Revitalizing damaged skin: a novel approach to addressing skin necrosis following dermal filler injection with IPL

Objectives: Skin necrosis after dermal filler injection can be distressing for physicians and some may even withdraw from the market after a single incident. Our study shows that Intense Pulsed Light (IPL) therapy is an effective alternative treatment option for skin necrosis. By providing more options for physicians, we hope to alleviate the burden and anxiety associated with this complication and prevent them from leaving the market. We also hope to increase awareness of the potential benefits of IPL therapy for a variety of skin conditions when more research is focused on it.

Introduction: Skin necrosis following dermal filler injection is a severe complication that can lead to vascular compromise. This study aims to assess the effectiveness of Intense Pulsed Light (IPL) for Chinese patients who have developed iatrogenic skin necrosis following dermal filler injection, subsequent to initial treatment with hyaluronidase. The study evaluates the efficacy of IPL therapy as an alternative management option for this condition in Chinese patients.

Materials / method: This study assessed the efficacy of IPL in 4 cases of skin necrosis resulting from dermal filler injection referred by other physicians. The severity of cases ranged from mild to severe, with 3 patients receiving 4-12 sessions of IPL within 4-10 days after the skin necrosis episode. The fourth patient, who developed a keloid scar, received only one IPL treatment and did not follow up. All patients had previously received hyaluronidase injection, and clinical photos were taken before each treatment session with two independent blinded investigators assessing and comparing the pre/post photos.

Results: The study showed an overall clinical improvement in scar hyperpigmentation, thickness, hardness, and redness in the three patients who received multiple sessions of IPL treatment. However, the fourth case, who received only one IPL treatment after initial medical treatment, did not show as significant an improvement as the other three cases.

Conclusion: The findings of this study suggest that Intense Pulsed Light (IPL) therapy is an effective treatment option for improving the erythema, hardness, thickness, and overall appearance of skin necrosis following dermal filler injection.

The safe delivery of RF microneedling for acne scars in Vietnamese patients receiving low-dose of isotretinoin

Objectives: RF needling seems to safe regardless of low dose of isotretinoin even more effect than without isotretinoin. However, mini invasive procedure within oral isotretinoin may induce longer erythema.

Introduction: The efficacy of scar treatment may be limitted if the scars last longer than 6 months. Many severe acnes required the long term oral isotretinoin, thus the optimal time almost passed. However, in previous studies supported for avoiding cosmetic procedure such as laser, chemical peel, microneedling within oral isotretinoin treatment.
We intent to investigate the effect and side effects of RF needling for acnes scars during oral isotretinoin treatment at the low dose.

Materials / method: The record of 30 patients who was remaining low-dose of isotretinoin (0.2mg/kg/day) and 30 patients who stoped oral isotretinoin more than 6 months, accepted to scars treatment were including in this study. All patients received 3 times of RF needling treatment with the same intensity, 300 shots per time and moisturizing topical treatment for full face during a month interval. Follow-up was for 6 months to evaluate efficacy and side effects by both dermatologists and patients.

Results: The scar improvement was evaluated by 2 blinded dermatologists using the photographs was higher in isotretinoin group compared to control group (p<0.05). Pore large and wrikle was reduced in both group compare to baseline (p<0.001). Patients in isotretinoin group was observed the longer duration of redness 15+/- 1 days after procedure compared to non-isotretinoin group (0<0.05). The higher prevalence of patients in isotretinoin group complained about redness compared to control group (P<0.05).

Conclusion: RF needling seems to safe regardless of low dose of isotretinoin even more effect than without isotretinoin. However, mini invasive procedure within oral isotretinoin may induce longer erythema.

Microneedling RF with platelet rich plasma therapy for skin quality

Objectives: Microneedling Radiofrequency with Platelet-Rich Plasma (PRP)

The micro needles in MNRF creates micro injuries to the dermis to initiate a healing process,Injured cells release growth and wound healing signals to undifferentiated cells which cause production of new collagen for rejuvenation. This new collagen fills in the wounded areas, acne scars, and thickens the overall skin. This process of new collagen can last from weeks, to up to 6-8 months after the last session. The heat generated by RF (Radio Frequency) produces a healing response that allows the skin to tighten & thicken naturally

Introduction: Microneedling RF for acne scars is a FDA approved minimally invasive, minimal downtime treatment that helps to build collagen, tone and tighten the skin and minimize signs of ageing. Microneedling with RF combines different modalities proven to help the appearance of the skin by stimulating collagen and elastin.

Microneedling also known as collagen induction therapy uses very small needles to penetrate the skin layers. The RF stands for radio frequency, which automatically delivers heat to the deeper skin layer through the microscopic needles stimulating collagen and elastin for tighter skin.

Materials / method: After cleansing your skin, your doctor will apply a topical anesthetic. This will need to set for at least 30 minutes before microneedling begins.
During this time your doctor will apply a numbing cream for 30mins and a FDA-approved MNRF device will be used for your face and move it across the face in stamping motions..
A syringe of blood will be drawn, usually from your arm, while your face is numbing. The blood is then put into a centrifuge, which separates the PRP from the blood. The PRP solution is then injected into the treatment area, either during or after microneedling.

