Threads and collagen: minimal invasion, maximum result

Objectives: Fuzzy facial contours during aging are an aesthetic problem in men and women. The aesthetic appeal of the face is made up of the quality of the skin, the strength of the ligaments, which ensure the correct oval and the ability of the skin to resist deformations. With age, face ligaments are loosened, as a result, the tissues undergo gravitational ptosis and rush down. Therefore, working with the ligaments in restoring the correct face oval is primary. The second step is to "fill" the dermal matrix with mechanical strength, which collagen can provide.

Introduction: The aim of the work was to compare the results of using mono therapy with injectable collagen and the complex effects of collagen and threads.
For comparison, we took groups of patients with approximately the same manifestations of aging according to Glogau (2 - 3), one of whom received a mono course of injectable collagen to improve the oval of the face, and the other - a combination of a course of injectable collagen and the introduction of lifting threads

Materials / method: Both groups of patients received Injectable collagen at a dose of 70 mg of lyophilized hydrolyzate of type 1 collagen diluted in 5 ml of 0/9% NaCl solution. Injections were made intradermally and at the sites of projection of the main ligaments of the face. The course consisted of 3 procedures with an interval of 2 weeks. After a course of injected collagen, the second group of patients underwent the introduction of lifting threads in order to move the ptosis tissues and fix them in the ligaments and rapper zones. The result was assessed 4 weeks after the end of all procedures using the GAIS.

Results: When comparing the results, patients receiving the combined use of injectable collagen and threads showed the best results in restoring facial contours, improving the quality of the skin and its mechanical properties (GAIS 2 - 3). Patients who received collagen injectable mono therapy showed an improvement in skin quality and mechanical properties, but at the same time they were outperformed by patients in the combination therapy group (GAIS 1).

Conclusion: The combined use of injectable collagen and suture insertion is a minimally invasive protocol that significantly improves the clarity of the facial contour and the overall aesthetic appearance of the skin.

Threads for browlift - New approach: A.I.L.A. technique

Objectives: upgrading the outcomes of thread lifting especially in the peri-orbital area is an important step in order to accept the possibility of using threads as alternative for surgery or to combine both : surgery and threads , to achieve the best results

Introduction: brow lift is one of the most requested aesthetic treatments , with or without upper eyelid blepharoplasty, starting from the medical indication in cases of severe ptosis , ending with the trend “ cat eye , fox eye, etc “, we know different non invasive, minimal invasive , and surgical techniques used in order to correct the position of the eyebrow.

Materials / method: inverted lifting approach is the latest update of eyebrow lift using “double sharp – double needle” threads , with a new entry point and the simplest technique of implantation ,
We applied the new approach either as a simple brow lift or during upper lid blepharoplasty with direct anchoring of the subcutaneous layers across the open wound.
We suggest 3 different variations of our technique with different levels of stability and durability.

Results: we applied AILA technique to more than 25 cases, 35% combining thread lifting with blepharoplasty, 65% simple brow light lift. Analysis of results, comparisons and durability show that: AILA technique is a successful update or upgrade of brow lifting using threads

Conclusion: A.I.L.A. technique is a serious alternative of brow lifting with excellent results and feedback, safety and comfort depend on the ability of practitioner to respect anatomy , we observed not only a nice elevation of the brow, but also an improvement of the lateral cantus.
Suggesting 3 different variations will expand the area of selection between patients, it will allow practitioners to achieve desired final results.

Using ultrasound to diagnose thread complications

Objectives: The main objective of this lecture is to review the sonographic aspects of the main thread complications in order to help physicians choose the best treatment.

Introduction: The popularity of threads lifts has been rising in the last few years. The most popular threads are polydioxanone (PDO), polycaprolactone, and poly-L-lactic (PLA). Although threads have been applied for more than 30 years with relatively few complications, the lack of experience and skills of those who are starting to apply the threads are probably responsible for a relatively increase in the rate of complications. Ultrasound is a noninvasive, dynamic technique, sensitive and accurate for skin examination, which can help locate the threads and study its complications.

