Tubular Gynecomastia Correction Using Ultrasound-Assisted Liposuction and Gland Excision Through a Minimal Incision Technique
Objectives: To share a minimally invasive technique for correcting tubular gynecomastia using ultrasound-assisted liposuction and gland excision through a lateral areolar incision, with a focus on achieving contour improvement and high patient satisfaction while minimizing scarring. The session will highlight patient selection, outcomes, and the avoidance of traditional peri-areolar incisions.
Introduction: Tubular gynecomastia presents aesthetic and psychological challenges. Traditional correction methods often involve extensive peri-areolar incisions, which many patients are reluctant to accept. This study explores the feasibility and outcomes of using a minimal-incision approach combining ultrasonic liposuction (VASER) with targeted gland excision, offering an alternative with reduced visible scarring.
Materials / method: A retrospective case series of 10 patients with tubular gynecomastia treated between 2010 and 2025 using ultrasound-assisted liposuction (VASER) and gland excision via a minimal lateral areolar incision. Only patients who refused traditional or peri-areolar incisions were included. Patients seeking extensive excision were excluded. Outcomes were evaluated based on contour improvement, satisfaction, and complications.
Results: All patients demonstrated visible correction of the tubular chest shape. Eight reported high satisfaction, while two were moderately satisfied but declined additional procedures for residual areolar fullness. No complications such as hematoma, seroma, or contour irregularities occurred. Scars remained minimal and well-concealed at the lateral areolar edge.
Conclusion: Minimally invasive correction of tubular gynecomastia using ultrasound-assisted liposuction and lateral areolar gland excision can offer excellent results in carefully selected patients. This technique achieves significant contour improvement with high patient satisfaction and minimal scarring, and is an appealing alternative to conventional methods for patients avoiding visible incisions.
