Objectives: We look into the importance or restoring the degenerated basement membrane in managing melasma cases in chronically UV exposed population in South East Asia.

Introduction: Even with the newest technologies, managing melasma in tropical countries can be challenging. This is due to years of UV exposure leading to basement membrane disruptions, proliferation of abnormal capilaries and degeneration of fibroblasts.

Materials / method: We look into patients that has treated with micro needle pulsed radio frequency and the success rate in terms of melasma management.

Results: We see great improvements in patients treated with micro needle pulsed radio frequency.

Conclusion: We suggest to have all patients to be treated with micro needle pulsed radio frequency for better melasma control.

Viol - Findings during the RF microneedling treatment in terms of skin remodeling and melasma

Objectives: Observation and medical studies of the relationship concerning RF microneedling to skin remodeling and melasma.

Introduction: Recently, we have succeeded in selectively inducing thermal coagulation in basement membrane and vasculature using P.W. (Pulsed Wave) RF, which periodically conducts multiple pulses. Our R&D team found that coagulation occurs first in the basement membrane and in the blood vessel prior to coagulation in the dermal matrix during RF conduction to the skin.

Materials / method: After RF treatment, skin samples exhibited RF-induced coagulation columns of thermal injury, separately generated around each microneedle in the dermis. In ex vivo bovine liver tissue, the thermal coagulation columns were found to be concentrated maximally around the pointed tips of each electrode. After a RF conduction time of 2 seconds, the individual areas of thermal coagulation began to converge with neighboring RF-induced coagulation columns

Results: PWRF microneedling delayed the recurrence of melasma and redness during a year after the treatment. PWRF microneedling improves fine lines, pores, skin texture and skin elasticity, too.

Conclusion: PWRF microneedling is a new game changer that can treat all the aging related skin problems at once, less pain than CWRF microneedling, less downtime due to gentle heat reaction, safe use for pigment lesions and vascular lesions.

Tulip Medical - OneGlobeBio - Mechanically generated stromal cell aggregates: a powerful graft with potent effects

Objectives: Prove that:
The amount of cells, its viability and function comprise the therapeutic agent of adipose-derived therapies.
Adipose tissue requires a given cell population (constitutive dose) to maintain its structure (volume) and reparative functions.
Mechanical disaggregation of liposuction generated tissue fragments render a near constitutive dose of cells, viable and functional.

Introduction: All components of liposuction-generated fat grafts including cells, matrix, soluble factors or even sub-cellular elements are extensively studied as potential therapeutic agents for Regenerative Surgery. Clinical evidence suggests that cells, and not other elements, seem to correlate with clinical outcome. However, cell dose remains elusive. In this study, we hypothesize that the amount of cells constitutively present in adipose tissue is the minimum cell density required to generate a clinically quantifiable effect.

Materials / method: Using Tulip Medical technology, lipoaspirate fat parcels were standardized (fat press) and then subjected to mechanical sheer force (Tulip Nano®) to burst mature adipocytes hence rendering fragments histologically characterized as "stromal aggregates". The total amount of cells (as per DNA elution) was quantified from excised adipose tissue, mechanically-generated stromal aggregates, and enzymatically-digested isolated cells.

Results: Stromal samples contained 7.3 million cells/g, close to 80% of unprocessed dry lipoaspirate , and 70% of native excised adipose tissue (the constitutive dose). Cell viability was not altered by mechanical disaggregation. Nanofat samples contained 6.63 million aggregated cells/g , whereas enzymatic digestion of the same tissue preparations resulted in only 0.68 million isolated cells/g.

Conclusion: Mechanical disaggregation renders a 10 times denser cell inoculum in aggregated form versus conventional enzymatic dissociated isolated cells, reaching a near constitutive dose. Because this high cell yield, mechanical disaggregation methods require 10 times less stock tissue, delivering a viable, higher cell yield.

