Abstract

Objectives: Reposition diode endolaser as a mature, surgeon-controlled platform to complement fat-based surgery by addressing the “tension/architecture” component of contouring (beyond volume removal or transfer). Introduction: In fat grafting, we restore volume and improve trophicity, but final contour depends on how volume is supported by the subcutaneous fibroseptal framework. In face/neck and body contouring, structural modulation can enhance results while remaining minimally invasive. Materials / method: Perspective talk built on: (1) mechanistic rationale (980 vs 1470 nm targets), (2) device-delivery concepts (radial fiber, aiming beam/pilot light), and (3) comparative evidence across energy modalities. A systematic review + meta-analysis (laser/RF/ultrasound/plasma) is referenced, with full conclusions presented on Saturday. Results: 1470 nm targets water-rich connective tissue for controlled septal heating and contraction; 980 nm supports hemostasis and adipose emulsification. Radial fibers improve homogeneity (fewer hotspots), and the pilot light enables real-time tip localization. Comparative data include split-body evidence of greater shrinkage with LAL; safety varies across devices. Conclusion: Endolaser helps “master tension” by treating the connective scaffold in synergy with liposuction (volume reduction) and fat grafting (volume restoration + trophicity). As a mature, precise, minimally invasive surgical tool, its value depends on training, indication, wavelength/fiber choice, and staying in the right plane.
