Objectives: For future use of botulinum toxin it is important to raise more awareness on the role of facial expressiveness in nonverbal human communication and the effect botulinum toxin has on this. This topic is made visual in a video work which will evoke discussion and thought.

Introduction: The use of botulinum toxin is becoming exceedingly popular, particularly amongst younger people. Botulinum toxin works against wrinkles by relaxing facial muscles. Few people realize that it may also profoundly affect facial expressions. Facial expressions are an important part of one’s individuality. They are not only important to feeling emotions and expressing them, but also to interpersonal communication.

Research question:
Will we still understand one another if we can no longer fully utilize our facial expressions in non-verbal communication?

Materials / method: This question was explored by running a standardized and protocolized routine of 18 facial expressions before-, and two weeks after full facial injections with botulinum toxin. Facial expressions were read-out from high-resolution video using (1) facial emotion-reading software, i.e. artificial intelligence, and (2) the human eye.

Results: While the subject made the same effort to express emotions before and after botulinum toxin, outcomes were clearly different. Depending on the readout (i.e., emotion-reading software or the human eye) clear inabilities to display disgust or a genuine smile (‘Duchenne smile’) were detected after botulinum toxin treatment. Some discrepancies were noted between the two different readouts demonstrating the subjective and complex nature of emotions – and perhaps the complex nature of the effects of botulinum toxin thereon.

Conclusion: Botulinum toxin does not only prevent skin wrinkle formation but also affects the ability to display emotions. For the future use of this toxin, it is important to be aware of this – particularly in young people to whom facial expressiveness in nonverbal communication is still essential to their development. While botulinum toxin is highly effective in treating and preventing wrinkles, dosages and injection schemes must be carefully considered between doctors and clients, because the ability to express emotions is important for human development and interaction.

Objectives: To present a tool which supports health care providers and receivers in assessing objectively and reproducibly aesthetic status and outcome.

Introduction: According to the WHO, health is defined as a state of complete and not merely the absence of disease or infirmity. Aesthetic medicine has been shown to address all of the three aspects of complete health – physical, mental and social well-being – with both surgical and non-surgical procedures. However, the assessment of individual patient’s need is based on the individual experience of the health care provider (HCP). Furthermore, it will sometimes be influenced by available or preferred treatment options.

Materials / method: An assessment tool was developed by analyzing >200 original and derived facial variables in >15 facial regions via AI technique. The characteristic features found were validated using digital images and live assessments as well. The variables were summarized into common landmarks of clinical facial signs leading finally to a facial aesthetic index (FAI) and a facial youth index.

Results: This detailed facial analysis of >15 facial regions, e.g. using frontal, periorbital, and perioral wrinkles, nasolabial and melomental folds, cheek volume, skin color, regional asymmetry, etc., lead to a trustworthy FAI and facial youth index, that might be used to identify the potential interventions for individual personalized and prioritized treatment recommendations.

Conclusion: Current methods for assessing facial appearance, attractiveness and youthfulness are mainly done by subjective evaluation. CAARISMA was developed to avoid minimum discernable daily changes in subjective aesthetic assessment caused by various factors. This novel AI-based algorithms will help to standardize the patient assessment as well as to find the best individualized treatment option.
