The Diluent Dilemma: Investigating Hypersensitivity in Bacteriostatic Botulinum Toxin Preparations
目标: Although widely used, bacteriostatic normal saline may trigger inflammatory reactions after BoNT-A injections, even in patients with a negative patch test.
介绍: The global application of botulinum toxin type A (BoNT-A) continues to expand rapidly, highlighting its established role in both aesthetic and therapeutic medicine. In the United States, more than 2.3 million BoNT-A procedures were performed in 2019 alone, representing a 60% increase since 2012.
BNS, commonly used off-label to reconstitute BoNT-A, contains 0.9% benzyl alcohol, a known contact allergen. Despite FDA recommendations to use PFS, its anesthetic properties and multidose convenience have led to widespread clinical use. 80% of aesthetic physicians reconstitute BoNT-A with BNS.
材料 / 方法: A 37-year-old woman presented to our hospital complaining of angioedema in the face, two days after onabotulinumtoxinA injections in the forehead and crow’s feet
结果: Although widely used, bacteriostatic normal saline may trigger inflammatory reactions after BoNT-A injections, even in patients with a negative patch test.
总结: Clinicians should remain alert to immediate hypersensitivity responses, particularly in individuals with a history of fragrance allergy or presenting with acute urticaria or angioedema. In such cases, close monitoring and a low threshold for switching to preservative-free saline are warranted.
