Aging Lips Filler: "how to enhance, not erase"

Objectives: To describe the special technique to achieve natural looking lips in advanced age patient.

Introduction: The lips are considered as a key element of facial attractiveness due to their central position and their elemental role in verbal and non-verbal communication. Young age and advanced age patients commonly demand lip augmentation, but their specific purposes could be variety. Overfilled lips could lead to unnatural looking especially in advanced age patients.

Materials / method: To approach Aging Lip filler, you should comprehend 3 issues: anatomy, rheology, and technique.

Results: First, aging lips have more elongated, less defined vermillion borders. The loss of volume and muscular tone produces rhytids of dry cutaneous lips. Even though the loss of volume is easy to treat by filler but overfilled this area could lead to “duck lips”. Second, lip fillers are different in their viscosity and elasticity, so choosing the suitable filler is significant. Lastly, every patient has their unique lip structure and ideal definition. The injector should have expertise and knowledge to comprehend the patient’s desire. Patient natural smiling depends on the quality, correct filler,

Conclusion: Aging lip fillers should enhance the natural beauty, not erase all the aging signs. Lip enhancement requires knowledge of anatomy, filler rheology and expertise techniques.

Lip beautification with two rheological properties hialuronic acid

Objectives: This work presents and explores an advanced lip filling technique with two types distinct from hyaluronic acid, discussing benefits, limitations and clinical considerations. Aspects related to safety, patient satisfaction and consistent aesthetic results are addressed, highlighting the effectiveness of this procedure.

Introduction: Among the techniques used for this purpose, lip filling stands out, an approach minimally invasive and highly effective to provide fuller, more defined lips harmonious. Lip filler emerges as an innovative and versatile solution to these concerns aesthetics, with significant advances in recent decades in terms of materials and techniques.  Individualized approaches are essential to avoid artificial or disproportionate results.

Materials / method: Two types of hyaluronic acid with different rheological properties were used, the first being denser hyaluronic acid (which we named AH1) and the second less dense (which we named AH2).  The technique involves injecting the highest density hyaluronic acid (AH1) to create the contour, and the lowest density hyaluronic acid (AH2) to generate volume in lips that do not have a structured contour and very thin lips. For lips that need an increase in volume, and that already have a defined contour, we use the low-density product for the contour and the high-density product for the vermilion.

Results: When we use this technique with two fillers with different rheologies, we have several benefits, which combine aesthetics and function. We have greater safety through the use of the cannula, and the use of these different fillers can avoid palpable nodules resulting from the accumulation of hyaluronic acid. This way, we also avoid an inflated appearance, which would not be natural. And when we distribute properly filler material, we avoid asymmetries in the lips, leading to a more regular, more harmonious and elegant.

Conclusion: This technique has the advantages of better precision in the lip contour, leaving it more defined and preventing nodules from forming in the vermillion region of the mouth, minimizing the risks problems related to the procedure. Through this technique, we combine aesthetics and function optimized. This way, we have a safer, more beautiful and harmonious result when associated with the two rheologies of different hyaluronic acids.

Multilayer Dermal Filler treatment of the Temple-Ultrasound guided safest methods

Objectives: Temple area is one of the most challenging area to treat with dermal fillers. it has been always a debate between professional aesthetic practitioners whether Dermal fillers are better injected on the Periosteum or into the Interfascial space or Subcutaneous plane. With the guide of high frequency/high resolution Ultrasound all practitioners will be able to master their anatomical knowledge, Injectable techniques and secure their safe approaches.
As all Medical specialities, the future of aesthetic medicine require the most advanced technologies to help aiming those goals.

Conclusion: The aim of this lecture to introduce the safest methods, tools and techniques for the treatment of Temple area with dermal fillers. And also how to achieve the patient's desire with the help of ultrasound facial assessment, facial mapping and securing the techniques.

Targeted Contouring: Integrating Biostimulators and Lipolytic Agents in the Treatment of Facial Fullness in the Asian population

Objectives: Objectives:
1.To evaluate the effectiveness of using biostimulators alongside lipolytic agents to refine facial contours in Asian patients experiencing facial fullness.
2.To evaluate patient satisfaction and safety of the integrated treatment approach.
3.To document any changes in skin texture and overall facial aesthetics.

Introduction: Facial fullness, particularly prevalent among individuals of Asian descent, can impact aesthetic balance and confidence. Conventional treatments typically require invasive techniques and considerable recovery time, posing challenges. This study investigates a minimally invasive method tailored to the unique facial structures common in Asian populations, employing biostimulators and lipolytic agents for precise facial contouring. This multimodal approach aims to address the specific needs and aesthetic goals of patients of Asian heritage.

