Systematization of lateral canthopexy to improve periorbital contour in blepharoplasties

Objectives: Describe the benefits of the association of blepharoplasty and lateral canthopexy in improving the periorbital contour and palpebromalar region.

Introduction: The eyelids and periorbital tissues undergo significant changes with aging and often constitute a major concern for those seeking facial rejuvenation. Understand periorbital anatomy, age-related changes of the lid-cheek junction and the variables controlling lower lid tone and position is mandatory. Blepharoplasty surgery offers excellent results in the rejuvenation of the orbital region. Modern blepharoplasty techniques associate lateral canthopexy procedures to improve the contour of the entire orbital region, including the palpebromalar transition and the lid-check junction.

Materials / method: Patients presenting moderate to severe flaccidity of the lower eyelid and marked palpebromalar transition were submitted to an associated procedure of blepharoplasty and lateral canthopexy. The canthopexy was performed by detachment of the orbicularis muscle from the lower eyelid to the inferior orbital rim and reposition with fixation in the superior orbital ridge. Lateral canthopexy aimed to reposition tissues of the periorbital and malar regions.

Results: The use of the lateral canthopexy procedure associated with blepharoplasty prevents surgical stigmas such as scleral show and round eye and allows the treatment of the contour of the periorbitopalpebral region. The delicate and precise detachment of the orbicularis muscle from the lower eyelid to the inferior orbital border and its repositioning with fixation in the superior orbital rim makes lateral canthopexy a simple and effective procedure. The combined procedure is effective in remodeling the contour of the lower eyelid and the palpebromalar junction.

Conclusion: The association of lateral canthopexy to blepharoplasties in adequately selected patients offers great improvemet to the eyelid, the periorbital region contour and to the middle third of the face. Patients associate this procedure to a superior result when compared to conventional blepharoplasty surgery, which guarantees high patient satisfaction as a final result.

Objectives: The facial arterial branches represent a danger zone for filler injections. This becomes even more complicated when injecting to lips as superior and inferior labial arteries have numerous small branches. Minimising the risk of intra-arterial injection of fillers in this dangerous zone must be taken to consideration prior to injection of lips. A good knowledge of anatomy is necessary and essential when injecting filler with needle. A blunt cannula tip minimizes intra-arteries (labial) injection and prevent from accidental injection to the arteries.

Introduction: Lip enhancement is one of the most popular procedures in facial aesthetic which is gaining more popularity among young and old generation. Carrying out such a procedure for cosmetic doctor is not free of risk. A good knowledge of anatomy and injection techniques are paramount to avoid complication like Injection into a blood vessel and subsequent tissue necrosis, Lumps and irregularities, asymmetry and last but not least Severe and prolonged swelling or bruising.
Treatment with a cannula has been found to result in significantly fewer bruises so is gaining popularity with patients and doctors

Materials / method: The sample population (66) age group was from 20 to 40 years old and just female. Exclusion criteria were as everted lips, incompetent lips, previously lip enhancement done, maximally arched lip, full gummy smile asymmetric smile.
The sample population were randomly divided to two main groups (cannula versus needle). We use the same HA filler for all the sample population. To reduce the bias, the same experience injector injected all the cases. We used 30G needle and 27G cannula. The injector did not asses the lips after injection. The lips were asses immediately and 2 weeks after injection w

Results: We had 4 cases of Haematoma in the needle group. This group also developed 10% of non-inflammatory nodules in 2 week review. We did not have any inflammatory nodules in both groups. Despite the swelling and oedema in the needle group, they had high level of satisfaction from the achievement. There were not any vascular compromise cases in both groups. The cannula group reported less bruising and reduced downtime than in comparison to the needle group. Conclusion: Treatment with a cannula has been found to result in significantly fewer bruises and edema. Short-term immediate post intervention edema comprising transient swelling is normal and common to all dermal fillers. The safety profile of cannulas is considered higher than needles as cannula can glide through the tissue without damaging them due to its blunt tip. Using multi-peptide growth factor serum in management of non-healing wounds in compromised patients

Objectives: The main objective is to restore skin barrier, function as well as aesthetics and pain relief in a variety of non-healing wounds in compromised patients in the shortest duration possible using only topical application of multi-peptide growth factor material.
Burn patients , diabetic foot and infected wounds that are resistant healing are the main focus to detect the efficacy and potency of topical application multi-peptide growth factor material in a duration of seven days

Introduction: *Wound healing is a complex process that include inflammation, formation of granulation tissue, re-epithelialization, matrix formation and remodeling.

*An injury can result in loss of the integrity of the skin, loss of the protective barrier against the external environment causing major disability, infection

*A successful normal process of wound healing depends on the complex signaling integration between growth factors, cytokines and chemokines that coordinate the cellular processes to restore the skin barrier and restore function; the final product is an unaesthetic scar formation.

Materials / method: *Compromised patients with different non-healing wounds ranging from first and second degree burns to post-operative open wounds and bed sores were referred for treatment.

*Saline irrigation (high pressure in case of wound infection)

*Topical application of multi-peptide growth factor serum daily for one week

*If the patient is bedridden or of extreme age (either too young or too old) the care giver is instructed to do the application

Results: *Pain index comparison before and after the first application is recorded (started as 10 dropping down to an average of 4 after the first application)

Conclusion: *Treatment with epidermal growth factor significantly decreased the average length of time needed for healing, improving the tensile strength of the wound and minimize the scar formation.

