Objectives: 1. The diagnosis was unknown filler in breast with skin ulceration
2. The way to use the water-jet system assisted surgery for removal of unknown filler in breast with skin ulceration
3. Introduce the characteristics of breast tissue of patients after breast augmentation surgery with high volume of unknown injection, and how to use water-jet system to protect the thin epidermis while accurately flushing the subcutaneous adhesive artificial materials, so as to maximize the protection of the blood flow of breast skin, nipple and areola.
Introduction: After injection of artificial filler, due to the existence of a large number of artificial materials under the skin of the breast and subcutaneous tissue separation, and the nipple, areola and breast skin blood supply becomes very fragile, the use of scratching, simple surgical excision and other conventional methods are easy to lead to skin blood supply disorders, it is difficult to avoid the nipple, areola and thinning area skin necrosis. However, irrigation alone cannot completely remove the artificial materials injected under the skin, resulting in residual injection materials.
Materials / method: Cases of skin rupture after injection of unknown artificial material were removed by surgical excision combined with water-jet system. The impact of water can effectively remove the artificial materials scattered in the superficial layer and part of the deep layer, and protect the blood supply of breast skin, nipples and areola.
Results: Postoperative blood supply was good in bilateral nipples and areas. the incision healed in one stage, and there were no complications such as hematoma, effusion and infection. Three months after the operation, MRI review showed that the injection materials had been completely removed and no subcutaneous injection materials remained. The patient was satisfied with the therapeutic effect and had no complaints of discomfort.
Conclusion: For repeated infection after injection of breast augmentation, injection level is too shallow and other conditions should be removed as soon as possible, so as to avoid skin rupture. For the injection materials in the superficial subcutaneous layer, the water-jet system may be a good means to effectively remove the injection materials and avoid the destruction of blood supply to the skin, as well as the deep tissue without nodular wrapping.
