Objectives: NAC reduction in correction of gynecomastia to have a predictable outcome. Introduction: Technical approach to surgical correction of gynecomastia has changed a lot over last decade. Different technique by different authors have been tried to give a better and consistent aesthetic outcome. Liposuction has been the core entity in all approaches to correction of gynecomastia. Gland removal has been tried through the same port of liposuction or incision through nipple areola complex (NAC). Materials / method: The main area of concern remained is excess skin especially in grade 3 gynaecomastia. Though waiting for skin to contract or using energy-based devices during/after surgical correction have been tried but result is not predictable. NAC reduction by circum-areolar de-epithelialisation and purse string suture has produced consistent and predictable result with better aesthetic outcome. Moreover, it makes gland removal easier and more précised.
