Objectives: Hair restoration has always been a tug of war between the availability of scalp donor follicles (donor area) versus the demand of recipient area. This becomes a real challenge in patients with more than type IV MPB, where they have more than 60% of bald scalp wanting to be covered.
The best donor area is no doubt the back of the scalp. The quality of the hair is similar and the results are great, as we have witnessed over the period of so many years.

Introduction: The combination of both FUSH and FUE can maximize the donor harvest in more than type IV MPB. The FUSS will reduce the vertical height and some density keeping the minimum width of donor area intact. FUE will now take the random follicular units from this donor area without compromising the width, but only reducing the density to an acceptable width and matching the proposed recipient area density.
This way, the patients gets the maximization of the donor harvest and this can also be an economical option due to major chunk being the FUSS.

Materials / method: Mostly type V to VII MPB need this maximization due to severe discrepancy between the donor area availability and recipient area demand. Most patients were treated in stages to benefit from strategic placing technique. Initially the donor harvest was with FUSS and once a minimum acceptable donor width was achieved considering the age and family history of patient, FUE was conducted to harvest further grafts.

Results: The main difference was noticed in the patients who under went third or fourth session. They could really appreciate the difference. This technique makes the maximum usage of the available donor area. The harvested hair can easily be spread on the larger recipient area to give a reasonable acceptable density by using the technique of strategic placing and shingling effect of acute angle of exit.

Conclusion: We believe by taking care of these points, one can utilize more scalp donor area without a need for body hair transplant.
It’s a very simple practical point that a young surgeon can incorporate in his or her practice and can improve their results significantly.
It’s the small things in life that counts the most.

Introduction: The HairsteticsTM hair implant device is a novel comprehensive alopecia solution. The product has been approved in Israel, Brazil, New Zealand, Switzerland, EU and Thailand. Objectives: Due to the widespread practice of daily aspirin use as a prophylactic treatment for heart conditions, it is important to determine if prolonged aspirin use effects sulfotransferase activity in hair follicles.

Introduction: Topical minoxidil is the only US FDA topical drug for the treatment of pattern hair loss. Millions of patients, world-wide, use minoxidil daily. Minoxidil is a pro-drug converted into its active form, minoxidil sulfate, by the sulfotransferase enzymes in the outer root sheath of hair follicles. In human liver, the phase II metabolism of xenobiotics by sulfotransferase is significantly inhibited by salicylic acid.

Materials / method: Twenty-four male subjects with AGA were recruited to study. Plucked hair samples were collected at the initial visit and analyzed using the sulfotransferase activity assay previously reported by Goren et. al. Subjects were provided 14 tablets of 75mg aspirin. Subjects were instructed to use one tablet per day and return to the dermatology department 14 days after initiation of therapy. Plucked hair samples were collected during the visit on the 14th day and analyzed using the sulfotransferase activity assay.

Results: Of the 22 subjects that completed the study, 12 (55%) subjects had a significant reduction (p<0.0001) in follicular sulfotransferase enzymatic activity, i.e., >0.2 OD reduction following 14 days of aspirin therapy. More importantly, the sulfotransferase activity assay initially predicted that 11 (50%) of the 22 subjects may respond to topical minoxidil. However, following 14 days of aspirin administration only 6 (27%) subjects were predicted to respond to topical minoxidil.

Conclusion: While minoxidil has an excellent safety record, its efficacy in the treatment of AGA is limited by the sulfotransferase enzymes activity in ORS of hair follicles. The same enzyme is significantly inhibited in the human liver by another OTC drug used by millions of men, aspirin. In this study, we demonstrated that low dose oral aspirin negatively inhibits the activity of sulfotransferase enzymes in the human hair follicle. Therefore, chronic use of low dose aspirin is likely to reduce the efficacy of topical minoxidil treatment.
