fat grafts recently. The two most common complications in fat graft applications are:
1. Uncertainty of how long it will remain.
2. Visibility. A new paradigm is needed to prevent them.
Materials / method: In this study, using ultra-sharp blade systems, stromal cells in the adipose tissue were released and it was possible to prepare fat grafts in different sizes between 2,400 microns and 200 microns. In this presentation, the long-term results of the patients who were applied to 235 patients in different parts of the body are shared. Different sizes of fat grafts were prepared for different anatomical areas and different depths
Results: In adipose tissue, parenchymal and stromal cells are connected to each other by very tight bonds. Without killing the parenchymal cells, these bonds can be released with ultra-sharp blades and likewise the desired size can be given to the adipose tissue. The cutting of fat tissue with ultra-sharp blades is called “adinizing”. Liberated stromal cells switch to active mode and provide more retention. Similarly, obtaining adipose tissue in different diameters according to the desired anatomical area prevents visibility complications and allows natural results
Conclusion: Hyaluronic acid and similar applications are “confection” style approaches, while different sizes of fat tissue are taken with different diameter cannulas and used as a graft is “custom-made” tailoring. However, a personalized approach for each anatomical area and depth is “bespoke fat” grafting. This procedure will bring a new dimension and perspective in fat grafting applications in plastic surgery. The procedure is, in a way, the “Activated fat graft” application.