Introduction: Skin aging is influenced by genetic factors and life style, especially sun exposure and smoking. Topical cosmeceuticals are typically formulated to address specific age related cutaneous challenges. Identifying the most effective and proven ingredient blends allows the physician to provide patients with topical solutions to correct and prevent the foremost causes of visible facial aging. . Facial volumization with HA is widely applied for minimally invasive facial rejuvenation

Materials / method: A total of 15 subjects with skin phototype I – III participated in this study. Mid and lower face were treated with saypha® HA products depending on individual subject’s needs in one session. All procedures were performed with 25G 50mm length cannula. The formulations for topical skin care were prepared on the day of the first application and the subjects were supposed to use them twice a day for six weeks until the control visit.

Results: Fourteen subjects completed the study, and one subject was lost to follow-up. All subjects in the study were female. The age of the study population ranged from 36 to 58 years, with an median age of 48.6 years. The average amounts of saypha Volume Plus, saypha Volume and saypha Filler used per subject were 2.8 ml, 2,25 ml and 1.7 ml, respectively. The most typical skin dysfunctions were loss of elasticity (12 subjects) hyperpigmentations (9 subjects), erythema (8 subjects) and hyperseborrhea with enlarged pores (5 subjects). 97.4% of the patients were very satisfied and 2.6% were satisfied.

Conclusion: Combined therapies based on HA fillers and personalized skin care with Universkin products enhance the results obtained with HA fillers alone.

ExoCoBio Inc - Adipose Stem Cell Exosome (ASCE) - based regenerative therapeutics & aesthetics

Objectives: 1. To know that adipose stem cell exosome (ASCE) can be used as next generation regenerative therapeutics & aesthetics
2. To know that ASCE can reconstruct skin barrier functions and be a new therapeutic candidate for atopic dermatitis (AD)
3. To know that ASCE are toxicologically safe for skin

Introduction: Exosomes, nano-sized extracellular vesicles, are the most important mediator for intercellular communication. These days, ASCEs are being developed as cosmetics/cosmeceuticals and therapeutics as well. It was found that ASCEs can be very effective for skin rejuvenation/regeneration and atopic dermatitis through reconstitution of skin barrier functions. It was also found that ASCEs showed safe toxicological profiles.

Materials / method: Human adipose tissue-derived mesenchymal stem cell-derived exosomes (ASCE) were used for in vitro, ex-vivo, and in vivo studies.

Results: ASC-Exosomes,
1. contain active cargo: >1,000 proteins & ~600 miRNAs. Many of them are functional for skin;
2. Are effective for skin rejuvenation: increased collagen & elastin synthesis;
3. Are effective for atopic dermatitis in an inflammatory model: reduced clinical score, IgE level, cytokine mRNAs, & inflammatory cells;
4. Are effective for atopic dermatitis in a skin barrier model: reduced TEWL, epithermal thickness, & cytokines; increased the lipids; normalized gene expression for skin barrier, lipid metabolism, cell cycle, & immune; &
5. Are safe for skin: no toxicity was observed.

Conclusion: 1. ASCE can be next generation regenerative therapeutics and aesthetics, superior to conventional approaches such as skin boosters, growth factors, stem cells, stem cell conditioned media, PRP and others.
2. ASCE can be a novel promising cell-free therapeutic modality for AD treatment.
3. ASCE can be applied on skin with no significant toxicity.

Objectives: For many years, platelet rich plasma therapy is being used clinically to enhance the treatment success of soft tissue stimulation and bone injuries/defects regularly. By means of different clinical approaches, there may have various preparation methods are set to fulfill the requirements of the treatment idea. Within the frame of Platelet numbers, dose, recovery and even purity; whether platelets are to be separated from other cells are all in question. We demonstrated the characteristics of different preparation techniques.