Materials / method: FDA approved biostimulators and lipolytic solutions were used on 10 patients with facial fullness at a private clinic. A single treatment session employed biostimulators for collagen and elasticity enhancement, plus lipolysis for fat reduction. Follow-up at two weeks and one month included photographic analysis. Patient satisfaction, based on perceived enhancement and discomfort, was surveyed with pre and post questionnaires.

Results: All patients demonstrated noticeable improvements in facial contours and skin texture. The combination therapy effectively reduced facial fullness, particularly in the lower cheeks and jawline. Patients reported high satisfaction levels with the results, citing improvements in facial shape and firmness. Minor side effects included temporary redness and mild swelling, resolving within a few days post-treatment.

Conclusion: In addressing the distinctive characteristic of facial fullness common among individuals of Asian heritage, the synergistic application of biostimulators and lipolytic agents emerges as a viable non-surgical alternative. This innovative approach promises not just to redefine facial contours but also to enhance the texture and quality of the skin, thereby providing a twofold advantage. The favorable feedback from patients, coupled with a low incidence of complications, highlights the effectiveness of this contouring method for facial enhancement.

Comparing Calcium Hydroxylapatite to Hyaluronic Acid for Treatment of Nasolabial Folds

Objectives: This multicenter, blinded, randomized study compares patient satisfaction variables, efficacy, and durability of CaHA gel and HA for correction of nasolabial folds (NLFs) through 12 months after 3-month touch-up

Introduction: In this study, we examined Radiesse (calcium hydroxylapatite; CaHA), Juvederm for patient satisfaction, likelihood to return, immediate efficacy, and duration of correction.

Materials / method: A total of 105 randomized patients received either CaHA gel or HA for NLF treatment at initial visit and 3-month touch-up. Patients returned at 3, 6,9 and 12 months after the second injection for evaluation. Surveys measured patient satisfaction; effectiveness measures included the Global Aesthetic Improvement Scale (GAIS).

Results: More CaHA gel patients were satisfied or extremely satisfied than each HA tested. At 6 months, significantly more CaHA gel–treated NLFs were improved on the GAIS than any HA. The volumes of CaHA gel and three HA materials injected through 43months were 2.3, 3.0, 4.9, and 2.9 mL, respectively (p<005). No serious adverse events were observed.

Conclusion: In this controlled study, CaHA gel ranked highest in patient satisfaction and likelihood
to return. The material was more effective and longer lasting than each HA in maintaining NLF augmentation.

Rainbow (ROYGBbP) Symbol: A Methodological Injection Algorithm of Hyaluronic Acid Fillers Adapted to Characteristics of Asian Faces

Objectives: To discuss the efficacy and potential of Rainbow (ROYGBbP) Symbol, a methodological injection algorithm of hyaluronic acid (HA) filler, on Asian faces.

Introduction: The consumption of injectable hyaluronic acid (HA) fillers in Asian countries like China has been increasing tremendously in recent years. The optimization of HA filler application in Asian faces should be persistently explored. Rainbow (ROYGBbP) Symbol is a methodological injection algorithm of HA filler, which has been used in more than hundreds of Asian faces.

Materials / method: Rainbow Symbol is a set of anatomical subunits to inject HA fillers. It divided the facial injected areas into seven groups, marked by rainbow colors:
Red (temporal fossa)
Orange (the strip area from nasal base to zygoma to hairline above ear along zygomatic arch)
Yellow (area under zygomatic arch and buccal space)
Green (posterior line of mandibular angle)
Blue (chin, the area between the anterior buccal groove and the buccal-mental groove)
black (junction area of frontal bone and nasal bone)
Purple (forehead)
Depth, volume tool and technique of injection are also included in Rainbow Symbol.

Results: A multicenter study in China including 216 aesthetic patients showed 1 month, 6 months and 12 months after injection of HA filler according to Rainbow Symbol, 94.0%, 96.3% and 94.9% of the aesthetic patients rated their whole face appearance as "very much improved" or "much improved" on the GAIS, respectively.
Significant Alleviation of tiredness can be observed in a 22-year female after injection in R and O area, he two most fundamental areas. Further improvements can be easily identified after Rainbow Symbol was completely fulfilled.

Conclusion: Rainbow (ROYGBbP) Symbol, based on the anatomical feature and aesthetic culture of Asians, is a practical and efficient method to rejuvenate Asian aesthetics patients. It can be applied in a complete way or a simplified way according to the personalized requirement of the aesthetics patients.