Results: The microneedling treatment opens channels into your skin, allowing for good penetration of the PRP.Microneedling RF with Platelet Rich Plasma (PRP) will help you to improve your skin’s overall appearance, tone, laxity, fine lines, acne scars and stretch marks. micro needles and radiofrequency energy, induce new collagen formation creating tighter, firmer and brighter skin. Platelet Rich Plasma or PRP which contain nutrients from stem cells is applied to the skin after immediately after the microneedling.It takes some time for your body to produce collagen about 1 – 4 weeks for full result.

Solution for the toughest acne scar: nasal ice pick scars

Objectives: 1. Nasal ice pick scars are small and deep scars on the nose that are notoriously difficult to treat.
2.Traditional treatment options, such as TCA Chemical Reconstruction of Skin Scars (CROSS) technique and punch excision, have limitations and may not be suitable for all cases.
3. The novel treatment approach using fractional ablative CO2 laser is an accurate yet effective method to treat nasal ice pick scars, as this treatment method allows for the focal treatment of individual nasal ice pick scars while sparing the rest of the face.

Introduction: Nasal ice pick scars are a specific type of scarring on the nose that commonly arises from acne vulgaris, as well as self-picking and physical extraction of comedones or hair follicles. These small, deep scars are challenging to treat, with TCA CROSS and punch excision being the current best options. However, TCA CROSS is limited by the likelihood of affecting healthy skin due to nasal curvature whereas punch excision is only effective to small individual scars and may cause anatomical distortion to the relatively small nasal area.

Materials / method: Five patients participated in this case series. A fractional ablative CO2 Laser was used to focally treat individual nasal ice pick scars while sparing the rest of the face. A total of three treatments were done every four weeks, with follow-up at week 16. Images were taken prior to treatment and during each follow-up.

Results: Follow-up at week 16 revealed that four patients had clinical improvement of 76-100%, 1 had clinical improvement of 51-75%, and none had minimal or no improvements. No severe adverse effects were reported.

Conclusion: We propose a novel method to treat nasal icepick scars by utilizing fractional ablative carbon dioxide (CO2) Laser to focally treat individual nasal icepick scars. This novel method allowed us to achieve near complete clearance of nasal icepick scars while minimizing downtime and risk of adverse effects.

An innovative approach of treating acne scars using bipolar rotational stamping and monopolar criss-cross technique with insulated MNRF

Objectives: The objective of this study was to assess the efficacy and safety of a novel multiple depth bipolar rotational stamping and monopolar criss-cross method (Wosyet vital technique) with MNRF using insulated needles for the improvement of facial acne scars in Asians.

Introduction: Microneedling radiofrequency (MNRF) using insulated microneedles offers a great advantage to overcome the limitations of fractional lasers such as achieving greater depth, long downtime, and high risk of postinflammatory hyperpigmentation (PIH).

Materials / method: Thirty-two patients (20 females, 12 males, average age 30.3 years) with facial atrophic acne scars were treated with insulated MNRF by applying Wosyet vital technique. Most of the patients started noticing improvement in 4–6 weeks after the first session. All patients underwent four sessions at 1-month interval. Outcome assessments included subjective and physician evaluation of acne scars, pores, smoothness, tightness, and overall appearance. Objective assessment was determined by Goodman and Baron’s quantitative and qualitative analysis of the acne scars.

Results: All subjects noticed at least 30%–90% (mean––62.50%) improvement in acne scars, whereas unbiased physicians graded 40%–80% (mean––58.44%) at a 6-month follow-up visit. The mean Goodman and Baron’s score decreased significantly from pre- to posttreatment. All patients reported 30%–90% (mean––61.88%) improvement in facial contour and skin tightening. Many patients observe improvement in the open pores as well.

Conclusion: The possible explanation of improvement in the global appearance of skin and acne scars is the application of both monopolar and bipolar RF in the dermis through insulated microneedles. We did not find PIH after this technique in Asian patients despite of more aggressive treatment parameters and several treatment sessions.

Portable dermoscope as a clinical tool in laser dermatology for Asian skin

Objectives: 1. Learning the basic of dermoscopic views of common non-melanocytic pigmented conditions.
2. Learning the tips and hints to avoid laser or light induced complications from treatment.
3. Learning the benefit to have dermoscopic laser treatment end-points to lessen the risk of post-inflammatory hyperpigmentation.
4. Learning the technique to assess benign melanocytic lesions at a glance by dermosocope before laser removal in Asian.

Introduction: Laser dermatology is an important topic in asethetic medicine. The clinical diagnosis and treatment response are mainly depend on clinical observation by physicians. The portable dermoscope can be an useful acommpanying tool to assist in many practical aspects while using your laser or light devices.

Materials / method: Retrospective observation of Asian patients who have undergone dermoscopic guided laser and light treatment for various pigmented and skin conditions or lesions in a laser clinic in Hong Kong.

Results: Dermoscopy can differentiate non-melanocytic superficial pigment from deep pigment so as to facilitate the selection of laser. It can also identify inconspicuous dermal vessels and prevent laser induced post inflammatory hyperpigmentation.
Moreover, portable dermoscope helps us to visualize laser treatment endpoints clearly and to monitor treatment progress. Finally, it can assure the benign natures of lesions before performing ablative laser removal and decrease the need of skin biopsy.

Conclusion: The portable dermoscope can help laser physicians to diagnose various types of skin pigment and lesion more precisely and to avoid iatrogenic post inflammatory pigmentation and to decrease the need for lesional biopsy.

The combination treatment of QMR and thread lifting