Introduction: There are various techniques are being used to obtain platelet concentration to form autologous healing. Generally, a gel separation technique is being used by abducting higher percentage of erythrocytes by aiming the leukocytes level to be decreased compared to initial level of the whole blood. We demonstrate here with this study, Platelet Rich Plasma in different characterization, Platelet Rich Fibrin preparation methods and ideas taking into consideration of leukocytes, erythrocytes and platelet recovery along with purity levels in the final injectable milieu of Platelet Rich Plasma.

Materials / method: We used T-LAB PRP Kits, T-LAB PRP Tubes, T-LAB PRX Tubes in order to obtain platelet rich plasma and platelet rich fibrin using different protocols. We either used swing-out centrifugation and fixed-angle centrifugation devices in order to demonstrate the damage on the membrane of platelets to start clotting along with the free calcium ions role in activation in-vitro. We used different protocols of centrifugation and demonstrated the difference between the G force and how physical centrifugational movement effect the final product characteristics.

Results: We demonstrated with PRX tubes, without anticoagulant; it is possible to obtain liquidic form of platelet concentration which no substance was used in order to bind the free calcium ions to prevent clotting. In time, in-vitro activation occurs in the syringe or petri dish. With PRP tubes, swing out centrifugation is applied in different G force and the results are demonstrated with different in-vitro outcomes within the frame of final product characteristics. We analyzed pure platelet rich plasma with up to 98% recovery rate.

Conclusion: Further studies are needed to conclude the full-scale of understanding the possible clinical outcomes from different final product characteristics for injections and applications.

Regenera Activa SL - Micrografts technology and hair restoration

Objectives: To demonstrate the clinical efficacy of Rigenera® micrografting technology in the promoting hair restoration in patients affected by androgenetic alopecia also addressing for a correct diagnosis and indication of use of this technology.

Introduction: Androgenetic alopecia is a chronic process affecting 80% of men and 50% of women throughout life and hair transplantation is to date the gold standard method for its treatment. The micrografting technique is based on the concept that by enlarging the skin graft with a ten fold skin expansion, it is possible to cover wide lesions starting from a small sample of donor site. The Rigenera® micrografting technology is a promising approach allowing to obtain a suspension of autologous micrografts derived by the mechanical disaggregation of a small pieces of dermal tissue ready to use.

Materials / method: Clinical evaluations were performed after 4 and 6 months with a follow up until to 18 months from treatment. Scalp dermoscopy and histological analysis were performed to assess the hair growth and histological appearance of the scalp.

Results: Clinical evaluation was performed taking photos before and after treatment showing an improvement of hair growth and density due to an increase of total hair count and density accomplished by an increase and reduction of anagen and telogen phases, respectively. These results are confirmed by scalp dermoscopic and histological analysis reporting an amelioration of the scalp, development of new hair follicles and a beginning of cuticle formation.

Conclusion: Collectively, the obtained results suggest that micrografts can be considered a novel therapeutic option in the management of androgenetic alopecia.

Introduction: Androgenetic alopecia (AGA) is an androgen-related condition connected with genetic factors that affects up to 80% men and 42% woman in Caucasian population. Diagnosis of AGA is in most cases based on clinical and trichoscopic features and there are not many efficient treatment options approved by FDA so far.

Materials / method: New therapeutic approach based on FDA approved device has been introduced to the Polish market 3 years ago. It is a new method of skin progenitor cells isolation by mechanical centrifugation of punch biopsy from human scalp without culture. Obtained cell suspension (incl. SVF-stromal vascular fraction, HFSc-hair follicle stem cells) is injected in affected areas with deep mesotherapy technique as an autologous micrografts.

Results: The procedure is simple, fast and efficient with no downtime. First results in most cases occur after 30-60 days. This metod is considered to be effective in reversing hair miniaturization in AGA and as a result clinically improving hair density. Clinical response rate to this treatment is about 80-90% with proper qualification of the patient (clinical examination and trichoscopy). It can be considered as a monotherapy or additional treatment of androgenetic alopecia.
