Why Nobel winning studies focus on molecular mechanisms? The time/matrix cellular network model of anti-aging medicine

Objectives: Nobel Prize in Physiology or Medicine 2018 to Allison and Honjo for their discovery of Cancer therapy by inhibition of negative immune regulation. Allison studied the T-cell protein CTLA-4 & developed an antibody that could bind to CTLA-4 and block its function. CTLA-4 blockade could disengage the T-cell brake and unleash the immune system to attack cancer cells. In 2010 in several melanoma patients signs of remaining cancer disappeared by blocking CTLA-4 that disengages the T-cell brake allowing the immune system to attack cancer relentlessly.

Introduction: PD-1, another protein expressed on the surface of T-cells. PD-1, similar to CTLA-4, functions as a T-cell brake. PD-1 was more effective and positive results are being observed in several types of cancer, including lung cancer, renal cancer, lymphoma and melanoma. Nobel Prize in Physiology or Medicine 2017 was to Hall, Rosbash & Young "for their discoveries of molecular mechanisms controlling the circadian rhythm." Two proteins, TIM (timeliess) and PER (period) enter the cell nucleus where they block period genes activity. Such disturbance causes a number of skin diseases including psoriasis.

Materials / method: Nobel Prize in Physiology or Medicine 2013 for the machinery regulating vesicle traffic, a major transport system in our cells. A protein complex enables vesicles to dock and fuse with their target membranes, binding to each other like the two sides of a zipper. Vesicles are only allowed to release their contents when the cell signals to its neighbours a molecular machinery that responds to an influx of calcium ions and directs neighbour proteins rapidly to bind vesicles to the outer membrane of the nerve cell. The zipper opens up and signal substances are released.

Results: The plethora of new research focusing on molecular mechanisms compose a dynamic process of a matrix of signalling controls processes, delivered at specific times. It’s an elegant, almost symphonic interaction of cellular circadian clocks, the time dimension and the multi-dimensional intra- and inter- cellular signalling network. Identifying and reproducing signalling processes necessary to sustain health and the discrete intervals in which they have to be delivered is the ultimate goal of Signalling technology.

Conclusion: New discoveries in Quantum Physics principles underlying cellular signalling. Compromised signals from denatured proteins can be deleterious to this very specific process, while signals enhancing protein refolding can reinstate systemic functioning, restoring the flow of previously disrupted signalling pathways. In other cases, as in the case of CTLA-4 and PD-1 T-cell breaks, signalling that pauses certain mechanisms can be proven to be crucial both in Anti-aging Medicine and health status recovery.

My personal experience on lateral and lower lid wrinkles treatment in PRP

Objectives: The primary efficacy end point was the skin quality assessed by Visia on both groups at 12 weeks.
The second efficacy end point was the wrinkles reduction and the lifting effect assessed by the panel of two doctors and by patients from standardized digital photographs according to the SSAl and PSAl.
Validate the my personal technique of treatment of lateral and lower lid wrinkles with platelet rich plasma (Prp)

Introduction: Lateral (Crown’s feet) and lower lid wrinkles are among the first visible sign of skin ageing. Platelet rich plasma (Prp) has proved to be effective and safe on tissue regeneration in dermatology and reconstructive surgery. Despite this, guidelines on clinical indications and some technical aspects such its use (topical, intradermal, subdermal) or its frequency of treatments are lacking.
The goal of our prospective randomized study was to assess the efficacy of the topical Prp on reducing superficial eye wrinkles (lifting effect) and restoring the dermal matrix.

Materials / method: 50 women with moderate to severe periorbital wrinkles on a four-points facial wrinkles scale (FWS) were enrolled on the study. Patients were previously treated on the periorbital region by a single session of fractional CO2 laser and randomized in two homogeneous groups: 25 (group 1) applied 5 drops of topical Prp stored inside the poloxamer formulation twice a day for 12 weeks, and 25 (group 2) applied gentamicin and betamethasone twice a day for the first week and then hyaluronic acid gel for the following 11 weeks.

Results: Group 1: texture (5.8% Vs 14,3%; p<0.001), wrinkles (38% Vs 64%; p<0.001), pores (32,6% Vs 46,2%; p<0.001), PSAl (3.88 Vs 8.2, p <0.001) and SSAl (2.88 Vs 6.; p <0.001) improved significantly.
Group 2: texture (5.3% Vs 6,2%; p:NS), wrinkles (42% Vs 54%; p:NS), pores (34,2% Vs 49,6%; p:NS), PSAl (4,2 Vs 6,2 p:NS) and SSAl (3,1 Vs 5,6 p:NS) improved but failed to reach significative difference.

Conclusion: Our prospective study proves that Prp reduces superficial periorbital wrinkles and restore dermal matrix when used topically for a period of at least 12 weeks.

Is PRPF (Platelet Rich Plasma + b-FGF) therapy superior to fat injection?

Objectives: 1)Diagnose the decrease site of subcutaneous soft tissue due to aging.
2)Roughly predict the amount of tissue you want to inflate and collect enough blood.
3)Create PRPF by adding appropriate amount of b-FGF to PRP : PRP +b-FGF is PRPF.
4)Inject PRPF subcutaneously in the decrease site of soft tissue
5)Adipose tissue almost equivalent to the amount of PRPF injected is regenerated one month later.

