How to increase safety when performing fillers

Objectives: As the number of treatments performed with dermal fillers exponentially increases, complications that are rare, obviously also occur in greater numbers.
Therefore, it is very important to reinforce the measures that can help to avoid these situations that are at least unpleasant when not disastrous.
As in any medical act, success depends on: 1- the good evaluation and selection of the patient; 2- choice of the ideal product and the good practice of performing the procedure; 3- the ability of the performing physician.

The problem of filler complications: how bad is it?

Objectives: To understand the pathophysiology of filler complications.

Introduction: Different types of filler complications are seen. Over the past few years research at our university clinic has elucidated much of the background of these. Also the question whether a complication can be regarded as doctor-, patient- of filler-related is by and large been solved.

Materials / method: In the past 5 years almost 1200 new patients were referred to our out-patient clinic for filler complications. By systemically checking patient details and stratifying their problems we were able to understand much of the nature of their problems. In a number of patients biopsies from the filler material were analysed for both bacteria and immunological cells. Also every patient was analysed using ultrasound imaging.

Results: We found a correlation between complications and poor trained doctors. We established a new hypothesis about nature and treatment of vascular adverse events and non-inflammatory nodules with ultrasound imaging. We were able to assess the indicence of filler complications.

Conclusion: Resorbable filler complications are seldomly caused by the filler material, but mainly by less qualified doctors. Ultrasound imaging helps enormously in diagnosing and treating filler complications