Materials / method: Ultrasonographic images of thread lifts and its complications where compiled and reviewed retrospectively by the authors.

Results: The sonographic aspect of thread complications such as dimpling, abscess, inflammation, misplacement, extrusion and filler interaction were systematically reviewed and described by the authors as a series of cases.

Conclusion: Ultrasound is of utmost importance to help physicians locate the thread lifts and recognize its possible complications to help treating them.

Reasons for complications in thread lifting procedures: who is at fault, doctor or the patient?

Objectives: Objectives for this study are to provide doctors with necessary information to avoid possible complications, evaluate and prevent complications, and to manage complications in case of such occurrence. In this article, we review and evaluate several cases involving complications resulting from barbed suture.

Introduction: Thread lifting procedures has gained popularity over the past two decades, due to its numerous advantages such as mini-invasiveness, cost effectiveness, ease of performance, immediate results, and long-standing effects.There are many different types of threads represented in the global market, with variable composition, types, methods, and techniques. Along with the frequency of the procedures, different types of complications occur that are caused by incorrect techniques, incorrect techniques, incomplete examination of the patients, as well as by the patients ignoring the recommendations.

Materials / method: The thread insertion methods should be performed in the proper layer such as subcutaneous fatty tissue, since this the area where the barbed thread is inserted and where its fixation occurs, after which with a simple motion reposition is performed, on another hand such layer is the safest area since any big branches of arteries, veins and nerves aren’t placed there.The product which is used to insert the threads should be atraumatic, simple to use and easy to guide. The methods for reposition of soft tissues should be simple and considered with anatomical biomechanics of the face.

Results: patients are reporting different types of complications, including simple skin retraction, dimpling, asymmetry, contouring, migration, infection, abscess and even necrosis in few cases. In this study we evaluate all these cases with complications and as well as with adverse effects. We analyze ways for the prevention and management of such cases.

Conclusion: Thread lifting techniques are becoming more common and hence, complication rates may be higher than initially reported. We believe that, by being informed and aware of the human anatomy, aestheticians and surgeons who perform these procedures will be able to correctly evaluate individual cases, choose safe methods and products, following the aseptic and antiseptic rules, and will achieve effective results.For those patients who follow all the recommend. of the doctors, and in case of identifying any type of changes report and communicate with their doctor willsignificantly decrease the risks.

Get your best result with multi-thread type combination

Objectives: To get the younger appearance, and more beautiful look

Introduction: A thread lift is a procedure that uses a dissolvable suture to tighten and lift the skin. Barbed suture aims
to lift and sculpt the shape of the face. The question is why some procedure has a great result, and some
others don't. There are many factors, Right assessment (choosing the good candidate, choosing the right
case), Set Strategy (choosing the combination of the Threads, choosing the Combination with other
modalities), and Use of The Combination Technique (Deep & Superficial for different purposes).

Materials / method: many types of Threads, such as Multidirectional Barbs, Bidirectional Barbs, Spiral
Spring Threads, etc.

Results: The Multidirectional Barbs
have a few functions like Volumizing (if we go at the right deep layer of the Subcutaneous tissue),
Armoring, and also Lifting, while Bidirectional has a superior effect on Lifting than Multidirectional.
On the other side, for The Dynamic area of the face (like nasolabial and marionette line), they need a
specific flexible thread-like Spiral Spring Thread to minimize the line and volumize it.

Conclusion: In order to get maximum results, we also have to keep in mind that each area of the face is connected to
the other. We are not only aiming for the younger appearance, but we also aim for a more beautiful look
for our patients. It should be noted that the function of the threads nowadays, is not only for lifting, but
also for contouring a few parts of the face like the nose, eyebrow, and jawline

Injectable combination treatments for men featuring absorbable threads

Objectives: To recognize that the male clientele is a growing market and that treatments tailor-fit for men are in demand.
To understand aesthetic behaviours and goals of male patients.
To learn various treatment combinations of injectables for men.
To learn the how threadlifting can be part of the tools to obtain male aesthetic goals.

Introduction: In recent times, the male aesthetic market has grown remarkably. Men want an enhancement of male features but desire natural-looking results. In order to achieve this, a combination of non-surgical aesthetic procedures is key.

