From scar to solution: dupilumab's emerging promise in treating keloids' cosmetic and functional challenges

Introduction: Keloid scars, characterized by excessive collagen deposition from abnormal wound healing, present significant cosmetic and functional challenges. Traditional treatments, such as intralesional steroids and surgical excision, often yield limited results. Dupilumab, a cutting-edge IL-4 and IL-13 pathway inhibitor, has emerged as a novel therapeutic option. Initial case studies reveal promising outcomes, though variability in results highlights the need for further research to fully determine its effectiveness and refine its role in keloid management.

Materials / method: This review investigates the potential of dupilumab in treating keloid scarring through an extensive literature search. We explored PubMed, MEDLINE, and Embase using targeted Medical Subject Headings and keywords related to dupilumab and keloids. The search focused on recent, peer-reviewed articles, clinical trials, and reviews. We meticulously selected studies based on relevance, and rigorously extracted and synthesized data to evaluate dupilumab’s efficacy and safety in addressing keloid scarring.

Results: Keloids, characterized by excessive scarring from abnormal healing, remain challenging to treat due to their high recurrence rates. While traditional methods like steroids and surgical excision often fall short, dupilumab, targeting IL-4 and IL-13, has shown promise in reducing keloid size and symptoms, particularly in patients with coexisting atopic dermatitis. However, responses varied, with some patients improving significantly while others saw minimal or no benefit. These results highlight the importance of further exploration to optimize therapeutic approaches and enhance outcomes.

Conclusion: Dupilumab, targeting IL-4 and IL-13 signaling, emerges as a promising adjunct in managing keloid scarring, especially for cases resistant to traditional therapies. While some patients have shown notable improvements in keloid size and symptoms, variability in outcomes and limited long-term data necessitate cautious optimism. Personalized treatment plans and ongoing research are crucial to optimize dupilumab's role in keloid therapy, address potential biases, and establish its efficacy and safety profile comprehensively.

Efficacy of the combination therapy of intense pulsed light and microneedling with a postbiotic formulation for melasma

Objectives: To assess the clinical effectiveness of the combination therapy of intense pulsed light (IPL) and microneedling with a postbiotic lotion containing Lactobacillus fermentation lysate in facial melasma, with the objective of developing a comprehensive and effective treatment strategy for melasma.

Introduction: Intense pulsed light (IPL) alleviates skin pigmentation by the principle of selective photothermolysis. Lactobacillus fermentation lysate has the potential to repair the skin barrier and inhibit melanin production. Microneedling not only accelerates melanin metabolism and promotes skin self-repair but also enhances the transdermal absorption of therapeutic agents. However, the effectiveness and safety of the combination application of all three for melasma management remains to be fully elucidated.

Materials / method: Thirty participants were randomly divided into two groups. The IPL therapy group received IPL treatment (wavelength: 590-640 nm, pulse width: 3.0-4.0 ms, pulse delay: 40-50 ms, energy density: 14-18 J/cm2) at four-week intervals over a consecutive period of twelve weeks. The combination therapy group participated in a regime where IPL therapy and microneedling (needle length: 0.5 mm) were alternately carried out every two weeks throughout the 12-week treatment duration. The postbiotic lotion was applied immediately after the microneedling procedure.

Results: Compared to the baseline, both the IPL therapy group and the combination therapy group exhibited a significant reduction in the mMASI score, and the combination therapy group was more effective. The evidence of clinical photographic and VISIA-derived statistical data indicated that patients receiving the combined therapy experienced a decrease in facial speckle, a reduction in inflammation, and an improvement in hyperpigmentation, with over 73% of participants expressing very satisfied or satisfied. No significant adverse reactions were reported.

Conclusion: Our study demonstrated that combination therapy yielded significantly greater efficacy than IPL monotherapy. This combined treatment exhibited multiple synergistic benefits, including inflammation reduction, accelerated melanin metabolism, the restoration of skin microecological equilibrium and skin barrier enhancement. These advancements promoted postoperative skin recovery and reduced the incidence of adverse effects, presenting a promising therapeutic approach for individuals diagnosed with melasma.

Efficacy of topical timolol drops in the management of pyogenic granuloma of lip

Introduction: Pyogenic granuloma, also referred to as granuloma pyogenicum, is a prevalent, non-cancerous vascular tumor that develops in various tissues, including the skin and mucous membranes. The medically precise term for this condition is lobular capillary hemangioma. Treatment options include cauterization (either chemical or thermal), laser therapy, surgical excision, curettage, sclerotherapy, and cryotherapy. Additionally, topical timolol is gaining recognition as a non-invasive treatment option for pyogenic granulomas.

Materials / method: Individuals of both genders were enrolled in the study if they had pyogenic granuloma lesions that had been present for less than six weeks. A topical application of 0.5% timolol was administered thrice daily over a period of eight weeks. Evaluations were made regarding alterations in color, size, propensity to bleed, and any side effects experienced.

Results: All 10 patients demonstrated a full response within 8 weeks, and there was no recurrence observed during the 1-month follow-up. Additionally, none of the patients reported any local or systemic side effects.

Conclusion: Topical timolol administered as drops may be an effective, safe, and cost-efficient treatment for mucosal pyogenic granulomas. While topical timolol could be considered for early-stage pyogenic granulomas, further studies with extended durations are necessary to evaluate the rates of resolution and recurrence.

Innovative approach to facial pigmentary disorders: a pilot study on combined pulsed dye laser, RF microneedling, and rose stem cell-derived exosomes

Objectives: This case series aims to evaluate the combined effect of Pulsed Dye Laser (PDL), Radiofrequency Microneedling (RFM), and Rose Stem Cell-Derived Exosomes on various forms of facial pigmentation and assess the safety and tolerability of the combined treatment.

Introduction: Facial pigmentation disorders, including melasma, solar lentigines, acne scars, and post-inflammatory hyperpigmentation, can be challenging to treat especially in patients with darker skin types. Previous studies have demonstrated the efficacy of individual treatments such as pulsed dye laser (PDL), radiofrequency (RF) microneedling, and topical exosomes.

Materials / method: The study included 11 female patients aged 36 to 48 years, the majority of whom have Fitzpatrick skin type IV (73%), while the remainder are skin type III (27%). Patients underwent various combinations of PDL, RFM, and topical exosomes, with sessions ranging from (2) to (10) for PDL, (2) to (8) for RFM, and (2) to (8) for exosomes. Photographs and Visia skin analysis were used for monitoring results.

Results: Results showed that patients who received more sessions of the combined treatments generally experienced better improvement in facial pigmentation. Notable improvements were observed in conditions such as melasma, solar lentigines, and post-inflammatory hyperpigmentation. Visia skin analysis showed an average reduction of spots and brown spots by 31%. All patients tolerated the combination treatment well, and no severe adverse events were reported, indicating a favorable safety profile. This multimodal approach shows synergistic benefits with combined PDL, RFM, and topical exosomes.

Conclusion: There is benefit to combination therapy with PDL, RFM, and exosomes treatment; however, variability in treatment response suggests that individual factors such as skin type and the number of sessions may influence outcomes. The study’s small sample size and lack of a control group limit the generalizability of the findings, and the absence of long-term follow-up precludes conclusions about the durability of treatment effects. Future research should include larger, controlled studies to validate these findings and assess the long-term efficacy and safety of this combination therapy.
