660 nm, a safe wave length for treating pigmented lesions in skin of color

Objectives: The Q switched lasers have become one of the most versatile devices in laser practice in the recent past. The objective of this presentation would be to bring forth the latest techniques employed in the use of the Q switched Nd:YAG lasers and to demonstrate the versatility of these newer systems.

Introduction: The Q switched lasers have come a long way. From being the treatment for a very limted indications like tatoos and birth marks the scope of useful indications has increased tremendously. These machines are now used for Laser toning, carbon peels, textural improvement, pico for scar management, dyekit hand pieces producing different wavelengths and so on and so forth.

We have been using the modern Q switched Nd:YAG laser system with the latest technological advancements and attachments in our patients

Materials / method: The 532nm wavelength has always been a difficult wavelength to use in darker skin considering its absorption into both the melanin and the oxyhemoglobin. This leads to a greater generation of heat in the epidermis that can lead to more collateral damage in the dark skins with resultant likelihood of post laser complications like hypo and hyper pigmentation and scaring

The availability of the dye kit hand pieces in the newer QSLs has made it possible to prodproduce 660 nm wavelength which is safer than the 532nm due to its specificity of absorption into the melanin sparing the hemoglobin

Results: We have been using the 660 nm wavelength to treat the epidermal pigmented lesions like freckles and lentigines in our patients with dark skin and have found that it is equally efficacious as the 532 nm wavelength and at the same time is associated with lesser post laser complications

Conclusion: To conclude the newer additions to the QSLs has made these machines extremely versatile and useful in the present day derma laser practice. The availability of the 660 nm wavelength has reduced the possibility of serious post laser complications in skins of colour while maintaining the efficacy of treatment

Fractional CO2 laser on darker skin type

Objectives: Fractional CO2 is the gold standard while treating scars,however the limitation has been th longer downtime and the changes of PIH on darker skin types.This talk explains the various indications for a Fractional CO2 laser besides treatment of scars and also methods to avoid PIH and other complications.

The South-Asian Perspectives on Skin Colour and its Influence on Females

Objectives: “Beauty is proportionate or equivalent to the fairness of the skin” is an ancient adage in South-Asian countries.

Dark skin in females is considered ugly and discriminated from birth to death.

Colour of the skin adversely affects educational performance, social acceptance, career prospects, diminishes chances of marriage, and negatively impacts many other social and personal relationships in the lives of females.

There are a large and considerable number of reported incidents wherein dark complexion has led to being bullied, depression and even lead to committing suicide.

Is chemical peel a better option to treat melasma in skin of color compared to lasers and energy-based devices?

Objectives: Sharing my own experience in treating melasma of patients with skin of color and comparing chemical peels pivotal role in same compared to lasers and Ebds as darker skin types are more prone to worsening of melasmas with little mishaps in the parameters of lasers.
Also because of the tendency of self medications cligman formula and hydroquinone has their own limitations in darker skin.
Where our several patients has benefitted with our combination chemical peels with very less concentration of hydroquinone and other safer skin tightening agents and they have been able to maintain results too

Introduction: Melasma being a disorder of multifaceted etiologies to it's origin nothing is still full proof and always has been challenging in Indian skin and In other patients of skin of color as they are prone to get exacerbation In pigments of the melasma due to ethnicity and also lack of awareness and socioeconomic background.
Treatment options are of wide range but still in darker patients options are always limited and vulnerable to lasers as melasma is very unpredictable with all modalities.
Out of then all combination chemical peels has been easy to go procedure for even new dermatologist and safe.

Materials / method: We have tried chemical peels in most of the patients with melasma even we have seen that the melasma which has not responded to lasers or which has got exacerbated because of lasers and ebds even those we have been able to reduce with combination peels and safer lightning agents.
We will discuss few of very interesting cases of our happy patients which we have achieved with the combination chemical peels after they have come to us after failing in lasers for the same with other dermatologists. And several success stories like that.

Results: It has been very effective and safe option with the predictability in the outcomes.

Conclusion: It can be considered a primary adjuvent therapy option In patients with skin of color after topical and systemic therapy where there is doubt with the other modalities.
