Facial reshaping (peripheral approach)
Objectives: To present a structured peripheral facial contouring approach that prioritizes lateral and posterior support to restore youthful facial proportions while minimizing central overcorrection.
Introduction: Over the past decade, central midface volumization has often been used to create lift and soften nasolabial folds. However, excessive filler placement in the medial cheek and central face may result in unnatural “apple cheeks,” midfacial puffiness, and facial overfilling. A shift toward shape-based, peripheral volumization may offer a more natural and anatomically sound alternative.
Materials / method: The technique focuses on volumization of the posterior temple (supra-auricular region), anterior temple in moderation, lateral cheek, preauricular area, gonial angle, post- and prejowl sulcus, and chin. Emphasis is placed on posterior temporal augmentation using a 25G 40 mm flexible cannula and high G′ hyaluronic acid fillers. Anatomical considerations include targeting either the subdermal plane (layer 2) or interfascial plane (layer 4), while respecting the superficial temporal artery located primarily within the superficial temporal fascia (layer 3). Results: Peripheral volumization broadens the upper face, creating an optical illusion of a narrower lower face and restoring a youthful oval shape. Clinical cases demonstrate improved contour definition, softer transitions, and reduced detectability of filler compared to central volumization strategies.
Conclusion: The peripheral approach represents a shift from medial filling toward structural lateral support. When performed with anatomical precision and appropriate product selection, it allows efficient rejuvenation with natural outcomes and lower risk of visible overcorrection.
