Background

Facial aging in Asian populations differs considerably from that in Caucasian patients. Asian faces frequently demonstrate soft-tissue hypertrophy, heavier facial fat compartments, and progressive descent of the medial and lateral facial fat pads. Thread lifting has emerged as a valuable minimally invasive technique for repositioning descended facial tissues. However, optimal outcomes depend on accurately identifying whether facial aging is predominantly caused by tissue descent, volume loss, or a combination of both.

Case Series

Three distinct clinical patterns were identified:

Group 1: Predominant Tissue Descent:

Patients demonstrated descended medial and lateral facial fat pads, jowl formation, and loss of jawline definition, but preserved midface volume. These patients underwent cog thread lifting alone, which aimed at repositioning descended tissues toward their youthful anatomical position.

Group 2: Predominant Volume Loss

Patients demonstrated thin facial morphology, midface deflation, and pyriform fossa deficiency with minimal tissue descent. These patients were treated primarily with hyaluronic acid fillers targeting structural support and volume restoration. Thread lifting was intentionally avoided.

Group 3: Combined Descent and Volume Loss

Patients demonstrated moderate-to-severe tissue descent with concurrent upper and midface volume depletion. These patients underwent hybrid rejuvenation using cog threads and hyaluronic acid fillers to restore upper and midface volume.

Results

Patients in Group 1 showed significant improvement in jawline definition, lower-face contour, and lifting of descended tissues following thread lifting alone. Patients in Group 2 achieved substantial aesthetic improvement following volume restoration with fillers. The most comprehensive and natural-looking rejuvenation was observed in Group 3, where hybrid treatment simultaneously corrected tissue descent and volume deficiency.

Conclusion

This case series highlights that thread lifting is most effective in heavier faces with preserved volume and significant fat pad descent. Fillers alone are most beneficial in patients with predominant volume depletion and minimal ptosis. Careful facial assessment and individualized treatment planning remain the cornerstone of predictable outcomes and high patient satisfaction.