Introduction: I have been providing a treatment of augmenting tissue volume by subcutaneously injecting platelet-rich plasma (PRP) supplemented with basic fibroblast growth factor (b-FGF),I named it PRPF, since 2007. After adjusting the platelet concentration and the b-FGF content, we established a unique protocol in 2013, and named it as PRPF therapy. Since then, we have achieved many cases with stable augmentation effects with small variability in outcomes and little adverse events.
To perform this method safely and effectively, it is important to know cytokine functions well.

Materials / method: PRPF is prepared by adding 10μg of b-FGF to 1ml of PRP purified to 1 to 2 million/ μl by the double spin method. It uses a b-FGF preparation made by a biotechnology called Fiblast, which is sold as a treatment for skin ulcer in Japan.
PRPF is a liquid in vitro and can be injected with a 30 gauge needle. And it immediately changes to gel in vivo.

Results: The gelled PRPF remained there for a period of time and produces approximately equal amounts of fat in one month. After that, there is no change in volume.Not only that, skin rejuvenation effect is also recognized in all cases. Even better, these effects last an average of 4 years.
In addition, since the fat produced by PRPF is the same as local fat, it has the advantage of not being affected by weight gain and loss.

Conclusion: From these advantages, I can conclude that PRPF therapy is superior to fat injection.

We have performed PRPF therapy in more than 300 patients between 2013 and 2018.
In this presentation, I describe the outline of PRPF therapy and report its efficacy and long-term effects from the viewpoint of cytokine functions.

Fat Juice: Fat Water, a new procedure that's more than nanofat

Objectives: Recently, following the discovery of the adipose tissue, the stem cell research is a growing field of plastic surgery and also regenerative medicine. Nano fat which is full of mesenchymal stem cells having trophic effect, immunomodulation, homing to injured tissue and ability to differentiate to other cell lineages appears to be promising in application for regenerative purposes.

Introduction: Besides nano fats are also used chronic wound treatment, autoimmune diseases, tissue engineering research and many indications of regenerative medicine. Fat Juice which is for adipose-tissue micronizing, homogenizing and sizing are a game and paradigm changer of regenerative aesthetic fat grafting and mechanical SVF surgical techniques.

Materials / method: This will be the best regenerative aesthetic fat grafting technigue that also advances the non-enzymatic stromal cell techniques beyond the nano fat or micro fat techniques. And this is called Fat Juice, Fat water.

Results: Fat Juice is excellent for aesthetic micro-fat transfer (micro-fat grafting) surgery and also regenerative medicine treatments Without enzymatic digestion, so quickly in about 15 to 20 minutes, lots of viable and nucleated SVFs can be isolated and harvested.

Mechanical dessagregation of fat: SVF gel for the wrinkle treatment

Objectives: Stromal vascular fraction gel (SVF-gel), a mechanically processed fat-derived product has gained a lot of attention lately.We will present the method of preparing SVF gel and its application to the face.

Introduction: SVF-gel is prepared by a process of centrifugation and intersyringe shifting and is particularly rich in SVF cells and native adipose extracellular matrix. SVF-gel injection is used alone or in combination with micro fat from March 2017 to February 2018.

Materials / method: This prospective, single-center study included 46 female patients who underwent SVF-gel grafting. Patient satisfaction were evaluated. Samples of transferred SVF-gel were harvested and examined histologically.

Results: All patients showed improvements in facial augmentation and contour. Patients in the SVF-gel group experienced mild postoperative swelling. Assessment of patient-rated satisfaction on a 5-point Likert scale found that 79.4% of patients in the SVF-gel group were satisfied or very satisfied with their outcomes. SVF-gel showed effective anti wrinkle effect.

Conclusion: Stromal vascular fraction gel is effective for wrinkle treatment, and may be superior to conventional treatment options of wrinkles.No complications have been noted in this series of patients.

Nanofat nano-needling: a mininvasive implementation for facial rejuvenation

Objectives: The indications for noninvasive facial rejuvenation are steadily increasing.Skin boosters are growing, thanks to safer and more suitable Hyaluronic acid (HA), a biodegradable product, has become the "gold standard" for fillers. Mesotherapy, microneedling, and chemical peels are minimally invasive techniques used to combat facial aging. Chemical peeling is one of the oldest methods of facial rejuvenation, with reported complications.

Introduction: The viability of adipose derived stem cells in the nano fat sample has been used in different indications as scars, infraorbital dark halos and perioral rhytides and neck rhytides

The microtraumatization caused by the micro needle leads to an activation of the healing cascade, activation of growth factors, which activate cell proliferation in the wound, increased synthesis and deposit of collagen - elastin complex with successive transformation of collagen III to collagenI, to neoangiogenesis and thus to accelerated skin remodelling

Materials / method: Nanofat Nanoneedling was conducted on 88 patients(7 males, 81 females) between the period of December 2016 to June 2019 using a custom made nanoneedle device. The nanoneedling procedure was repeated three intervals approximately 6 to 8 weeks apart. In some cases the nanofat sample was mixed with PRP sample to induce a synergistic effect

Results: Although no viable adipocytes were observed in the nanofat sample, Adipose- derived stem cells were still richly present in the nanofat sample giving the treatment a superior effect.Applications showed remarkable improvements in skin quality, skin pliability, patient and observer perception starting 2 months postoperatively. No infections, granulomas, scarring or hyperpigmentation or other unwanted side effects were observed.

Conclusion: Nanofat nanoneedling might become a new concept in facial rejuvenation as an autologous skin booster. In clinical situations, nanofat seems to be suitable for skin rejuvenation purposes.