Endo-radiofrequency and plasma combination: tridimensional collagen stimulation

Objectives: The objective of this lecture is to demostrate the safety & the efficiency of thermal regulated endo-Radiofrequency & plasma energy combination in 10 patients treated.
The aim of the procedure is to obtain a tridimensional collagen stimulation in skins with medium laxity , in order to treat the superficial wrinkles and to obtain a deep skin thightening..

Introduction: The plasma energy has been used in aesthetic dermatology since many years specially for the peri-ocular area to perform an eyelid rejuvenation. Due to the tightening effect of the plasma energy, the immediate result was an eyelid lifting and an important neocollagenesis of the area treated. However the labio jugal & Jaw-line area need a deeper collagen stimulation & partial fat lipolisis to obtain a real endolifting contrasting the sagging of such weak zones. The combination of thermal regulated endo-radiofrequency was indipensable to obtain a deeper skin tinghtening in weak skin areas.

Materials / method: I have used a plasma device : Radiofrequency with plasma effect to treat the superficial wrinkles & moderate skin laxity : the technique was pulling small spots sublimating the superficial layers of the skin, living 1 mm between 2 spots, it allows a faster skin healing. Immediately after treating with plasma energy, i use a thermal regulated endo-radiofrequency using a thermal cannula setting an average temperature of 50 degrees C, the entry point of the cannula is on the lateral zygomatic area , tha cannula lenght is 7,5 mm 21G, the lasting time procedure is 15 minutes for each mid face.

Results: No oral medication was prescrived to the patients. A simple southing cream was recommended to apply on the superficial crusts. The patients was controlled 7 days, 1, 3 months after the procedure. The crust formed by the plasma energy fell spontaneously after 7 days, a smooth swelling was reported after the endo-radiofrequency lasting 2 days. Neither PIHP nor pain were reported by the patients. The maximum collagenesis and new organisation of the skin lines with lifting effect were obtained 3 months after the treatment.The patient satisfaction was very high: 9 patients on 10 were very satisfied

Conclusion: The thermal regulated endo-Radiofrequency combined with plasma energy is a very safe & efficient treatment to give a maximum collagenesis of the superficial & deep skin layers. The combination of both treatments give a tridimensional skin tightening, collagen stimulation & a new organization of the skin tension lines with lifting effect. The satisfaction degree of the patients is very high as the procedure is minimally invasive, painless with a minimum down-time.

BioLift - A new PMMA collagen biostimulation technique

Objectives: To present the PMMA BioLift technique for long-term collagen stimulation.

Introduction: The latest techniques of stimulating fibroblasts to produce collagen need a substrate like inert polymer - such as PMMA, to activate fibroblast cell division, neutrophil recruitment and histiocytes acting on the production of cytokines pro-inflammatories, new collagen fibers and dermal bundle remodeling in papillary end reticularis dermis. It is also proposed to stimulate the native steam cells at hair follicle bulge, with epidermal repair.

Materials / method: 10% PMMA was used in tissue suspension technique and tension vectors, in subcutaneous juxtadermis, at face and neck, with microcannula 22 gauge in 10 patients. Photograph register was done before the treatment and after 2 months to compare the results.
Performed the 400x microscopy of the particulate collagen biostimulators products available in the Brazilian market, such as polycaprolactone, polyactic acid and calcium hydroxyapatite and PMMA.

Results: An improved facial evidence of aging such as periocular lines, nasolabial fold, temporal skeletonization, eyebrow descent, perioral and mouth wrinkles, jow and neck laxity was seeing.
The 400x microscopy of PMMA, polycaprolactone, polylactic acid and calcium hydroxyapatite are very similar to 400x microscopy.

Conclusion: The facial and neck lifting and epidermis thickness was noticed at PMMA treatment. Minimal adverse effects like equimosis and transient edema were presents. This treatment was effective in the population studied, with minimal adverse effects, comparable to others collagen bioestimulators.

Body sculpting with combined 2mhz monopolar RF for fat reduction and bioelectric muscle stimulation for increased muscle mass

Introduction: Excess body fat is a common patient concern. Among the currently available treatment modalities, radiofrequency (RF) has been shown to reduce fat and improve skin laxity. Recently, the introduction of direct bioelectrical muscle stimulation and electro-magnetic muscle stimulation devices have offered an adjunct body sculpting procedure for patients who desire a more toned physique.
We present our clinical experiences using 2 MHZ monopolar RF-based device to reduce fat with a subset of patients receiving a series of bioelectric muscle stimulation to strengthen and tone the abdominal muscles.

Materials / method: A series of patients presenting with body mass indexes (BMI) ranging from 20 to 39 received a single treatment with a 2 MHz monopolar RF device. The areas treated were treated with a skin temperature of 44 to 45 degrees Celsius. A subset of patients received a series of 4 treatments with bioelectric muscle stimulation. Treatments were performed over two weeks, with follow-up at 12 weeks post final treatment. Patients were evaluated with photographs and ultrasound measurements.

