Objectives: Acne conglobata is a very severe form of acne, but seems different from the 2 other forms of severe acne: nodulocystic acne and acne fulminans, which are both complications of acne vulgaris. In the case of conglobata, the clinical features (abcesses, tracts, inflammatory lesions, trunk often involved) seem to be close to what is observed in forms of hidradenitis suppurativa. Maybe acne conglobata is not really acne, but should lead physicians to look for associated manifestations of hidradenitis suppurativa.

Objectives: Show Nd:YAP 1340nm laser efficacy in treating hidradenitis suppurativa.

Introduction: Hidradenitis is a chronic and recurrent inflammatory disease that affects flexural areas of the skin, such as the armpits, and the inframammary and groin regions. It is a painful and often disfiguring condition, which manifests after puberty and is characterized by deep inflammatory lesions in the region of the apocrine glands. There are many options for treatment but a few show efficacy. We present an excellent option for Hurley´s stages I and II

Materials / method: More than fifty patientes with hidradenitis at Hurley’s Stages I or II, were treated in our practice using this technology. We used Nd:YAP 1340nm laser one session per month, average 7-20 sessions.

Results: More than eighty percent of the treated patients got good to excellent results

Conclusion: Nd:YAP 1340nm laser is an excellent option to treat hidradenitis at Hurley’s Stages I or II in any kind of skin.

Objectives: HS in 2020 must be considered as an AI and inflammatory disease

avoiding mutilating surgery through conservative surgery and lasers
avoiding long term general treatments bringing no benefits and loads of side effects
favoring local treatments and an anti inflammatory nutritionnal approach

Introduction: Chronic inflammatory skin disease localised to intertriginous areas with a severe impact on the patient’s quality of life, personal relationship and self-esteem. It shouldn't be considered as a local disease but as an Auto Immune and inflammatory general disease that can be controled by nutrition. A conservative surgery as well a laser treatment can prevent the mutilating surgery and the long lasting general treatments that have been performed on those young patients.

Materials / method: TCA solutions can be of use to sterilize abscesses after surgery and induce a faster recovery, to treat recurrent fistulas, to reduce scars and to improve the quality of the damaged skin in the involved areas.
Anti inflammatory nutrition avoiding gluten and everything that can lead to leaky gut syndrom which triggers inflammation and immunity
Nutritionnal supplementation to fight the inflammation, to restore the immunity and the intestinal flora.
Hair removal laser treatment leading to a destruction of the apocrine glands linked to the hair follicule.
Scar treatments for the mutilated patients

Results: With conservative surgery, no recurences on the treated lesions.
Areas treated by hair removal laser show less and less recurences.
Patients following the nutritional council and supplementation, even when not treated with lasers, show a general improvement, less and less recurences, and no more lesions after one year as well as an improvement of the associated symptoms and diseases.
Relapses can happen when the nutritional councils are discontinued

Conclusion: Current treatments like long term antibiotherapie and biotherapies are not satisfying in term of results and side effects.
The recommanded surgery can lead to mutilations.
Conservative surgery can be of use as long as the lesions still appear.
Anti inflammatory nutrition and supplementation lead to total remission but must not be discontinued.

Objectives: The objective of the topic was to evaluate the importance of hair removal in the treatment of patients affected by hidradenitis suppurativa.

Introduction: Since 1996, lasers and IPL have been used for hair removal mostly for cosmetic reasons. However, in dermatology hair removal has also been used for the treatment of diseases, such as hirsutism, hypertricosis, and for hair-bearing skin grafts. Hidradenits Suppurativa (HS) is a chronic disabling disorder, with exacerbations, recurrence, and progression despite medical and extensive surgical treatment. Histologic studies have suggested that HS is a disorder of the terminal follicular epithelium with follicular hyperkeratosis and obstruction acting as an early event in the pathogenesis of HS.

Materials / method: PubMed was searched using terms ‘hidradenitis suppurativa,’ ‘acne inversa,’ ‘apocrine acne,’ ‘apocrinitis,’ ‘hidradenitis axillaris,’ and ‘Verneuil’s disease’ and laser hair removal. Only studies written in English were included. Case reports and case series were included when larger trials were pending or unavailable.

Results: The treatment goal for the hair removal lasers and light devices is, therefore, to destroy the hair follicles and thereby reduce the disease activity. Significant therapeutic benefit has been reported in this condition after LHR with diode laser, Nd:YAG laser, alexandrite laser and IPL devices. Removal of hair with lasers and light improves the HS symptoms in several studies, but repeated treatments seem to be necessary. Long-term follow-up is lacking.

Conclusion: Although Laser Hair Removal started as a cosmetic modality, it is fast gaining significance as a therapeutic modality in wide range of dermatological and surgical indications. Laser Hair Removal is safe and well tolerated even in pediatric population. It has reduced the morbidity associated with various chronic and recurrent disorders and thus improved the quality of life of these patients.
