Managing the GLP-1 facial phenotype: a layered regenerative approach to volume and laxity restoration
Objectives: This session presents a structured, layered regenerative strategy for managing the GLP-1 facial phenotype, characterized by rapid adipose deflation, dermal laxity, and structural imbalance following significant weight loss. Integrating facial biotype assessment, vector-based analysis, and layer-specific intervention, the approach restores deep support, midface volume, and dermal integrity. The aim is controlled structural regeneration to rebalance volume–laxity dynamics and reestablish facial harmony beyond simple volumization.
Introduction: GLP-1 receptor agonist–associated weight reduction frequently produces accelerated facial deflation distinct from chronological aging. Patients demonstrate midface volume loss, relative dermal redundancy, and envelope-support mismatch. Conventional volumization alone risks overcorrection. A biologically guided, anatomy-based regenerative sequence is required to restore structural integrity and tissue balance.
Materials / method: Patients presenting with GLP-1–associated facial deflation underwent a three-layer regenerative protocol:
(1) deep structural restoration using bio-regenerative dermal HA fillers placed at anchor points and deep compartments;
(2) compartment-specific volumetric rebalancing;
(3) energy-based dermal stimulation to induce collagen remodeling and improve laxity.
Assessment included facial biotype profiling, vector mapping, 3D volumetric imaging, GAIS scoring, elasticity measurement, and skin texture analysis at D0, D14, D30, D90, and D180.
Results: Patients demonstrated restoration of midface projection, reduction in visible laxity, improved contour continuity, and enhanced dermal firmness and radiance. The layered sequencing achieved proportional correction without volumetric exaggeration, with high satisfaction and minimal downtime.
Conclusion: The GLP-1 facial phenotype requires a regenerative, layered strategy addressing both structural deflation and dermal envelope integrity. Sequenced restoration provides anatomically coherent, natural, and durable outcomes in rapid weight-loss patients.