Results: Fat reduction was achieved in as few as one treatment, with an average reduction of 24%. Patients also saw an improvement in skin texture and skin laxity. There was mild, transient erythema post-treatment that resolved within 3 hours. No edema or ecchymosis was observed post-treatment. The subset of patients who received the bioelectrical muscle stimulation treatments saw an average of 30% muscle mass increase and 25% average in fat reduction. Patients tolerated the treatments well, with no adverse events observed.

Conclusion: The 2 MHz monopolar RF device is safe and effective for the treatment of unwanted fat. Results are seen in as few as one treatment with an average of 24% reduction in the thickness of fat as measured by ultrasound. Bioelectric muscle stimulation is an effective treatment to increase muscle mass as well as strengthen, firm, and tone muscles with results observed after a series of 4 treatments.

Facial volumizing with HA dermal fillers and multilayer technique for male and female patients

Objectives: This study aimed to present a new approach for facial reshaping and jawline redefinition with the multilayer technique focusing in particular on male and female patients.

Introduction: The use of hyaluronic acid (HA) dermal fillers for the third midface reshaping and jawline definition is growing in popularity. Moreover, newer understanding about the anatomical fad pads and the causes of facial volume loss have led to a development of new treatment techniques with a gender different approach.

Materials / method: The Multilayer is an injective technique that takes into account both anatomy of cutaneous layers and gender facial features. This technique was performed using different HA dermal fillers that were injected in a specific depth based on their rheologic properties. Facial volume was restored starting from the deeper to the more superficial and dynamic layers with static and dynamic HA dermal fillers respectively.

Results: The Author explains the anatomy of the various facial compartments and shows the Multilayer technique with the results obtained from this new approach.

Conclusion: The Multilayer is a technique of aesthetic medicine to reshape the facial volume respecting the dynamism of the face. This is an anatomical approach that takes into account various cutaneous layers and gender differences. The practitioners should have a deep knowledge of facial anatomy to choose the ideal HA fillers based on the plane of injection and their rheology properties to achieve the desired results.

The cervicofacial lift under pure local anaesthesia diminishes the incidence of post-operative hematoma

Objectives: This presentation shows the benefit of the cervicofacial lift under pure local anesthesia. The preparation, the formula of the local anesthesia, and the surgical technique will be revealed.

Introduction: The cervicofacial lift is a frequently performed procedure in plastic surgery. It is the reference technique for facial rejuvenation and restoration of the oval form of the face and it is essential to treat excess skin. One of the most frequent complications of this procedure is the formation of haematomas. The aim of this article is to analyze the incidence of bleeding in the standard cervicofacial lift carried out under pure local anaesthesia.

Materials / method: The entire operation, including liposuction and tightening of the SMAS, can be done under local anaesthesia. A total of 1500 patients, who have undergone cervicofacial lift under pure local anaesthesia between November 1995 and January 2016, were included in the study.

Results: The incidence of early bleeding (in the first 15 days following the operation) was recorded and analyzed. Pre- intra and postoperative monitoring of the arterial blood pressure revealed stable pressure in the peri-operative period. The mean difference in blood pressure when comparing the intraoperative to the postoperative periods was 7 mmHg for the systolic BP [range from 3 - 25] and 4 mmHg [range from 2 - 12] for the diastolic BP, with lower mean values in the postoperative period. We identified nine bleeding events in 1500 patients (0.6%).

Conclusion: The cervicofacial lift under pure local anaesthesia does not radically modify the surgical technique. It allows better control of arterial blood pressure of patients throughout the surgical procedure and avoids fluctuations in blood pressure, which is one of the main causes of bleeding and haematoma formation. The infiltration of xylocaine adrenaline combined with the absence of hypotensive general anaesthesia diminishes the incidence of haematoma and ensures an early return home for the patients.

Background

In this study we have based our research on Botulinium toxin injection via targeted neuromuscular end plate zones, specifically in muscles with diffuse distribution of the latter. The muscular surface anatomical variety was also taken into consideration with thorough pre-injection examination of every subject.

Materials / method: Method

With consideration of the facial muscles anatomical variation and neuromuscular bundle distribution, we have applied the method of retrograde Botulinium toxin injection on a series of 10 patients on the right side of the face with the left side as a control side, it was injected in a regular perpendicular manner in order to conclude the efficacy of targeted motor end zone injection.

Using an auto-injecting syringe we have delivered 6 units per zone (Frontalis, glabellar lines of corrugator supercilii and orbicularis oculi crow’s feet)

Results: Results:

On control day 8 we have noticed a more prominent effect mainly on the level of the orbicularis oculi muscle and the corrugator supercilii muscle on 9 out of 10 patients.

9 patients out of 10 needed the delivery of an additional 6 units per zone on the control side’s glabellar lines and crow’s feet rhytids. Where 1 out of 10 patients needed the reinjection of the control side on the glabellar lines zone.

Conclusion: Conclusion:

The musculature of the face varies when it comes to neuromuscular plate distribution. Muscles with scattered distribution patterns show an optimal response to Botulinum toxin injections when delivered in a retrograde manner.

This has allowed us to achieve of optimal results while minimizing injection sites and hence pain, the use of lower dosage and hence treatment cost, as well as lowering the risk of antibody formation or unwanted product dissemination to the neighboring mimic muscles.

