Objectives: To analyze the difference between polynucleotide alone therapy and polynucleotide + botulinum toxin combination therapy, and the recent trend of the entire polynucleotide therapy (including both group) for skin rejuvenation in efficacy, safety, and patient’s satisfaction.

Introduction: Polynucleotide therapy for skin rejuvenation which our group adopted in July 2018 for improving the skin quality of especially the lower eyelid has been more popular overtime. We performed 55,410 cases in total from July 2018 to the end of January 2023.

Materials / method: We investigated the number of treatments of polynucleotide alone group and polynucleotide + botulinum toxin group, and number of adverse events and patient’s satisfaction from our database and medical record from 2018 to the end of January 2023. The aim of this study is to analyze the difference of both group and the recent trend of the entire polynucleotide therapy (including both group).

Results: Since last year, when a mixture product of polynucleotide and botulinum toxin, the number of cases of polynucleotide + botulinum toxin combination therapy (n = 12,890; Feb 2022 to Jan 2023) has been higher than the number of cases using polynucleotide alone therapy (n = 6,738), which is one of recent new trends on polynucleotide therapy. The entire number of polynucleotide therapy was 19,628, which has been increasing year by year since 2018 (n = 1,975 in 2018, n = 5,078 in 2019, n = 4,536 in 2,020, n = 6,463 in 2021, and n = 17,730 in 2022).

Conclusion: We can see from the above history of the number of the entire polynucleotide therapy that the total number of polynucleotide therapy cases increased dramatically in 2022 when polynucleotide + botulinum toxin combination therapy was adopted. We report the more detailed results of this study and show some findings from the results.

Objectives: The objective of this study effectiveness and safety of PDLLA on skin rejuvenation.

Introduction: Recently, poly-L,L-lactic acid (PLLA) as collagen stimulator is used for skin rejuvenation. However, irregular-shaped particles always pose a risk of nodules. Unlike PLLA, the polymer structure of poly-D,L-lactic acid (PDLLA) is round, which makes PDLLA less rigid and has good tissue affinity. In recent cell and animal studies with several collagen stimulators, PDLLA demonstrated excellent effects in collagen and elastic fiber regeneration by increasing adipose tissue-derived mesenchymal stem cell (AD-MSC) survival rate and decreasing the level of matrix metalloproteinase 2,3, and 9.

Materials / method: We investigated the efficacy of the PDLLA/HA dermal filler injection for skin rejuvenation on 17 patients who visited a dermatology department at Korea University Ansan Hospital. All patients received two or three sessions at four-week intervals.

Results: There are improvements in fine wrinkles, skin texture, skin tone, telangiectasia, and facial erythema. Forty-seven percent respond with 50% or more significant overall improvement. Additionally, the histological examination shows increased collagen and elastic fiber in the superficial dermis after the procedure. There are no treatment-emergent adverse events (TEAEs) without transient facial erythema and pain during the procedure.

Conclusion: Therefore, our preliminary findings suggest that the PDLLA/HA dermal filler injection is a safe and effective method for skin rejuvenation.

Objectives: Botulinum Toxin type A and exosomes have been cited for their capabilities of synthesizing collagen and elastic fiber. However, the delivery of these compounds has been focused on the dermis level. This clinical study explores a unique protocol for delivering a cocktail of exosomes and botulinum toxin non-invasively to the dermal-epidermal junction.

Introduction: The dermal-epidermal junction (DEJ) intersects the epidermis and the dermis. The functions of the DEJ are epidermal-dermal adherence, mechanical support for the epidermis, and a gate for the passage of cells and molecules across the junction. With aging, the altered composition of proteins in the skin, such as collagen, integrin, and laminin, affects the physiology of the DEJ. This flattening of the DEJ reduces its surface area leading to the increased fragility of aged skin.

Materials / method: This clinical study explores a unique protocol for delivering a cocktail of exosomes and botulinum toxin non-invasively to the dermal-epidermal junction.

Results: The results showed the increased synthesis of collagen and elastic fibers at the junction, improving skin elasticity and firmness. This also resulted in enhanced epidermal-dermal adherence, which provided better mechanical support for the skin and reduced the risk of skin tears or injuries. Furthermore, the increased passage of cells and molecules across the junction potentially improved skin hydration and nutrient delivery. The overall skin quality, including skin tone, texture, and radiance, improved significantly, resulting in increased patient satisfaction with the treatment.

Conclusion: In conclusion, delivering a combination of exosomes and botulinum toxin to the dermal-epidermal junction is a promising non-invasive technique for improving the physiological function of the DEJ and enhancing overall skin quality. This was evidenced by self-reported improvements in skin appearance and reduced signs of aging. Further studies may help optimize the protocol for different non-invasive procedures and improve our understanding of the interactions with energy-based devices. Ultimately, this technique can potentially become a new paradigm in anti-aging treatment.