Materials / method: In this lecture, various injectable treatments and a combination of these will be presented. These includes toxin, fillers, lipolysis and threads. Short video clips of procedures will be shown as well as subsequent before and after photos.

Results: Depending on the age, shape/morphologic features of the face, and goals of the patients, some or all of these maybe combined to have an excellent outcome while maintaining a natural look. Although the upper and mid face are common treatment areas, focus should also be placed on the lower face. Toxins will address dynamic wrinkles and bulky masseter muscles. Fillers can reshape and give structure to the jawline. Lipolysis can reduce superficial fat, usually the jowl and submental fat. Threads can tighten the skin of the neck, elevate the mid/lower face as well as reposition fat

Conclusion: To address the demands of the growing male market, doctors should be cognizant of the aesthetic goals of men. Treatment strategies should be designed in consideration of cultural differences as well as personal preferences. The key to success is to be able to tailor-fit combination treatments to achieve the aesthetic goals. The incorporation of threads in the aesthetic toolbox completes the armament of procedures for an enhanced and natural outcome.

Choosing the best technique according to age to treat the facial sagging using PLLA/PCL threads: a mother/daughter comparison and one-year follow up.

Objectives: My objective is to demonstrate the need to use different techniques for different levels of sagging.

Introduction: I selected two patients that presented similar/familiar facial appearance and complained about facial sagging and treated them using only PLLA/PCL threads.

Materials / method: I treated these two patients using PLLA/PCL threads , linear patterns but different vectors of traction and number of threads in way to achieve the best results.

Results: I have a one year follow up to compare the response of the same treatment but with different levels of sagging and different ages. The patients were advised not to perform any kind of other minimally invasive procedures during this period.

Conclusion: When treating different levels of sagging in similar face shape we need to choose appropriate techniques and numbers of threads to achieve the best and long lasting result.

Threads for lip beautification

Objectives: For correction of perioral wrinkles and creating of some kind of border of the lips’ commissure we suggest to use of co-called dynamic spring PCL/PLLA threads with dynamic memory. This spring threads without cogs and barbs were indicated for improving of the quality of skin and creating some volume in the area of implantation.

Introduction: The concept of so-called dynamic thread lifting involves the use of threads with dynamic memory in the form of a spring, which after stretching, return to their position. This does not apply to threads simply twisted on a needle or cannula, but on the threads, which are precisely springing with a dynamic memory return. They belong to the so-called dynamic group thread correction, that is, they can be used in areas of intense physical (muscle) activity like as perioral and lip area.

Materials / method: For these cases we observed the 4 dissections of female cadavers in the Medical Faculty Department of Anatomy and Pathology of Comenius University in Bratislava.
Were treated 30 patients with the presented method and 2 histological analysis were provided.

Results: We have some anatomical nuances for safe implantation of the threads in a spring form. It was confirmed not only by anatomical researches, but also with histological tests. All of patients were satisfied with the treatment, for some of them the lip augmentation was provided without previous complication of migration of the hyaluronic acid filler into skin lip area.

Conclusion: The implantation of the threads in the spring form with dynamic memory are based on some anatomical nuances and rules for safe implantation like as layer of implantation and right choice of the area of implantation.
Good choice of material has great deal on the satisfaction of the procedure and good and long-term result.

Neck rejuvenation: liposuction plus threads

Objectives: Aging processes in the face and neck and the request for its correction are one of the most common reasons for middle-aged people to visit plastic surgeons. Aging in the face and neck, although have the same etiologies in most items, different clinical manifestations of them appear. For example, the presence of platysma band, double chin, fading of aesthetic contours, and changing the angles are some of the issues that belong to the neck.

Introduction: Various surgical and non-surgical techniques have been introduced to correct the aging processes of the neck, and most of the surgical techniques are combined with facial rejuvenation, which, while having a good effect, sometimes also contains complications. In contrast, non-surgical techniques are associated with very minor side effects, short recovery period, and reasonable cost, so there has been able to be favored.