The effect of hydrogel dressing on recovery after facial needle mesotherapy with the use of autologous plasma concentrate

Objectives: The aim of the project was to assess the suitability of hydrogel dressings in revitalizing treatments using autologous platelet-rich plasma, in particular to reduce pain during the procedure, to reduce swelling, redness and postprocedural bruising.

Introduction: Hydrogels are defined as systems with at least two components, in which one component is a hydrophilic polymer, insoluble in water due to its chains being linked in a spatial network, whereas the other component is water. Hydrogels accelerate wound healing in various phases, accelerate autolytic wound cleansing, and reduce pain.

Materials / method: Thirty patients undergoing the PRP procedure were enrolled in the study. The patients included in the study were treated routinely, without above-standard procedures. After the procedure, the patients assessed pain during injection according to the NRS scale - separately for the test side, and separately for the control one. Both the researcher and the patient assessed the overall appearance of the skin, redness, swelling, bruising and bruises.

Results: The pain during the procedure was perceived as much less on the test side (p<0.05) according to the NRS scale. In the opinion of the researcher, the appearance of the test side was significantly better than the control one (3.6 vs 2.6, p <0.005). Both redness (2.3 vs 3.4, p <0.05) and edema (2.1 vs 3, p <0.05) were significantly lower.

Conclusion: The application of hydrogel dressings significantly improves comfort and reduces patient pain during mesotherapy using autologous plasma concentrate. In addition, it improves the appearance of the skin after the treatment, reducing edema and redness.

Lateral platform lifting (LPL)

Objectives: Delicate safe injection lifting represent an objective for all patients, and prolonged action with preserving the natural features of the face and minimizing complications are the main targets of the treatment. LPL lifting have immediately visible results on the face and neck are simultaneously involved.
A positive point is the stimulation and restructuring of the hypodermal-dermal layer, which leads to a marked improvement in the condition of the skin.
One of the LPL benefic effects are avoiding excessive fibrosing, compression and migration of fillers fragments.

Introduction: LPL is based on the division of the face into the medial and lateral parts - into the mobile and more motionless zone. The accumulation of surface fat in the medial part and resorption in many lateral areas are taken into account.
In the lateral zones of the face, neck and decollete, lateral platforms are formed, to which the skin is displaced by a special technique.
In mobile areas, skin contraction occurs by creating certain tension vectors to lateral platforms, as well as through the use of specific preparations based on hyaluronic acid: fibrosants and armirants.

Materials / method: LPL use partially stabilized HA drugs on lateral platforms (prefillers and skinboosters ) and non cross-linked fibrosing and reinforcing drugs in the medial area of the face (“liquid threads” or armirants) for relatively young patients.
With age, stabilization on the platforms should increase, and more stable preparations are used (prefillers with an increase in the percentage of cross-linked HA) due to the progression of surface fat resorption.

Results: The procedure is performed by a cannula for minimal external trauma, as well as for greater hypodermal trauma with the subsequent formation of controlled microfibrosis.
Patients are conditionally divided into 1-2 and 2-3 age groups according to the principle of engaging the deeper layers, if necessary, and using a small amount of cross-linked HA for age-related patients during the resorption of deep fat compartments.

Conclusion: Clinical results showed the absence of negative reactions, high efficiency for a long time (several months) and complete patient satisfaction. LPL represent an important alternative to have results without down time and to combine LPL with other procedure.

Rehabilitation in oncosexology

Objectives: Most prevalent sexual dysfunctions in cancer patients and survivors are loss of sexual desire, erectile dysfunction and pain to penetration (associated to vaginal atrophy). Sexual absenteeism is a frequent endpoint.
This work highlights the OncoSexology Clinic experience in a major Portuguese Cancer Center, focusing on multidisciplinary model, medical & psychological interventions, main surgical reconstructive treatments used in head & neck, breast, vulvar, penile cancer patients.

Introduction: Fifty per cent of cancer patients and survivors have sexual difficulties ensuing cancer and its treatment.
Most prevalent sexual dysfunctions in cancer patients and survivors are loss of sexual desire, erectile dysfunction and pain to penetration .
Even when cancer is cured, sexual problems tend to become chronic if not prevented or immediately treated. Sexual absenteeism is a frequent outcome.
To provide information and screening for sexual problems, to undertake treatment and rehabilitation programs for sexual dysfunctions are the goals of OncoSexology Clinic in Lisbon Cancer .

Materials / method: OncoSexology Clinic data analysis, 2 years of functioning 2017- 2019, 900 patients: organization, multidisciplinary team, medical data, main rehabilitation treatments.

Results: Most common dysfunctions were hypoactive desire, erectil dysfunction and dyspareunia (secondary to vaginal atrophy).
An intensive approach based on psychoeducation for both sexes plus 5-PDE inhibitors and alprostadil for men and vaginal lubricants, massage techniques and dilators for women, achieved a satisfactory outcome.
CO2 Laser vaginal rehabilitation might be a prospective alternative for many patients with vaginal atrophy.
Reconstrutive Surgery is now a frequent treatment used in Oncosexology rehabilitation mainly in head & neck, breast, vulvar and penile reconstruction.

