Refrashing patients aspects: SMAS lifting with HA
Objectives: The aim of our treatments is to improve our patients' appearance, even better if we can achieve a natural-looking result that refreshes the face without altering it. When used wisely, hyaluronic acid can give us beautiful results.
Introduction: Hyaluronic acid has always been used in the middle third of the face. Initially, it was used in boluses to pull the tissues, but later, with improved anatomical studies, we used techniques that combine the filling of deep fat pads and positioning them under the skin to lift the middle third of the face, also volumizing the area. But the question that many patients have asked me is: "Is it possible to lift the face without adding volume?"
Materials / method: The author used hyaluronic acid with medium-high G. The treatment is performed using a 25G, 50mm cannula. The cannula entry point corresponds to the intersection of two lines. The first connects the tail of the eyebrow with the mandibular angle, and the second connects the line corresponding to the direction of the zygomatic arch. According to the retrograde technique, 0.2 cc of hyaluronic acid is injected along three lines from the entry point. The first line is towards the pyramidal fossa, the second is at the midpoint of the nasolabial fold, and the third is towards the modiol.
Results: The results are always improving and incredibly natural. Through ultrasound and anatomical dissection, the author demonstrates how the injection is not made into the subcutaneous layer but deeper into the SMAS. This enhances the superficial fat pads, which are able to lift the tissues of the middle third of the face.
Conclusion: Hyaluronic acid is always used to improve the middle third of the face. The aim of the treatments should be to improve and refresh the appearance of our patients without changing them. This technique has proven to be effective in improving appearance, but also in minimizing the use of hyaluronic acid, which does not excessively volumize the face but only lifts it gently without weighing down the tissues.
