Objectives: Understanding the technique of using autologous costal cartilage for dorsal onlay graft and its clinical outcomes.

Introduction: In conventional rhinoplasty, silicone is used for dorsal augmentation along with tip plasty. However, complications such as inflammation or deformation can occur due to silicone implants. In such cases, autologous grafts are employed for dorsal augmentation, with rib cartilage being a good option.

Materials / method: To perform a dorsal onlay graft using autologous costal cartilage, an adequate amount of rib cartilage is required. Typically, the seventh rib cartilage is preferred due to its length and thickness. The entire seventh costal cartilage is harvested, and the straightest portion is designed into a boat shape similar to a silicone implant. The cartilage is then cut and carved to the desired shape with the appropriate thickness.

Results: Good surgical outcomes were achieved using only autologous tissue in cases of contracted or deformed noses.

Conclusion: Dorsal augmentation using autologous costal cartilage can be a good alternative for contracted or deformed noses.