Conclusion: OncoSexology is a cornerstone of comprehensive cancer care and allows patients and survivors to achieve a better quality of life.
A multidisciplinary approach empowers cancer patients to prevent and cope better with the negative consequences of disease and treatments in sexual life.
Besides medical and psychologic expertizing, breast and genital reconstructive surgery is actually an option for an increasing group of cancer patients and survivors with serious sexual impairments.

Modern trends in the correction of the age-related changes of the face

Objectives: Creation of the concept of an individual plan of aesthetic correction of age-related skin changes according gender, sex , age, morphological skintyp and psychotyp of the patient with a long-term effect.

Introduction: The modern history of beautification began with the discovery of an anti-age effect after the use of botulinum toxin by ophthalmologists Alistair and Jane Carruthers and the creation of the first synthetic hyaluronic acid filler under the brand Restyline. Over the past 40 years there has been a significant increase in the use of intradermal fillers for the correction of age-related changes in the face and body. The emergence of new digital technologies affects the appearance of new trends in aesthetic medicine; rigid and soft liberal and conservative.

Materials / method: For the classification of types of aging I.I. Kalgunenko in 1974 proposed a classification based on morphological changes in the skin and subcutaneous fat tissue of the face, which allows to definite 5 main types of aging: fine-wrinkled, deformed, combined, muscular and tired, which remains relevant today as well and allows to make individual plans correction of age-related changes for all types of skin.

Results: A new psycho-morphological classification is proposed that defines trends in aesthetic correction, are chosen individually, the frequency of repetition of correction methods, which allows to avoid the overdone effect and to achieve a natural effective correction of age-related skin changes with a long-term effect.

Conclusion: The proposed concept may be especially interesting in training young specialists in aesthetic medicine and its use in the daily practice of doctors to create individual communication with patients.

Self-acceptance after rhinoplasty at Vanest Plastic Surgery Center

Objectives: Self-acceptance is an individual’s acceptance of all the positive or negative attributes. Lack of self-acceptance may lead to underachievement, stress, anxiety, eating disorders and mental health problems. This study has determined the self-acceptance of the 33 aesthetic patients at least 6 months after they underwent Rhinoplasty (RP) at the Vanest Plastic Surgery Center (VPSC) in Erbil City, the capital of Iraqi Kurdistan Region during the period of 1st of January, 2017 to 1st of January, 2019. he study revealed that most respondents had desirable self-acceptance level.

Introduction: Self-acceptance is an individual’s acceptance of all the positive or negative attributes. Lack of self-acceptance may lead to underachievement, stress, anxiety, eating disorders and mental health problems. This study has determined the self-acceptance of the 33 aesthetic patients at least 6 months after they underwent Rhinoplasty (RP) at the Vanest Plastic Surgery Center (VPSC) in Erbil City, the capital of Iraqi Kurdistan Region during the period of 1st of January, 2017 to 1st of January, 2019.

Materials / method: The responses of the participants to the Likert scale had been analyzed through using of inferential statistics, SPSS (Version 22) and Microsoft Excel. The P value of ≤ 0.05 was considered as statistically significant. The alpha coefficient reliability test was 0.859. The mean age ± SD of the participants was 29.64 ± 9.39 ranging between 19 – 55 years.

Results: The study revealed that most respondents had desirable self-acceptance level. The highest mean score (4.3) had been recorded within the body acceptance factor. Significant correlations had been found between the body acceptance factor with gender and jobs of the patients, moreover between the factor “Feeling and believing in one's capacities” and the jobs of the studied sample.

Conclusion: Self acceptance level is high among the patients who underwent aesthetic Rhinoplasty surgery at the Vanest Plastic Surgery Center. Comparative self acceptance studies are recommended to assess the impact of RP on self acceptance of persons who undergo RP.

Periorbital filler's complications: how to avoid, how to recognize and how to treat

Objectives: to provide tips and suggestions on periorbital injections in order to avoid problems and promptly recognise any complications and treat them appropriately. Too often complications caused by fillers are not recognised and the patients are treated in many different ways without solving the problem

Introduction: Hyaluronic Acid (HA) has been used in the periorbital area for over 20 years. Appropriate selection and placement of the product is helpful to avoid complications. However, an incorrect or excessive application of the product is not always immediately evident and therefore complications will arise when patients request further repeat or top-up treatments at a later stage.

Materials / method: We are presenting 33 female patients requiring improvements of periorbital contour irregularities with a translucid mass under the skin. 12 patients denied they had received fillers in this area, 8 admitted they used “revitalisation” products, the remaining 15 admitted they had problems appearing after a number of HA injection. One patient underwent sinus surgery, a tooth extraction and a blefaroplasty without solving anything as the real problem had not been properly recognised. Ecography showede xtraneous matter in all the cases under review. All patients received Hyaluronidase's injection

Results: Seven patients required further hyaluronidase injections. One patient showed changes in skin quality with a depression requiring filler injection to achieve a good final result.
5 patients complained about wrinkles on the lower eyelid and were treated with laser resurfacing. All patients had good results clearly visible in the pictures and documented by ecography. All of them were very satisfied.

