Objectives: To study the aesthetic outcome of various techniques of Genioplasty such as sliding Osseous Genioplasty and Alloplastic chin augmentation via Implants like High Density Poly-ethylene (HDPE), Poly-ether-ether-ketone (PEEK), Silicon (Si) and Hydroxyapatite (HA) implants.
Introduction: Chin is the pivotal feature in harmonizing the overall facial appearance. 1st Genioplasty dates back to 1934 when Aufricht utilized nasal hump for chin augmentation. Over the years, various techniques of Genioplasty have been revolutionized through innovations in digital imaging and 3D virtual surgical simulations that has enhanced surgical precision and predictability. We present our institutional techniques of Genioplasty via Osseous and Alloplastic Implants such as High Density Poly-ethylene, Poly-ether-ether-ketone, Silicon and Hydroxyapatite implants.
Materials / method: The study was conducted over a period of 20 months from August’23 to March’25 at Sir Ganga Ram Hospital, New Delhi, India. Pre-operatively, a 3D virtual simulation of expected post operative outcome was generated. Of 15 patients who desired chin augmentation for Aesthetic purpose, 4 underwent sliding osseous genioplasty and remaining chose chin implants. The choice of implants included High Density Poly-ethylene, Poly-ether-ether-ketone, Silicon and Hydroxyapatite implants in 4, 3, 3 and 1 patient respectively. Patients were evaluated for complications 6 months after surgery.
Results: Of 15 cases, 9 were males and 6 were females, in the age group of 18 to 35 years. All Genioplasty techniques were done using intra-oral approach. Maximum horizontal and vertical projection achieved was of 8mm and 7 mm in patients of Silicon and High Density Poly-ethylene chin implant respectively. There was no major complication noted like Neurosensory deficit, extrusion, or wound dehiscence. All patients were satisfied with their aesthetic outcomes except one patient who had slight asymmetry due to implant migration 1 month postoperatively.
Conclusion: The described Genioplasty techniques, be it Osseous or via Alloplastic Implants achieve good outcomes with high patient satisfaction. Pre operative 3D virtual simulation of results is important to have predictable results. Deep knowledge of surgical technique is fundamental to minimize complications. Caution is required to avoid potential under-correction or over-correction.
