Refining photoaging therapy: Superior efficacy and safety with combined picolaser 1064nm and thulium 1927nm laser
Objectives: To unlock the full potential of combined Laser therapy for photoaging using 1064nm Picosecond Laser and 1927nm Thulium Fractional Laser in improving skin texture, wrinkles, spots, and pores, while reducing downtime and enhancing overall treatment outcomes.
Introduction: Photoaging is a major factor in skin aging, responsible for up to 80% of visible skin changes. The growing demand for effective treatments has led to the rise of Picosecond lasers, known for their promising results. However, optimal outcomes often require high fluence, increasing downtime and safety concerns. The Thulium 1927nm laser, a fractional non-ablative device, stimulates collagen and reduces melanin while preserving the epidermis, promoting faster recovery. Combining Picolaser 1064nm and Thulium 1927nm offers a more efficient and safer approach to treating photoaging.
Materials / method: Twenty Asian participants with Fitzpatrick skin types III–V underwent three sessions of combined 1064nm Picosecond laser and 1927nm Thulium fractional laser treatments at 4-week intervals. The Picosecond laser was applied using a two-step method with zoom and fractional handpieces, followed by the Thulium laser with a roller handpiece. Both treatments targeted the entire face. Standardized photographs were taken before the first session and 4 weeks after the final session.
Results: Improvements were observed across all key photoaging features, including skin texture, wrinkles, spots, and pores. These changes were confirmed using VISIA imaging, comparing follow-up measurements taken 4 weeks after the final treatment with baseline assessments.
Conclusion: The combined use of 1064nm Picosecond Laser and 1927nm Thulium Fractional Laser effectively improved photoaging-related concerns, including skin texture, wrinkles, spots, and pores. This protocol demonstrated enhanced efficacy while reducing downtime, offering a safe and efficient treatment option for managing photoaging in Asians.