Conclusion: after hyaluronidases injections 5 patients complained about wrinkles: wrinkles of the lower eyelid should never be treated using filler injections, to avoid a bad result and complaints by the patient. 20 out of the 33 patients had not been diagnosed correctly and had been treated for sinusitis or inflammation. Always consider fillers whenever there is a “sausage” effect of the lower eyelid and a translucid mass under the skin. Good results for all patients, clearly visible in the pictures and documented by ecography. All patients were very satisfied.

How to win acne, rosacea with the help of botulinum toxin type A

Objectives: Clinic of Dr.Maria Shirshakova, Moscow - We used botulinum toxin type A (BoNTA) to optimize the complex treatment protocol for acne of various severity.

Introduction: Acne, also known as acne vulgaris, is a pleomorphic skin condition caused by multiple factors and associated with inflammatory processes in hair follicles and their sebaceous glands. Based on cholinergic action of BoNTA, potential pathogenetic justification of botulinum therapy in acne patients includes the inhibition of sebum production. BoNTA normalizes the processes of keratinocyte migration. Anti-inflammatory action of botulinum toxin is due to the blockade of certain mediators of inflammation, including substance P.

Materials / method: We analyzed 35 clinical cases: intradermal injections of BoNTA were used in the areas of sebaceous hypersecretion (T-zone — forehead, central part of the face, and cheeks) according to the following scheme: 1 U/cm2 or 0.5 U/0.5 cm2, not more than 6–8 U per zone (forehead or a cheek); the recommended dilution was 100 U/3–4 mL of saline. Topical anesthetic (EMLA cream) may be used 20–30 minutes before BoNTA injections.

Results: Clinical effects of BoNTA, such as reduced greasiness of the skin, pore contraction, improvement of atrophic scars, are seen from day 24 and persistent for 4–5 months. The treatment was well tolerated and all the patients showed the high compliance.

Conclusion: BoNTA can be therefore used in acne vulgaris in remission (anti-inflammatory effect) and in seborrhea oleosa to inhibit sebaceous secretion. BoNTA injections are not indicated in exacerbation of acne vulgaris with concomitant systemic and/or antibacterial therapy.

Managing impending skin necrosis of upper lip post nose tip filler injection

Objectives: After being treated many times, patients can experience perception drift where they want more and more treatments. A woman, 27y.o has history of filler injections on her nose, experienced ischemic event onset on her upper lip following her last injection. This case proves that joining areas can be involved through the retrograde flow. The 1st pulses of Hyaluronidase are very important to reduce the major filler load, because patient didn’t come on 2nd day.

Introduction: Even a dosage as low as 0,2cc Belotero Intense at the tip of the nose, columella and the alar base can cause persistent numbness on the upper lip which can be an early indication of intravascular complication, even without any detectable blanching. This initially vague symptom can rapidly develop into significant tissue necrosis; livedo reticularis, slow capillary refill, pain, swelling, blistering on her upper vermillion border and small pustules on the skin of the upper lip. This proves the retrograde flow of the HA filler injection from the columellar A backdown to the superior labial A.

Materials / method: In this case I injected a total of 2280 units in 3 pulses of Hyaluronidase, following the New High Dose Pulsed Hyaluronidase Protocol for HA Filler Vascular Adverse Events in her first visit within 2 days of the ischemic event onset. 4 hours after the 3rd pulse had been given, the capillary refill was back to 90% normal, the skin color had become hyperemia, pain sensation reduced. Only the small pustules remained.

Results: The next day, the number and size of the pustules greatly increase eventhough most of the other symptoms had abated. The patient had no further Hyaluronidase treatment although she did visit dermatologist to treat the pustules. 8 days later she updated me; the pustules were 70% recovered and had started to dry and after 30 days her skin had returned to normal despite some tiny, almost imperceptible scars left on her upper lip.

Conclusion: This proves retrograde involvement of the upper lip and the initial course of Hyaluronidase injection is absolutely crucial in the prevention of necrosis. There are two competing theories about what causes the pustules, the first is bacterial infection while the second suggests the connection between intravascular embolism of HA Filler injection with the cutaneus involvement.

Fat compartments of the submental region: anatomic study and guidelines for treatment (from cryolipolysis to surgery)

Objectives: 1: Understand relevant anatomy of submental fat and double chin
2: Formulate a tailored approach to each individual patient
3: Manage submental fat with diverse treatments involving cryolipolysis, injection lipolysis and surgery.

Introduction: Treatment of submental fat includes criolipolysis, injection lipolysis, liposuction and excisional surgery. A better undestanding of relevant anatomy may allow better and more constant results and explain some cases of unexpected failure.

Materials / method: Cadaveric dissections were performed with the injection of blue dye in the subcutaneous and subplatismal plane. Results were used as a guide for providing refinements and an algorithm for treatment of excess submental fat (double chin).

Results: A previously not described midline partition of fat compartments of the submental region was found. It may be responsible for poor results in some cases of cryolipolysis. Relationships between subdermal, subplatismal and deep submental fat were assessed and correlated to different treatment options including criolipolysis, injection lipolysis, liposuction and excisional surgery.

Conclusion: Partition of fat compartments of the submental region and the different contribution of three levels of fat compartments may be responsible for poor results in some cases of criolipolysis and liposuction and has to be clearly understood for either medical and surgical approach to submental fat.

