Paradoxical effect: increase in penile length secondary to restructuring of the Dartos fascia by circumferential injection of HA dermal filler
Objectives: Evaluate the efficacy of hyaluronic acid dermal filler injection in patients not satisfied with their penile aesthetics. Also, to assess changes in penile dimensions, including length, diameter and shape, along with patient satisfaction and follow-up outcomes.
Introduction: Penile dissatisfaction due to size can significantly impact self-esteem. Minimally invasive techniques like hyaluronic acid (HA) dermal filler injections gained popularity for penile augmentation. HA injections is promising in improving penile shape, girth and length, addressing issues like small penis syndrome or patient unsatisfaction with penis size and/or shape.
Materials / method: Fifty-four patients underwent penile augmentation with hyaluronic acid filler. Injections were performed under the Dartos fascia using predetermined volumes accordingly to patients’ individual needs. The product was a multiphasic HA dermal filler with a high G-prime and R2 technology. The filler was applied circumferentially under the Dartos fascia, administered at a ratio of 2.09 ml/cm. Another filler with lower G-Primi and IPN-Like technology was used to supplement the treatment applying an average of 0.46 ml/cm. Dimensions, adverse events, and function changes were monitored.
Results: The procedure applying HA dermal filler injections for penile augmentation yielded highly satisfactory outcomes with increase in penile dimensions and improvement on shape. Patient reported significant contentment post-intervention.
Interesting to note that although the filler is applied circumferentially increasing penile girth, there is a paradoxical effect and it is also observed an increase in the length of the penis. In one case, per example, a patient measured at baseline (flaccid penile) a length of 4.3 cm and diameter of 2,4 cm had a notable increase to 7.8 cm in length and 3.5 cm in
Conclusion: HA dermal filler injections for penile augmentation led to highly satisfactory outcomes, increasing both penile dimensions and shape, with patients expressing significant contentment post-procedure. Interestingly, despite primarily targeting girth, a paradoxical effect was observed, resulting in increased length as well. For instance, one patient initially measured 4.3 cm in length and 2.4 cm in diameter, which notably increased to 7.8 cm in length and 3.5 cm in diameter post-intervention. No adverse events were observed, and patients reported no functional complaints.
