Objectives: Melasma is a frequent cause of hyperpigmentation and still remains difficult to treat. The diagnosis of melasma is clinical but several pigmentary disorders can lead to a similar pattern and may be misleading. However, in most cases a precise diagnosis can be done thanks to the study of the clinical history and a careful examination of the pigmentation and the entire skin surface. Such as systematic approach is necessary to make the right diagnosis and provide the most adapted treatment.

Introduction: Check list in front of a hyperpigmentation of the face:
- precise analysis of history (familial history, age and factors of onset)
- treatments
- others clinical signs (asthenia…)
- phototype
- complete physical examination of the entire skin and mucosal areas
- improvement or not with solar eviction (during winter)
- improvement or not with triple topical therapy (if already done)
- blood check-up (inflammatory, immnunology, endocrinology…) might be useful in some case depending on the clinical examination
- histology might be also useful

Objectives: We studied the safety and efficacy of a combination of pulsed dye laser (PDL) and fractional ablative CO2 laser (FACL) for the attenuation of post-lumpectomy scarring.

Introduction: Light-based modalities appear to be effective for ameliorating surgical scar appearance, however, protocols for achieving such outcomes have yet to be established.

Materials / method: We conducted a prospective, evaluator-blinded, comparative split-scar study in post-lumpectomy patients. One-half of the scar was treated with three sessions of 595 nm PDL and FACL at one-month intervals, starting within six weeks after suture removal. The entire scar was also treated with standard moisturizers and silicone gels. Six months after the last treatment, the two halves of the scar were assessed by three uninvolved physicians who used the Observer Scar Assessment Scale as well as by the patients who used the Patient and Observer Scar Assessment Scale.

Results: Eighteen female patients (mean age, 51.3 years) with a mean scar length of 7.8 cm completed the treatment and follow-up. Six months after the last treatment, both the physician evaluators and the patients noted significant improvements for all assessed scar parameters in the laser-treated scar area compared to the untreated scar area. The treatment was well tolerated, and no remarkable adverse events were reported. All 18 participants were satisfied with the treated scar areas.

Conclusion: A combination PDL and FACL protocol starting up to 6 weeks after suture removal is a safe and effective method for the attenuation of post-lumpectomy scar formation.

Objectives: The learning outcomes of this presentation is to understand the importance of laser safety to avoid laser accidents.

Introduction: Laser safety is of overall importance to minimize the risk of laser accidents.

Materials / method: According to wave length and power lasers may be classified from class 1 representing lasers with no hazard during normal use to class 4 lasers with severe hazard for eye and skin. The hazard of medical lasers may be divided into beam hazard and non-beam hazard. The laser beam may induce a wide range of ocular damage as well as undesirable skin reactions. Non-beam hazards cover electrical hazards due to high voltage devices that may cause fire and explosion. Furthermore, non-beam hazards are plume contaminants including bacterial, viral and carcinogen plume.

Results: A practical guide to protect patients, staff and laser devices will be presented.

Conclusion: No one should initiate medical laser treatments without being thoroughly instructed in laser safety.