Complication treatment and management of thread applications

Objectives: How can a practitioner understand, treat and manage thread related problems and complications…

Introduction: 7 Years experience with thread applications and last 5 years study in country as complication management author. My aim is to explain complications treatment and management are crucial for all practitioners.

Materials / method: Cases from all over the Turkey and World.
Surveillance, threat and manage of patients.

Results: Treatment and management results were shared with colleagues and patients, both group were satisfied for results.

Conclusion: Thread applications are a new, uncontrolled, less experience & less training area for medical aesthetics. For that reason, most of patients suffered and faced similar problems. Complication treatment and management should be added to the training programs and experienced doctors should manage complications.

A corrective cosmetic make-up significantly improves quality of life, as well as skin dryness and tautness

Objectives: to assess the impact of a corrective cosmetic (CC) on QOL, as well as on skin dryness and tautness.

Introduction: Facial flaws from skin indications may impact quality of life (QOL). Specific high coverage cosmetics may improve QOL and the quality of the skin.

Materials / method: An international observational study was conducted on subjects with visible facial flaws that would benefit from corrective Make--up. Investigators collected data about demographics, QOL using the SKINDEX-16 Scale, skin dryness and tautness, subject and investigator satisfaction, cosmetic acceptability and tolerance at baseline and after 4 to 6 weeks of daily use.

Results: 1840 subjects participated; mean age was 31.5±11.1 and 95% were women. Acne accounted for 48.9% of skin conditions.81.3% of the subjects had at least 10% of their face affected, the condition was rated disfiguring by 45.3% of the subjects; 45.0% and 44.4%, respectively had neither taut nor dry skin on baseline.. SKINDEX-16 scores had significantly (p<0.0001) improved for all parameters, so did skin dryness and tautness at the end of study. Subject and investigator satisfaction, cosmetic acceptability were high or very high andlocal tolerance good or very good.

Conclusion: Corrective cosmetics can significantly improve the QOL of subjects with significant facial flaws, improve skin tautness and dryness, and are well tolerated.

A novel approach: color doppler ultrasound guided HA filler injection

Objectives: In this paper, I describe a method of real-time ultrasound imaging of the face while injecting hyaluronic acid (HA), referring illustratively to temple and lateral forehead augmentation using HA fillers.

Introduction: Several recently published articles describe the examination of facial vessels prior to these injections.
To my knowledge, this paper is the first to describe the real-time ultrasound examination during filler application.
Ultrasound is a non-invasive imaging tool that, in this case, enables us to bring all fillers, vessels, and needles or cannulas into sight very well.
Using the temple as illustrative region, I describe the technique, also showing videos and pictures to visibly elucidate the process.

Materials / method: Using an ultrasound device with a 8MHz probe, a 23g cannula is superficially inserted into the temple and forehead region. Arteries, veins, and cannula are made visible by using color doppler ultrasound.
The plane in which the cannula is inserted is located and the injection can start slowly. One can see the injected filler, as well as the vessels, live while the fluid is entering the temple region.
Using the color doppler ultrasound, the HA filler is visible as a hypoechoic mass. The procedure has been filmed, and will be shown in the presentation.

Results: In my experience, using real-time ultrasound while injecting, can be considered a safe and satisfying technique: at no point does one need to fear injecting HA into an artery. The vessels, cannula, and HA are all clearly visible.
It is possible to learn and develop the technique in a reasonable amount of time.

Conclusion: After having learnt and mastered the technique, color doppler ultrasound can be used very well in HA filler treatment.
It is possible to learn this technique within reasonable time, and considering that ultrasound devices are becoming more and more affordable, it can now be used in special cases, such as after facelifts or in treating anxious patients- Especially, since cases of vessel occlusion are increasingly reported in the media.
Real-time ultrasound is also of interest for injectors, trying to get even a deeper knowledge of the patients’ individual anatomy.

Combination of non-invasive multisource radiofrequency (RF) with pulsed-vacuum and RF microneedles for cellulite treatment

Objectives: To test the safety and efficacy of a combined protocol of multisource RF with RF Microneedles for cellulite appearance improvement.

Introduction: Cellulite is a skin condition affecting predominantly post-pubertal women (prevalence is around 85%-98%). It causes topographic changes to the skin surface as a result of alternations in dermal and subcutaneous tissues, leading to “orange peel” like skin. A consecutive application of different treatment modalities is a frequent approach to improve clinical efficacy when treating cellulite, such as the application of noninvasive radiofrequency (RF) immediately followed by RF Microneedles (RFM).

Materials / method: 7 female patients, who exhibited cellulite completed a course of 5 combined sessions using two RF based treatment modalities: (1) a noninvasive, pulsed vacuum, multisource RF applicator designed for body areas and (2) Microneedling RF treatment. The RFM treatment was performed immediately after the noninvasive treatment. An area size of 15X15 cm was treated. Power for multisource RF was 85W. Surface temperature maintained at least 40°C. RFM parameters were 17W, pulse width of 170msec and 5 mm needles depth. All patients were photographed before each treatment, and at follow-up visits.