Materials / method: This study aimed to introduce a new and effective technique for neck rejuvenation as a combination of liposuction to reduce subcutaneous fat content and angulation, as well as lift using Thread for skin contraction. For the patients, the intended markings were designed in a sitting position first, and after the prep and drape, under local anesthesia with the injection of tumescent solution in the anterior space of the neck, neck liposuction was performed through three small incisions under the lobes of the ears and submental liposuction respectively, and so on neck lift was done in each side b

Results: Comparing the photographs of people before the intervention and three months later, the results of this technique based on the criteria of theVisual Analog Scale (VAS) and neck wrinkle rating scale using paired T-test analysis with a p-value > 0.05 were significant.

Conclusion: In this way, this technique did not have any complications, it achieved the full satisfaction of the patients, and thus it can be introduced as a suitable method for non-surgical neck rejuvenation.

The role of thread lifting methods in aesthetic gynecology

Objectives: This study aims to provide necessary information for doctors to understand how thread lifting methods can rejuvenate intimate zone, and understand the methods how it perform. To understand that thread lifting methods is modern approach that can be alternation of surgical approaches.

Introduction: in modern medicine now its very demanded to treat and rejuvenate intimate area. Lot of methods are presented nowedays for such kind of treatments: lasers, dermal fillers, fat grafting, PRP and etc. to achieve desirable result we need to combine at least two or more methods, but regarding threads lifting methods which is mini invasive can provide safe and effective results immediately after, and lasts long time.

Materials / method: Thread materials that are using for intimate area treatment can be various, a lot of type of threads can be used depends the treatment zones. thread methods should be appropriate for intimate zone, such as composition, barbs configuration and the most important is the technique that gives us safety and effectiveness.
threads can be used for vaginal rejuvenation, tightening and narrowing, reconstruction of perineal area, elevation and rejuvenation of labia majora.
the most interesting is that with threads can be treat functional problem such as stress urinary incontinence.

Results: providing threads in intimate zone is safe, effective, mini-invasive procedure with short rehabilitation period, without incision and scars, need only local anaesthesia.
if we chose right thread material, right technique, proper indication the results will be satisfied.

Conclusion: Thread lifting methods for intimate zone is most popular treatment, due to minimal downtime and high effectiveness that is alternation of surgical approaches gives the patient comfort, self confidence. After thread lifting procedures raise quality of sexual life but also quality of life in both of gender.

Non-surgical Rhinoplasty using Polydioxanone Threads in Asian Patients

Objectives: To determine the safety and efficacy of using Polydioxanone (PDO) threads in non-surgical rhinoplasty for Asian patients.

Introduction: The nose occupies the center of the face; the size, shape and angles of the nose dictates the profile and balance of the face, in relationship with the other facial features. Asians have lower and flatter nasal dorsum, a rounded or bulbous tip, weak lower lateral cartilages, rounded nostrils, and a short columella. Non-surgical rhinoplasty using threads, is one of the most common and popular procedures in Asia. The goal of thread rhinoplasty is to increase the height of the nasal dorsum, and lengthen the columella to improve the nasal tip angle, and nasolabial angle.

Materials / method: This study examined 10 Southeast Asian patients, aged 18 years and above, with no previous procedures done on the nose. Digital photographs were taken before and immediately after the procedure. Using these photos, the following were measured: nasofrontal angle, nasolabial angle, nasal dorsum length, nasal tip projection and width of nasal bridge.

Results: The average decrease in nasofrontal angle is 14 degrees, ranging 10 to 20 degrees. The average increase of nasolabial angle is 12 degrees, ranging from 5-26 degrees. The average decrease in nasal dorsum length is 2.20cm, ranging from 0-4cm. The average increase in nasal tip projection or columellar lengthening is 2.8cm, ranging from 2-4cm. The average decrease in the width of nasal bridge is 2.6cm, ranging from 2-4cm. There were reports of swelling and bruising in which lasted for an average of 5 days. There were no infection, migration, and rejection reported.

Conclusion: It is safe and effective to use polydioxanone threads as a non-surgical option in aesthetically improving Asian noses.
