Objectives: The medical aesthetics industry has focused on dermal rejuvenation, but epidermal health, which is directly influenced by the dermal-epidermal junction (DEJ), is equally crucial. As we age, the DEJ flattens, weakening the connection between the epidermis and dermis, affecting skin resilience, hydration, and renewal. This presentation will review a retrospective case study and real-world clinical cases that followed a DEJ-focused protocol across diverse age groups and ethnicities, highlighting its potential for optimizing skin rejuvenation treatments.
Introduction: Skin aging is often addressed through dermal remodeling, overlooking the epidermis and the dermoepidermal junction (DEJ), a critical interface responsible for mechanical support, signaling, and nutrient exchange. As skin ages, the DEJ flattens, weakening epidermal adhesion and barrier function. This study investigates the effects of a topical exosome formulation on DEJ structure and skin quality by promoting regenerative cellular signaling and improving structural integrity.
Materials / method: Subjects received microneedling at 0.3 mm depth followed by topical application of an exosome product (E 50 Skin Booster). Skin biopsies were collected before and eight weeks after treatment from the postauricular area. Samples were stained and analyzed using brightfield microscopy and artificial intelligence based image analysis to quantify rete ridges, ridge amplitude, epidermal thickness, basal keratinocyte density, and collagen fiber density.
Results: For the 63 year old subject observed in the case study, Treatment improved multiple skin parameters. Rete ridge count increased by 17.08 percent and ridge amplitude by 7.75 percent, indicating DEJ restoration. Epidermal thickness rose 52.33 percent and basal keratinocyte density increased 60.90 percent, reflecting stronger regeneration. Collagen density improved by 5.77 percent. No adverse reactions were noted, and histological review showed no inflammation or fibrosis, supporting excellent safety and tolerability.
Conclusion: This exosome based treatment improved epidermal thickness, DEJ structure, and dermal collagen density. By targeting the DEJ, it strengthens epidermal adhesion and promotes renewal, offering a new path for regenerative skin therapies. The formulation was well tolerated with no observed side effects. Future studies should evaluate long term effects and molecular changes in key DEJ proteins to support further protocol development.