Results: Patients reported the treatment was comfortable and mostly painless. Mild edema and erythema were observed up to 24 hours post treatment. No adverse events were reported during and after the treatments. Patients’ before and after photos, were graded according to Hexsel Dal'Forno and Hexsel Celluite severity scale. Depth of depressions, skin surface and laxity parameters were improved according to this scale.

Conclusion: Cellulite is a big concern of many female patients, who are looking for treatment alternatives to surgery with minimal downtime. Combined protocol of multisource RF with RF Microneedles was safe and effective for cellulite appearance improvement. The combined treatment enable to enhance the result by focusing long term thermal heating which increase blood circulation and dermal coagulation at the septa’s area for cellulite improvement.

PDO threads on body treatment

Objectives: With a rapid growth in the technologies surrounding the field of aesthetic dermatology, high praise has been awarded to a lift treatment using Polydioxanone threads in recent years. Commonly known as PDO threads, these highly absorbable threads have the power to stimulate collagen growth, which results in the tightening of sagging skin tissue. Consequently, PDO threads have been widely sought by patients looking to combat skin aging and flaccidity through a procedure that is minimally invasive, and yet highly effective.
In their conception a few decades ago in South Korea, PDO threads were primarily used in acupuncture. Throughout the years, they were adopted as a tool for facial rejuvenation, and more recently, they’ve also been useful in diminishing the appearance of cellulite, localized fat pockets, and skin laxity.

Introduction: The efficacy of PDO threads for the beautification and rejuvenation of face, and body has been broadly applicable. Additionally, this treatment can be offered with ease and safety to patients, who typically report not feeling any pain nor discomfort, but who manifest great satisfaction with their results. The author is one of the pioneers in the emergence of the PDO thread market in Brazil, she frequently administers trainings for physicians who are looking to learn her techniques. She will share about her experience using PDO threads in aesthetic medicine.

Hiding the translocated excess skin in facial thread lifting

Objectives: Translocated skin excess is mostly placed over the zygomatic arch and expected to be contracted in facial thread-lifting. However, this might prevent the patient’s socialization. The main objective of this study is hiding the excess skin in a less visible area to provide early recovery and faster socialization.

Introduction: Degeneration of the elastic fibers within the skin, decrease in the quality and quantity of the collagen, redistribution, and atrophy of the facial fat pads and resorption of the bony skeleton cause ptosis of the facial soft tissues by the gravitational force. Many surgical procedures have been defined to reverse these changes. However, many patients hesitate to undergo such face-lifting surgeries due to having general anaesthesia, long operation time, potential complications such as facial paralysis, long postoperative recovery period, and evident changes.

Materials / method: Basically, absorbable materials have the potential for unpredictable effects during the resorption process, however, non-absorbable materials have more predictable and stable effects. We use flexible silicone coated thread because of their more predictable outcomes. There are many laying techniques where the midpoint of the thread is placed over the zygomatic arch. In order to evenly distribute and hide this skin excess in a less visible area, the midpoint is inserted behind the ear.

Results: Placing the midpoint of the thread behind the ear equally distributes the skin excess and provides early recovery and faster socialization of the patient. This technique demonstrates a higher patient satisfaction rate compared to the other laying techniques.

Conclusion: In conclusion, the optimal composition of the thread materials, laying techniques and retightening methods, the long-term histomorphometric changes of the skin need to be investigated in order to have more reliable, stable, and safe results.

Correction of epicanthal fold by injection of filler

Objectives: We tried to correct epicanthal fold by injectiing high viscosity filler as points deep into a part of brow-tip line from nasofrontal angle to medial of eyebrows. Filler columns based on the bone push skin externally and upward and leads to flattening of folded skin Which makes epicanthal fold. Excluding a case with high elasticity skin, the result satisfied clients although changes were subtle. It should be considered that visibility of punctum is main criterion and punctum width is about 2mm or less.

Introduction: An epicanthal fold is a skin fold of the upper eyelid covering medial canthus an punctum ( partially or completely). It is often seen as a normal finding but some people want it to be removed which needs surgical procedures with inevitable scars. A small change is enough to make punctum more visible and significant change in the face.

Materials / method: We tried to correct epicanthal fold by injecting filler into some points of a part of brow-tip line of each side from nasofrontal angle to medial part of the eyebrow (particularly radix). Theoretically columns of filler which are injected vertically push medial end of eyebrows upward and the skin of radix anteriorlly. The slided skin pulls and flattens (partially or completely) folded skin so epicanthal folds decrease or disappear. This method was done for few cases considering all ethicall aspects.
High viscosity filler was injected and in order to avoid intra arterial injec

Results: Deep radix cases with tight skin are the best cases for this method. Deep radix lets you make higher columns of filler that not only flattens the fold more but corrects deep radix as well.
Patient satisfaction in our cases was 5 to 10 of 10 excluding one case of zero. Cases of high elasticity and expandable skin do not respond to this method.
Subtle changes in epicanthal fold leads to significant correction because width of punctum is about 2mm or less and a minimal increase in visibility of this organ is significant and satisfies the client.

Conclusion: The same as any method this injection has pros and cons and final result can not be as high as surgical intervention but it must be considered that almost all clients for correction of epicanthal fold are young girls and they prefer smaller change to better one but with a scar on each side if the nose and other potential irreversible side effects of the surgery.
